      • 폐암환자를 위한 웹기반 교육프로그램 개발 및 평가

        The purpose of this study were to develop an web-based education program for Lung cancer patients and to test its effects on patients' self-care knowledge, compliance to medical regimen, nutrition status and pain.<br/> The program was developed by the following process: first, Lung cancer patients demand on the web-based program was investigated. and second, the program was developed with the help of various reference books and then validation of experts group. last, educations effects on the patients is evaluated and compared the differences in self-care knowledge, compliance to medical regimen, nutrition status and pain between on experimental group and a control group on before discharge 1day and 3weeks after. SPSS/Win 11.0 program was used for data analysis. It was proven with χ2 test and t-test. and Pearson Correlation coefficient, and Chronbach's alpha coefficient were done for the reliability of measuring instruments. <br/> 1. The summary of the Program development is as follows. <br/> The program is based on patients' questionnaire and reference material and is made for users friendly. Not only Bigger font size and bright colors but also illustrations or pictures were adopted to help enhance patients' understanding.<br/> 2. The summary of the study results is as follows.<br/> 1) Compared with control group, the web-based educated experimental group showed a statistical significant difference on self-care knowledge, Especially disease, radiation treatment, medication & analgesics, chemotherapy side effect. but there was no significant difference in the field of chemotherapy, in the fields of operation, diet & general knowledge.<br/> 2) Compared with control group, the web-based educated experimental group showed a statistical significant difference on compliance to medical regimen, especially in the field of follow up care, everyday life, diet. but there was no significant difference in the field of medication, exercise.<br/> 3) Compared with control group, web-based educated experimental group showed no significant difference in nutrition status, but partially significant difference in body weight.<br/> 4) Compared with control group, the web-based educated experimental group showed no significant difference in pain level.<br/> 5) The significantly positive correalation self-care knowledge with the compliance to medical regimen.<br/> 6) Users satisfaction with the web-based education program of the contents quality, the level of recommendation to others, content layout, medical information quality, but interesting got a low mark.

        여고생의 체육수업 재미 요인이 학업 스트레스 해소와 학교생활 만족에 미치는 영향

        The purpose of this study was to investigate the relationship among fun factors of physical education (PE), academic stress, and school life satisfaction. A total of 165 1st and 2nd-year female high school students participated in this current study. To collect the data, three questionnaires measuring three were used. The correlation coefficient(r), frequency analysis, and multiple regression were performed by using SPSS 22.0. The analysis founds the followings. First, the fun factor was negatively correlated with academic stress and was positively correlated with school life satisfaction. Also, academic stress and school life satisfaction were negatively correlated. Second, friendship socialization showed a significant impact on five elements (grade stress, class stress, study stress, career stress, interpersonal satisfaction). Third, physical fitness had a significant impact on the school rules and the satisfaction of special activities. Also, social support was significantly influenced by friendship satisfaction. The results of this study suggest that the fun factors of PE influences female high school students’ academic stress and school life satisfaction positively. Based on the results, it is suggested that the fun factor in PE should be increased to reduce female high school students’ academic stress and enhance their satisfaction with school life.

        폐암환자 증상관리를 위한 전화상담 가이드라인 개발 예비 조사

        Purpose: This study was conducted to develop the telecommunication guideline that was possible to make the immediate,logical decisions and to evaluate the possibility of the use by verifying it from the specialists’ group. Methods: Telecommunication guideline was designed with selected six symptoms which were pain, dyspnea, nausea/vomiting, diarrhea,fever, and cough based on the results of literature. Results: This study showed that the patients understood well about the contents of the questions, examination, and education. Also, 85% of them fulfilled the guidelines of the adjustment and more of them were satisfied with the results shown to all of them compared to the previous telecommunication. And 95%of them replied that the information from the telecommunication was more useful than any other information sources related to the information of cancer and self management that were used previously. Conclusion: The telecommunication was considered to complement the weakness during the time the cancer patients spent at home after the treatments. Also,it was expected to be a role as an immediate and comprehensive method of decision making and a passage for information supply which was considered the specialty of individual patient related to an inquiry of the self management. 본 연구는 폐암 환자가 경험하는 증상 중 전화 상담 문의가많은 증상을 파악하여 이에 대한 즉각적, 논리적 의사결정이 가능한 증상관리 전화상담 가이드라인 개발을 위한 예비 조사연구다. 예비 가이드라인을 개발하고 이를 전문가 그룹을 통해 검증함으로써 그 활용 가능성을 평가하였다. 문헌고찰 및 설문조사 결과를 바탕으로 호흡곤란, 오심/구토, 설사, 발열, 기침의 5가지 증상을 추출하여 이에 대하여 예비 폐암환자 증상관리를 위한 전화상담 가이드라인을 개발하였고, 이후 전문가 집단의 타당도 검증 및 수정 보완 과정을 거쳐 최종 전화상담 가이드라인을 확정하여, 3월 1일부터 15일까지 15일간 환자에게 적용하여 평가하였다. 1) 항암화학요법 및 방사선 치료를 받는 폐암환자의 전화 다문의 증상은 통증(83.3%), 발열(75.0%), 호흡곤란(70.8%), 오심/구토(70.8%), 설사(62.5%), 기침(50.0%) 순이었다. 2) 다문의 증상 중 관리지침이 개발되어 있는 통증을 제외한5개 증상에 대하여 Telephone Nursing Practice and Symptom Management Guidelines6) 및 Telephone Triage6)를 기초로 예비 전화상담 가이드라인을 도출하였고, 이후 전문가 타당도 검증을 거쳐 수정 보완하여 각 증상별로 정의, 사정(일반적 정보, 증상정보), 지침적용(A: 즉시 응급실 방문, B: 빠른 외래 방문, C: 예정일 방문), 교육내용으로 분류하여 최종 전화상담 가이드라인을 개발하였다. 최종 전화상담 가이드라인은 호흡곤란은 사정 11항목, 지침적용 6항목, 교육내용 11항목으로 하였으며, 오심/구토는 사정13항목, 지침적용 11항목, 교육내용 8항목으로 하였고, 설사는사정 16항목, 지침적용 12항목, 교육내용 9항목으로 하였고, 발열은 사정 12항목, 지침적용 9항목, 교육내용 7항목으로 하였다. 마지막으로 기침은 사정 10항목, 지침적용 9항목, 교육내용4항목으로 구성하였다. 3) 전화상담 가이드라인 평가는 6개 항목, 즉 이해정도, 원하는 정보 획득 여부, 제공된 정보의 정확도/신뢰도, 전반적 만족정도, 재이용 의사, 기존 정보와 비교 시 전화상담의 도움 정도로 분석하였으며 평가 항목 중 가장 높은 평균 평점을 보인 항목은‘전반적 만족정도’와‘재이용 의사’로 각 평점 평균은 4.80±.40이었다. 4) 전화 상담 가이드라인 적용 대상자는 20명이었고, 증상별로 보면 오심/구토 7명, 발열 6명, 호흡곤란 4명, 기침 3명, 설사 0명이었다. 권고를 받은 지침에서‘원하는 정보획득 여부’에 따라 이행정도에 차이가 있었으며(t=4.400, p<.001), 또한‘제공된 정보의 정확도/신뢰도’에 따라 이행정도(t=-2.400,p<.05)에 유의한 차이가 있었다. 5) 전화 상담결과‘즉시 응급실 방문’이 요구되었던 대상자는 4명이었으며, 이들은 모두 권고를 받은 지침을 이행하여 응급실을 방문하였고, 이 중 3명은 입원 지시되었으며, 1명은 특수 영상검사가 필요하여 이를 시행하고 관련 약제 처방 후 귀가조치 되었다. 결론적으로 전화상담 가이드라인은 암환자가 치료 후 가정에서 보내는 시간 동안의 취약함을 보완해 줄 뿐 아니라 증상관리 문의와 관련하여 개별 환자의 특수성이 고려된 즉각적이고이해하기 쉬운 의사결정도구와 정보제공 통로로 활용할 수 있을 것으로 기대된다.
