        Induction of apoptotic cell death by betulin in multidrug-resistant human renal carcinoma cells

        <P>Betulin, a triterpene from the bark of various species of birch tree, has various biological effects, including antiviral, antifungal and anticancer activities. The aim of the present study was to elucidate the mechanisms underlying the apoptotic effect of betulin in RCC4 multidrug-resistant human renal carcinoma cells. To evaluate anticancer activity, we performed cell viability and caspase activity assays, a proteome profiler array and western blot analysis in RCC4 cells. Betulin significantly decreased RCC4 cell viability in a time- and concentration-dependent manner. Betulin activated caspase family proteins, including caspase-3, -7, -8 and -9, and increased the expression of apoptosis-related proteins, including PARP and Bcl-2 family members. In an apoptosis array, betulin activated the tumour necrosis factor-related apoptosis-inducing ligand (TRAIL) receptors TRAIL R1/DR4 and R2/DR5, and tumour necrosis factor receptor 1 (TNFR1), suggesting that betulin treatment leads to induction of apoptosis through both intrinsic and extrinsic apoptosis pathways in RCC4 cells. Notably, betulin significantly enhanced cytotoxicity and PARP cleavage in etoposide-treated RCC4 cells, and downregulated the expression of multidrug resistance protein 1 (MDR1). Taken together, our findings suggest that the anticancer effects of betulin involve induction of apoptosis and sensitisation of RCC4 cells, providing potentially useful information applicable to the use of betulin in renal cancer treatment.</P>

        Protective effect of Jageum-Jung on chlorpyrifos-induced acute toxicity in ICR mice

        Chlorpyrifos (CPF) is one of the most heavily used organophosphate pesticides and is useful as an insecticide drug. However, CPF also causes toxic effects in nontarget organisms, including humans and animals. Jageum-Jung (JGJ) is a traditional oriental medicine, composed of five specific herbs with antioxidant and hepatoprotective properties, used for detoxification. In the present study, highly concentrated CPF was orally administrated to male Institute of Cancer Research mice to produce acute toxicity, and the protective effects of JGJ administration were investigated through statistical analysis of changes in body and organ weights and serum biochemical parameters. JGJ caused body and organ weights to recover and reduced the levels of serum biochemical parameters indicative of liver damage, such as glutamic oxalate transaminase, glutamic pyruvate transaminase, alkaline phosphatase, lactic dehydrogenase, urea, glucose, total cholesterol, and triglyceride, that had been increased by CPF treatment. Our results demonstrated that JGJ ameliorates the effects of acute chlorpyrifos-induced toxicity. Therefore, JGJ has the potential to be used as a traditional medicine to alleviate insecticide toxicity.

      • Sesquiterpene Furan Compound CJ-01, a Novel Chitin Synthase 2 Inhibitor from <i>Chloranthus japonicus</i> S<small>IEB</small>.

        <P>A novel sesquiterpene furan compound CJ-01 was isolated from the methanol extract of the whole plant of <I>Chloranthus japonicus</I> S<SMALL>IEB</SMALL>. by monitoring the inhibitory activity of chitin synthase 2 from <I>Saccharomyces cerevisiae</I>. Based on spectroscopic analysis, the structure of compound CJ-01 was determined as 3,4,8a-trimethyl-4a,7,8,8a-tetrahydro-4a-naphto[2,3-b]furan-9-one. The compound inhibited chitin synthase 2 of <I>Saccharomyces cerevisiae</I> in a dose-dependent manner with an IC<SUB>50</SUB> of 39.6 μg/ml, whereas it exhibited no inhibitory activities against chitin synthase 1 and 3 of <I>S. cerevisiae</I> up to 280 μg/ml. CJ-01 has 1.7-fold stronger inhibitory activity than polyoxin D (IC<SUB>50</SUB>=70 μg/ml), a well-known chitin synthase inhibitor. These results indicate that the compound is a specific inhibitor of chitin synthase 2 from <I>S. cerevisiae</I>. In addition, CJ-01 showed antifungal activities against various human and phytopathogenic fungi. Therefore, the compound might be an interesting lead to develop effective antifungal agents.</P>

        Robotic single-port transumbilical total hysterectomy: a pilot study

        <P><B>Objective</B></P><P>To evaluate the feasibility of robotic single-port transumbilical total hysterectomy using a home-made surgical glove port system.</P><P><B>Methods</B></P><P>We retrospectively reviewed the medical records of patients who underwent robotic single-port transumbilical total hysterectomy between January 2010 and July 2010. All surgical procedures were performed through a single 3-4-cm umbilical incision, with a multi-channel system consisting of a wound retractor, a surgical glove, and two 10/12-mm and two 8 mm trocars.</P><P><B>Results</B></P><P>Seven patients were treated with robotic single-port transumbilical total hysterectomy. Procedures included total hysterectomy due to benign gynecological disease (n=5), extra-fascial hysterectomy due to carcinoma <I>in situ</I> of the cervix (n=1), and radical hysterectomy due to cervical cancer IB1 (n=1). The median total operative time was 109 minutes (range, 105 to 311 minutes), the median blood loss was 100 mL (range, 10 to 750 mL), and the median weight of the resected uteri was 200 g (range, 40 to 310 g). One benign case was converted to 3-port robotic surgery due to severe pelvic adhesions, and no post-operative complications occurred.</P><P><B>Conclusion</B></P><P>Robotic single-port transumbilical total hysterectomy is technically feasible in selected patients with gynecological disease. Robotics may enhance surgical skills during single-port transumbilical hysterectomy, especially in patients with gynecologic cancers.</P>

        Background: A low-fluence 1064-nm Q-switched neodymium-doped yttrium aluminum garnet (QSND) laser has been used for treatment of pigmented disorders. However, the photobiological mechanism of the low-fluence 1064-nm QSND laser is not yet fully understood. Objectives: To evaluate inhibitory effects of the low-fluence 1064-nm QSND laser on the melanogenesis and assess whether increasing the number of passes of low-fluence the 1064-nm QSND laser has positive correlation with improvements of the pigmentation. Methods: The skins of six Hair Moprpholoy-2 melanin-possessing hairless mice were treated with ultraviolet B radiation followed by 0, 2, 4, 6 passes the low-fluence 1064-nm QSND laser. Pigmentation changes of skins were evaluated with spectrophotometry and pigmentation-related protein expressions. Additionally, biopsy specimens were obtained to figure out the difference of melanin particles. Results: The low-fluence 1064-nm QSND laser improved the lightness of the skin in comparison to the control skin. Biopsy specimens showed decreased amounts of melanin particles. In the western blot of the specimen, expressions of MITF and tyrosinase were reduced. Conclusion: The low-fluence 1064-nm QSND laser has effects on the melanogenesis which change expressions of the pigmentation-related proteins. Moreover, the more passes of the laser toning can bring the more progress on the skin pigmentation.

        Background: Grid fractional radiofrequency (GFR), fractional radiofrequency microneedle (FRM) and high intensity focused ultrasound (HIFU) have each specific mechanism for skin rejuvenation. Nasolabial fold wrinkles can be treated by applying consecutive sessions of three modalities. Objectives: To evaluate the clinical efficacy of the combined treatment of nasolabial fold wrinkles in Asians. Methods: This was a prospective study that enrolled twenty subjects with moderate nasolabial fold wrinkles. Subjects visited the clinic every four weeks for a total of three treatment sessions. Subjects received a combination therapy of GFR, FRM, and HIFU. Evaluation included medical photographs and satisfaction questionnaires. Two dermatologists evaluated the clinical photographs using the modified Fitzpatrick wrinkle scale (mFWS). Results: A total of eighteen subjects were assessed. The median age was 56.5 (Range 49-64). mFWS showed an improved score of 1.0 (IQR 0.5-1.375) at twenty weeks compared to a baseline score of 1.5 (IQR 1-1.875). (p <0.005) Six subjects (33 %) reported more than moderate improvement at twentieth week. There were no long-term complications associated with the treatment. Conclusion: This study was the first of its kind to use a triple combined therapy for the treatment of skin wrinkles. The results demonstrated the efficacy and safety of the combination therapy on nasolabial fold wrinkles.

        Inverted type A (clonal) nevus is a unusual type of melanocytic nevus. It contains pigmented nests of type A nevus cells in the depth of a banal. The nest is occasionally surrounded by type B or C nevus cells. Therefore, type A nevus cells are layered in deeper portion of nevus and Type B and C nevus cells are observed on superficial layers. An unusual presentation of inverted type A nevus have been rarely reported with superficial neurotization. We described a case of a 68 year-old female complained of solitary, 0.4 cm-sized, dome shaped, brownish to skin-colored papule on the occiput. There was no pruritus or pain on the lesion. The excision biopsy was performed. The histology examination revealed clustered, swirling, epithelioid, type A nevus cells proliferation in the deep portion of perifollicular area. The dense, basophilic type B nevus cells were surrounding the nests of type A cells. Moreover, extensive neurotized type C nevus cells appeared in the upper dermis. No atypia and abnormal mitosis were observed. It could be diagnosed with inverted type A nevus based on the clinical and histological findings.

        Background: Becker`s nevus is a relatively common harmatoma that, in adult patients, is usually accompanied with hair growth. Objectives: The purpose of this study is to evaluate the efficacy of epilation laser treatments, namely, long-pulsed 1064-nm Nd:YAG and 755-nm Alexandrite laser, in the treatment of Becker`s nevus Methods: Retrospective chart reviews and photographic analyses were performed for patients treated with epilation lasers. Clinical improvement was assessed by two blinded physicians using 5-point global assessment scale. Results: Total 13 patients (8 males, 5 females) with Fitzpatrick skin types IV, a mean age 2 of 5.0±9.4 years, were included. The most common lesions were on the cheeks (4 patients, 30.8 %) and every lesion showed scattered patterns of hair growth. 8 patients were treated with long-pulsed 1064-nm Nd:YAG laser; 4 patients were treated with 755-nm Alexandrite laser and one patient was treated with both lasers. The mean treatment interval was 28.7±6.0 days. The median total treatment session was 2.0 (interquartile range 1.0). After laser epilation therapy, 11 patients (84.6 %) showed an improvement margin of 50 %. The difference of treatment response between two lasers was not clinically significant. (p=0.80) No persistent and serious adverse events were observed during the follow-up period. Conclusion: Laser epilation can be an effective, early therapeutic option for the treatment of Becker`s nevus.

        Background: Onycholysis is the detachment of the nail plate from the nail bed that occurs at distal portion of nail plate. It is commonly associated with psoriasis, lichen planus, onychomycosis, trauma and wet work. Objectives: The purpose of this study is to evaluate the clinical efficacy of 1064-nm picosecond domain Nd:YAG (P-Nd:YAG) and cryotherapy on patients with onycholysis. Methods: Retrospective chart reviews and photographic analyses were performed. Nail plate and matrix lesions were irradiated with P-Nd:YAG laser. Nail bed and hyponychium lesions showing subungual hyperkeratosis were treated with cryotherapy. Clinical improvement was assessed by two blinded physicians using 5-point global assessment scale. Results: Three patients (all females, mean age 47.0±9.5 years) were analyzed. A total of 28 nails, consisting of 22 finger nails and 6 toe nails, were treated with P-Nd:YAG and cryotherapy. Three patients received both modes of treatment simultaneously. The mean treatment interval was 18.1±3.6 days. The mean treatment session was 4.0. After receiving the combined P-Nd:YAG and cryotherapy, all Three patients (100 %) showed more than 50% improvement. There were no serious adverse events associated with the treatment during follow-up period. Conclusion: Concurrent P-Nd:YAG and cryotherapy can be an effective and safe treatment choice for refractory onycholysis.

        Methylphenidate is the drug of choice for patients with Attention deficit hyperactivity disorders (ADHD). It acts as a dopamine and norepinephrine reuptake inhibitor, resulting in a prolongation of dopamine receptor effects. Most common side effects associated with its use include nervousness, insomnia, and dizziness. Although patients on methylphenidate frequently complain of dermatologic symptoms such as hyperhidrosis, hypersensitivity, pruritus, and hair loss, cases of peripheral vasculopathy without systemic involvement have rarely been documented. We describe the case of an 11 year-old male with ADHD who has been on methylphenidate for two years. On his initial visit to this hospital, he complained of regional erythematous reticular patches on both hands, lower legs, and both feet. The patient denied having systemic symptoms such as fever, malaise and arthralgia. Subsequent blood tests and rheumatologic labs were unremarkable. After discontinuing methylphenidate for two weeks, the patient`s symptoms were relieved. This case shows that the latent reaction of methylphenidate can induce symptoms of peripheral vasculopathy, such as livedo reticularis and the Raynaud phenomenon.
