        증례 : 감염 ; 근육근막염을 동반한 Taguchi 항원형 중증 쯔쯔가무시병 1예

        쯔쯔가무시병은 O. tsutsugamushi에 의한 내피세포 파괴로 전신적인 혈관염이 발생하여 고열, 두통, 근육통, 가피 형성, 림프절 종대가 발생하는 급성 열성 질환으로 다양한 임상 증상을 보일 수 있으며 중증 합병증을 야기하여 사망에 이를 수 있는 병이다. 조기에 적절한 치료는 완치 가능하며 빠른 진단이 중요하다. 현재까지 폐렴이나 뇌수막염의 동반은 보고가 되어 왔으나 근육근막염까지 복합적으로 동반된 중증의 쯔쯔가무시병은 보고된 적이 없다. 이에 저자들은 폐렴, 뇌수막염과 함께 근육근막염이 동반된 중증의 Taguchi 항원형 쯔쯔가무시병을 적절히 진단하고 치료하여 완치한 경험을 보고하는 바이다. Scrub typhus is an acute febrile illness with focal or general vasculitis that is caused by Orientia tsutsugamushi. The spectrum of clinical severity ranges from mild to severe with potentially fatal complications including acute renal failure, gastrointestinal bleeding, meningitis, myocarditis, and pneumonia. However, myofascitis with scrub typhus has not been reported to date. We report a case of pneumonia, meningitis, and myofascitis in a 56-year-old male patient with scrub typhus due to the Taguchi strain of O. tsutsugamushi. (Korean J Med 2014;86:253-257)

        Graves` disease following subacute thyroiditis is uncommon. Some patients in these cases showed positive for thyroid antibody only transiently in the resolving phase. However, Graves` disease can rarely be caused by the presence of antibodies after subacute thyroiditis, although the pathophysiology of this is unclear. A 40-year-old woman presented with anterior neck pain and swallowing difficulty. Thyroid function testing showed reduced thyroid-stimulating hormone (TSH) and elevated free thyroxine levels. A thyroid scan revealed decreased uptake in the bilateral thyroid gland. The patient was initially diagnosed with subacute thyroiditis and treated with steroids. Five months later, thyroid function testing showed recurrent hyperthyroidism with positive conversion of TSH receptor antibody, indicating Graves` disease. Since then, she needed the long-term methimazole treatment. In summary, we herein report a case of Graves` disease occurring after subacute thyroiditis. (Korean J Med 2015,89:225-228)

        Background: Acute gastric anisakiasis is referred as an incidental infestation of third-stage Anisakis spp. larvae in the gastrointestinal tract, while consuming contaminated raw seafood. It is diagnosed by direct visualization of the larvae on upper endoscopy and treated by extraction. It has been suggested that Anisakis spp. is one of the causative agents for food hypersensitivity, called gastroallergic anisakiasis. Case History: We encountered a 47-year-old malepatient who visited our emergency department due to whole body hives, angioedema on the face, stomachache, nausea and shortness of breath that had occurred a few hours after raw seafood ingestion. At presentation, he was diagnosed with food hypersensitivity resulting anaphylaxis and treated by epinephrine, prednisolone and antihistamine. Most of his symptoms improved except for mild urticaria and epigastric pain. One week later, he underwent upper endoscopy for his epigastric pain, and two anisakislarvae were removed. Thereafter, he had no symptoms. Serum specific immunoglobulin E to anisakis antigen, but not to common seafood was detected. Results: A case of anaphylaxis caused by anisakis infestation was confirmed. Conclusion: Anaphylaxis can be caused by Anisakis spp. infestated in gastrointestinal tract. Gastroallergic anisakiasis should be suspected in case of anaphylaxis after raw seafood ingestion.
