임종기 환자의 자발적 죽음 수용을 위한 시론 : 『華嚴經』離世間品 中心으로

세계 암 보고서에 따르면 년 이후 전 세계적으로 매년 천만 명 이상이 암으로 사망할 것이라고 보도되었다. 그로 인하여 말기 암 환자 수도 늘어날 추세다. 따라서 많은 환자들이 임종기에 접어들었을 때 죽음에 대한 두려움 및 공포심으로 여명 동안 불행한 시간을 보내는 것이 다반사이다. 자신의 죽어감 및 죽음을 받아들인다는 것은 매우 어려운 일이다. 죽음에 대한 준비 보다는 살아가는데 시간을 더욱 많이 관심을 기울였기 때문이다. 인간은 삶에서 죽어감 그리고 죽음을 결코 피할 수 없다. 특히 임종기 환자가 느끼는 감정과 고통은 매우 주관적이며 그들은 죽음을 오롯이 수용하기란 매우 어려운 일이다. 필자는 죽어가는 환자와의 만남을 통하여 문답으로써 그들이 스스로 죽음을 수용할 수 있도록 적극적인 방법에 대하여 많은 고민을 하게 되었다. 본고를 통하여 임종환자의 심리상태와 문답을 통한 치유기제를 발견하는데 의의를 둔다. 따라서 죽음 수용 상황 건립을 통한 죽음 수용에 대한 태도 변화를 화엄경의 이세간품을 근거로 호스피스 환자 사례를 들어 죽음에 대한 바른 이해를 통하여 수용에 이르기까지의 여정을 밝히고자 한다. 그들이 지금 이 순간에 머물러 고통을 직면하여 삶과 죽음이 결코 둘이 아님을 자각할 수 있는 여정이 되기를 기원하는 바이다. According to the World Cancer Report by WHO, more than ten million people will die of cancer yearly after 2020, which means the number of terminal cancer patients will also be on the rise. Many patients suffer despondency out of fear of death. It is strenuous for people to accept death and dying, because they have divested more time and energy toward living rather than preparing for their death. This is especially true for terminal patients, and their emotions and sufferings are highly subjective. The significance of this paper can be found in its search for a dynamic way for dying patients to accept death, through acknowledgement of their psychological states, question-and-answer sessions and the resultant discovery of healing mechanisms. Through the employment of the chapter “Detachment from the World (離世間品)” in the Avatamasaka Sutra (大方廣佛華嚴經), this paper illustrates case studies of hospice patients who have undergone change in their perception of death - how it helped them in their journey to understand and accept death - and aspires to help more patients ground themselves in the here and now, face their pain, and come to recognize that life and death are one.
