Background and Objectives: A plunging ranula represents a mucus escape reaction occuring because of the disruption of the sublingual salivary gland. It is commonly a condition of young adults, although the reported age range is 6-43 years. There is said to be a slightly female preponderance of about 1.9:1(F:M). Surgical incision has been considered the definite treatment, but sometimes complete excision is very difficult. Non-surgical treatment of these lesions has been attempted, but the results have not been satisfactory. In this study, we present our experiences with picibanil (OK-432) sclerotherapy for a plunging ranula. Materials and Methods: We retrospectively reviewed 41 patients who have undergone sclerotherapy with picibanil for plunging ranula. Information was collected on age, sex, a number of injection, post-sclerotherapy side effect and outcome of treatment. Results: 17 patients (41.5%) showed a complete response, and 20 patients (48.7%) showed inter-mediate response. No response was seen in 4 patients (9.8%). As a side effect of intracystic OK-432 injection therapy, fever (26.8%) and pain(39.0%) were observed. However, fever and pain disappeared after several days in all cases. Conclusion: The results are showing that OK-432 injection is an effective and safe treatment for plunging ranula.

Traumatic pseudoaneurysm of the intracavernous internal carotid artery(ICA) is extremely rare, but it is life threatening condition because of massive recurrent epistaxis. Unfortunately, the prompt diagnosis and treatment of this disease are frequently delayed due to its rarity and variable latent period, so this disease is regarded as a challenge to clinicians. Optimal therapy for this diseases demands rapid suspicion for it and is essential in order to give the best functional outcome with minimizing its morbidity and mortality. The authors present a case of male patient with traumatic pseudoaneurysm of intracavernous ICA accompanying severe epistaxis. This patient was a 37-year-old male with unilateral blindness and recurrent massive epistaxis after suffering trauma to head. Computed tomography, MRI and carotid artery angiogram showed pseudoaneurysm of intracavernous ICA with sphenoid bone fracture. The patient was effectively managed with occlusion of the pseudoaneurysmal circulation by endovascular interventional embolization technique utilizing mechanically detachable tungsten coils.

This paper studied the cause of the deterioration of the four 3-Arch tunnels built in mid-1990. The common deteriorations of the four 3-Arch tunnels were longitudinal cracks, leakage and efflorescence at the same parts of lining concrete. Three fourths of 3-Arch tunnels, there was high percentage longitudinal cracks and a quarter was low frequency about longitudinal cracks. So the material reviewed to find out the differences between two groups in construction process and analysis was conducted such as non-destructive testing, precise visual survey and safety evaluation of one tunnel which had bad ground condition As the result, the tunnels were safety condition and the primary deterioration occurred during the construction process, namely, problems arrangement of rebar and the effects of the blast at middle tunnel. 3-Arch터널 시공초기인 1990년대 중반에 시공된 4개소의 3-Arch터널을 대상으로 열화발생여부를 조사하였으며, 그 결과 4개 터널 모두 중앙기둥측 상, 하선 아치부에서 발생형태가 유사한 종방향 균열이 조사되었다. 4개 터널 줄 3개소의 터널에서는 종방향 균열의 발생빈도가 높고 1개의 터널에서는 종방향 균열의 발생빈도가 낮아 이 2개군의 시공과정상 차이점을 분석하였으며 비파괴시험 및 정밀육안조사, 지반조건이 취약한 1개의 터널을 대상으로 안전성평가를 실시하여 이러한 열화들의 원인과 이에 따른 안전성여부를 판단하였다. 그 결과 안전성은 확보되었으며 중앙터널 기둥측 상, 하선아치부에 발생한 열화는 중앙터널굴착에 앞서 선시공되는 상, 하선 콘크리트라이닝의 철근배근불량과 이후의 시공단계인 중앙터널 굴착시의 영향이 큰 것으로 분석되었다.
