        Prognostic Value of Hematological Markers in Head and Neck Cancer Patients With a History of Hematological Malignancy

        Background and Objectives Head and neck cancer can occur as a second primary tumorin patients with a history of hematological malignancy, and the pathological features of thoseare more aggressive than de novo primary head and neck cancer. However, research related tothe prognostic value of hematological markers in the head and neck cancer patients with a his-tory of hematological malignancy is scarce, so we performed this study. Subjects and Method We reviewed a total of 89 head and neck cancer patients with hemato-logical malignancy diagnosed at a single tertiary hospital between 1997 and 2021, and enrolled29. The analyzed hematological parameters included pre- and post-treatment hemoglobin, he-matocrit, lactate dehydrogenase, absolute neturophil count (ANC), absolute platelet count, ab-solute leukocyte count, neutrophil-to-lymphocyte ratio, and platelet-to-lymphocyte ratio. Results Hematological malignancy was diagnosed almost 62.14±96.10 months before headand neck cancer. The most common hematological malignancy was acute myeloid leukemia. The overall survival (OS) rate was lower in patients with second primary tumor than in thosewith de novo primary head and neck cancer. The cox-proportional hazard ratio analysis showedthat the post-treatment absolute neutrophil count was significantly correlated with disease-freesurvival but not with OS. Conclusion The post-treatment hematological profile, particularly ANC, can be considereda useful prognostic marker in the head and neck cancer patients with a history of hematologi-cal malignancy.

        Age- and Sex-Specific Trends in Oral Cavity Cancer Cancers in a Single Institution From 2000 to 2019 in Korea

        Background and Objectives Head and neck cancer (HNC) represents a significant publichealth challenge worldwide, impacting mortality rates and economic resources. This studyaimed to analyze the age- and sex-specific trends in the prevalence and prognosis of HNCsover two decades. Subjects and Method A retrospective analysis was conducted utilizing data collected dur-ing the period of 2000 to 2019 from the hospital’s Head and Neck Cancer Center. Results We observed that, of the 4167 HNC patients analyzed, the proportion of female withthis cancer has remained unchanged over the past two decades. However, the proportion ofpatients under 40 years of age with oral cavity cancer has significantly increased. We observeda significant improvement in the 3-year overall survival (OS) in HNCs during 2010-2019 c om -pared to 2000-2009. This improvement was particularly pronounced in male, whereas no sig-nificant change was observed in female. Notably, the 3-year OS has significantly improvedover the past decade for advanced-stage (III & IV) patients. Conclusion The incidence of oral cavity cancer has increased among patients under 40years of age over the past two decades. Additionally, the prognosis of oral cavity cancer pa-tients notably improved, particularly in male under 40 years of age.

        사르코이드증에 의한 일측 성대 마비 1예

        Sarcoidosis is a multisystem granulomatous disease of unknown etiology. Vocal fold paralysis secondary to sarcoidosis is extremely rare but it can develop as a result of compressive lymphadenopathy, granulomatous infiltration, and neural involvement. We report the case of a 56-year-old woman who presented with unilateral vocal fold paralysis and enlarged supraclavicular lymph nodes. Computed tomography of the neck revealed multiple, enlarged, and matted lymph nodes at the cervical level of IV. An ultrasound-guided core needle biopsy of the lymph node was performed, and a histopathological diagnosis of sarcoidosis was made by validating the presence of noncaseating granuloma. After implementation of steroid therapy, the patient exhibited immediate recovery from vocal fold paralysis. Although an extremely rare disease, sarcoidosis should be included in the differential diagnosis of vocal fold paralysis. Accurate diagnosis and prompt steroid treatment may reduce the morbidity of patients with vocal fold paralysis secondary to sarcoidosis.

        쇄골상동맥피판을 이용한 두경부 영역의 재건

        Reconstruction of various defects following head and neck surgery is crucial for restoringboth function and aesthetics. Over the years, various flap techniques have been developed tomeet these demands. The supraclavicular artery flap is a versatile option readily accessiblewithin the same surgical field during head and neck surgery. Its similarity in texture, color,and contour to the facial region makes it particularly useful for reconstructing defects in theneck and lower face. Additionally, its thin and pliable nature, coupled with a moderate vol-ume, renders it suitable for a wide range of applications, including intraoral defects, pharyn-geal defects, and contour refinement in facial surgery. In this paper, we present a method forreconstructing defects in the head and neck region using the supraclavicular artery flap, anddiscuss its utility, advantages, and limitations.
