        In EVAR procedure using long time C-arm, we studied exposure dose reduction and effeciency through measuring surgical staff's ESD by installing lead curtain operating table next to. The height 3 the spot (50cm, 100cm, 150cm) dose was measured on 2 locations for 600sec in the X-ray radiation considering the surgical staff's movement. To install the curtains, we compared before and after the dose. As a result, it can confirm that dose of the installation former of 50cm height and after is reduced about 75% and 91% in 2 locations. In 100cm height, the reduction of the dose was a bit confirmed. There as to dose value, measured on 150cm height the installation former and after was nearly no change. This research examined the exposure dose about the radiation of the surgical staff during EVAR procedure in which the operation time is the long time. It was implemented in the object that it reduces the radiation exposure.       • 간섬유화 검사 시 탄성도의 재현성과 체질량지수의 상관관계에 대한 연구

        목적 : 간섬유화가 진행되면, 간경변증(Liver Cirrhosis)을 발생시켜 여러 합병증을 유발하며, 간암으로 진행 될 위험성이 매우 높다. 따라서 간 섬유화의 평가는 환자의 예후 및 치료 방향 결정에 매우 중요하다. 간섬유화 검사는 검증된 여러 장점들이 있는 반면, 비만인 환자 검사 시 재현성이 낮은 제한점이 존재한다. 이에 본 연구에서는 간섬유화 검사 시 탄성도의 재현성과 체질량지수 간 연관성을 분석하여 환자의 체형이 검사의 재현성에 미치는 영향을 알아보고자 하였다. 대상 및 방법 : 2020년 6월부터 12월까지 서울아산병원 초음파실에 내원하여 간섬유화 검사를 시행한 환자 중 조건에 부합하는 대상의 검사 결과를 분석하였다. 연구대상을 선정한 방법으로는 세계보건기구(WHO)의 체질량지수 분류 기준을 적용하였고, 이를 바탕으로 환자를 정상체중, 과체중, 비만에 해당하는 세 그룹으로 선정한 후 그 검사 결과를 연구하였다. 각 그룹은 100명으로 하였고, 총 300명을 대상으로 하였다. 재현성을 평가한 방법으로는 간섬유화 검사 시 획득한 탄성도의 유효측정값을 이용하여 모든 대상의 SD(Standard Deviation)값을 구하고, 각 그룹별로 비교해 보았다. 통계학적 검정은 SPSS 27 (IBM Statistics)을 이용하였고, 일원배치 분산분석의 Welch 검정을 대입하였다(P<0.05). 결과 : 각 그룹별로 유효측정값의 SD값을 구하고, SD값들의 평균 수치를 얻은 결과 정상체중의 경우 0.39, 과체중 0.57, 비만에서 0.84로 비만 체형에서 가장 높았다. 그 분포도 또한 SD값이 0.5이하인 경우가 정상체중 73%, 과체중 49%, 비만에서는 17%로 나타났으며, 1을 넘긴 검사 결과가 평균체중 그룹에서는 없었던 반면, 과체중에서 9%, 비만에서는 21%이었다. 총 300명의 검사 결과를 분석한 결과 정상체중에서 탄성도의 재현성이 가장 높고, 비만에서 가장 낮았다. 즉 체질량지수가 높아질수록 재현성은 떨어지는 상호 반비례의 상관관계를 탄성도의 SD값고 그 분포도를 통해 확인할 수 있었다. Purpose : when liver fibrosis progresses, cirrhosis occurs, causing various complications, and there is a very high risk of developing liver cancer. Therefore, the evaluation of liver fibrosis is very important in determining the patient’s prognosis and treatment direction. While Fibroscan® has several proven advantages, there is a limitation of low reproducibility when testing obese patients. Therefore, in this study, by analyzing, the relationship between the reproducibility of elasticity and body mass index during Fibroscan®, we tried to find out the effect of patients body shape on test reproducibility. Materials and Methods : The examination results were analyzed for patients who visited the ultrasound department of Asan Medical Center from June to December 2020 and performed Fibroscan®. As a method of selecting the study subjects, the World Health Organization’s body mass index classification criteria were applied, and based on this, patients were selected into three groups: normal weight, overweight, and obesity, and the results of the examination were studied. Each group consisted of 100 people, and a total of 300 people were included. As a method of evaluating the reproducibility, Standard Deviation values of all subjects were calculated using the effective measurement value of the elasticity obtained during the Fibroscan®, and compared for each group. The statistical test was performed using SPSS 27 (IBM statistics), and the Welch’s test of one-way ANOVA was substituted(P<0.05). Results : As a result of obtaining the SD value of the effective measurement value for each group and obtaining the average value of the SD values, it was found to be 0.39 for normal weight, 0.57 for overweight, 0.84 for obesity, and the highest in obesity. The distribution also showed that when the SD value was less than 0.5, 73% of normal weight, 49% of overweight, and 17% of obesity were found, while test results exceeding 1 were not found in the average weight group, but 9% of overweight and 21% of obesity.

        In this research, the optimal reduction of linear multivariable systems with white noise are studied. Performance indices are minimized by gradient technique and optimally reduced systems are obtained by digitalcomputer simulation. By changing initial values, the original systems of the fifth order are reduced to the systems of the fourth, third and second order, while preserving the properties of the original systems, which are stable, controllable and observable. The mean-errors of the output response in steady state are 0.97%, 6.36% and 2.52% for the reduced systems of fourth, third and second order, respectively. Therefore, by preserving the initial value with dominant eigenvalue, high order systems can be reduced to desired low order systems by optimal reduction.
