        Objectives : The directional supplementation and draining and twirling supplementation and draining have been used widely in oriental medical clinic. The aim of this study is to compare the effects between directional supplementation and draining method and twirling supplementation and draining. Methods : Clinical experiment was performed with 21 students of oriental medical college of Daegu Haany university. Acupuncture was applied at left Hapgok (LI4) with directional supplementation and twirling supplementation respectively, and the thermographic change was checked using DITI (Digital Infrared Thermographic Image). Results and Conclusion : A significant thermographic change was observed at bilateral Igan (LI2), Yanggye (LI5), Gokji (LI11), and Yeonghyang (LI20) in directional supplementation. A significant thermographic change was observed at bilateral Igan (LI2), Yanggye (LI5), and Yeonghyang (LI20) in twirling supplementation. Based on the thermographic change, it could be demonstrated that directional supplementation is more effective that twirling supplementation.

        手少陽三焦經 正格의 自經補瀉(中渚 補, 液門 瀉) 刺鍼이 정상 흰쥐의 腦血流量 및 血壓에 미치는 影響

        Objectives : Joongjeo(TE3) Supplementation Aekmoon(TE2) Draining is a method belongs to Ohaeng-acupuncture, using directional supplementation and draining. Methods : This study was designed to investigate the effects of TE3 supplementation TE2 draining on changes in cerebral blood flow (rCBF) and mean arterial blood pressure (MABP) in normal rats. For these reasons, the present author investigated rCBF and MABP using laser doppler flowmeter in normal rats. In addtion, the present author also investigated action mechanisms of TE3 supplementation TE2 draining on changes in rCBF and MABP too. Results : In this results, TE3 supplementation TE2 draining elevated rCBF in time-dependent manner, but MABP levels decresed by TE3 supplementation TE2 draining. Pre-treatment with indomethacin (IDM), an inhibitor of cyclooxygenase, inhibited increase of rCBF effectively. But pre-treatment with methylene blue (MTB), an inhibitor of guanylate cyclase, decreased rCBF levels. In addition, pre-treatment with IDM also decreased MABP levels, but pre-treatement with MTB increased MABP levels. Conclusions : In conclusion, these results suggest that TE3 supplementation TE2 draining is effective to treat patient with disease related to cerebral ischemia, because TE3 supplementation TE2 draining can increase rCBF. In addition, the mechanisms are thought to be related to guanylate cyclase pathways.

        足少陽膽經 正格 刺鍼이 白鼠의 腦血流力學 變動에 미치는 영향

        object : The purpose of this report is to examine the effect of BL66․GB43 supplement and LI1․GB44 draining in acupuncture for cerebral apoplexy treatment. methods : I designed to investigate whether BL66․GB43 supplement and LI1․GB44 draining in acupuncture affects cerebral hemodynamics〔regional cerebral blood flow(rCBF), mean arterial blood pressure(MABP)〕in normal rats, and to make manifest whether BL66․GB43 supplement and LI1․GB44 draining in acupuncture is mediated by cyclooxygenase or guanylate cyclase. The changes of rCBF and MABP were determinated by laser-doppler flowmetry(LDF). Results : 1. BL66․GB43 supplement and LI1․GB44 draining in acupuncture increased rCBF significantly, but decreased MABP. These results suggest that BL66․GB43 supplement and LI1․GB44 draining in acupuncture increased significantly rCBF by dilating pial arterial diameter(PAD). 2. By pretreatment with indomethacin(1 ㎎/㎏, i.p.), the effect of BL66․GB43 supplement and LI1․GB44 draining in acupuncture induced increase of rCBF was significantly inhibited, the decreased MABP by treatment with BL66․GB43 supplement and LI1․GB44 draining in acupuncture was increased. 3. By pretreatment with methylene blue(10 ㎍/㎏, i.p.), the increased rCBF by treatment with BL66․GB43 supplement and LI1․GB44 draining in acupuncture was decreased conversely, the decreased MABP by treatment with BL66․GB43 supplement and LI1․GB44 draining in acupuncture was decreased and then increased. Conclusion : I suggest that BL66․GB43 supplement and LI1․GB44 draining in acupuncture has an anti-ischemic effect through the improvement of cerebral hemodynamics. 동신대학교 한의과대학 침구학교실

        염전보사의 정량적 기준에 대한 고찰

        Objectives and Methods : The aim of this study was to investigate twirling and supplementation and draining by analyzing ancient acupuncture literature and modern research papers, and to discuss the quantitative standards for the procedure. Results : Twirling supplementation and draining techniques were first described in Huang do neijing, after which they were recorded in various other ancient acupuncture texts. Not only were these techniques used in the active treatment of patient historically, but they also form the foundations of modern-day acupuncture treatment. Twirling supplementation and draining techniques described in ancient literatures can be divided into Yin and Yang. Examples of Yang characteristics include Up, Male, Morning, Hands, the governor vessel, Outside, and Left. The left rotation technique is a reinforcing method performed by moving the thumb forward while rotating the needle. Examples of Yin characteristics include Down, Female, Afternoon, Foot, the conception vessel(CV), Inside, and Right. The right rotation technique is a reducing method performed by moving the thumb backward while rotating the needle. Twirling supplementation and draining has been divided for four ways by classical literature and research that worked before. the direction of acupuncture, the stimulus-quantity of acupuncture, the performance of doctor(the uses of both hands, left and right hand) and the feeling of patient. Conclusions : Therefore, continuous research is required to implement Twirling supplementation and draining in a consistent manner. In addition, more active clinical trials using the techniques are necessary as well.

        대돈(大敦) 보(補) 음곡(陰谷) 사(瀉) 자침(刺鍼)이 정상 흰쥐의 뇌혈류력학(腦血流力學) 변화(變化)에 미치는 영향(影響)

        Objectives: The purpose of this study is to investigate the effects of Daedon($LR_1$) Supplementation and Eumgok($KI_{10}$) Draining on changes of cerebral blood flow in normal rats. Methods : Regional cerebral blood flow(rCBF) and mean arterial blood pressure(MABP) in normal rats are observed, and those mechanisms were also investigated with pre-treatment of indomethacin (IDM) and methylene blue(MTB) each. Results : In this study, $LR_1$ supplementation and $KI_{10}$ draining elevated level of rCBF after 30 min, but MABP level was lowered at 30 min, then recovered toward normal level. Pre-treatment with indomethacin (IDM), an inhibitor of cyclooxygenase, inhibited increase of rCBF effectively, and pre-treatment with methylene blue(MTB), an inhibitor of guanylate cyclase, also inhibited increase of rCBF levels. On the other hand pre-treatment with IDM or MTB did not affect MABP levels. Conclusions : In conclusion, these results suggest that $LR_1$ supplementation and $KI_{10}$ draining can increase rCBF, and the mechanisms are thought to be related to both of cyclooxygenase and Guanylate cyclase pathways.

        『黃帝內經』에 나타난 員方補瀉에 관한 考察

        Object : 본 연구는 『靈樞·官能』과 『素問·八正神明論』에서 상반되게 제시된 員方補瀉의 원리를 파악하고자 하였다. Method : 두 편의 원문을 분석하고 모난 것(方)과 둥근 것(員)의 의미와 부합하는 보사원리를 발견하고자 하였다. 특히 「八正神明論」의 경우 방과 원에 대한 추상적 설명과 구체적 수기법 사이의 연관성을 파악할 수 있는 가설을 세우고 원문과 역대의 주석을 통해 검증하는 방식으로 논의를 진행하였다. Result & Conclusion두 편에 사용된 方은 ‘方正하다’, 員은 ‘원활하다’는 의미를 공유한다. 차이점은 『靈樞·官能』의 員方은 시침자의 動靜이 기준이며, 『素問·八正神明論』에서 설명한 員方은 환자의 호흡이 기준이 된다. 또한 전자는 補瀉의 대상이 正氣와 外邪로 명확히 구분되는 반면, 후자는 正氣의 치우침으로 생겨난 虛實을 다스린다는 점에서 補瀉의 대상이 모두 正氣에 속한다. 이를 통해 자침의 보사가 正氣와 邪氣를 구분 짓는 방법과 정기의 인체 내 허실을 조절하는 방법으로 구분됨을 알 수 있다. Objectives : This study aims to grasp the principle of the Won Bang supplementation and draining method as mentioned in the 『Lingshu·Guanneng』 and the 『Suwen·Bazhengshenminlun』, which have contrasting properties. Methods : The texts in each chapter were analyzed to understand the supplementation and draining principle that matches the meanings of round[won,員] and angular[bang, 方]. Especially in the case of the chapter 「Bazhengshenminlun」, a hypothesis was drawn upon the relationship between the abstract explanation of the round[won,員] and angular[bang, 方] and the actual manipulation technique. This hypothesis was tested against other texts and annotations for further discussion. Results & Conclusions : The expressions ‘bang[angular, 方]’ and ‘won[round,員]’ refer to the same meanings in both chapters, as ‘to be upright’ and ‘to be smooth,’ respectively. The difference between the two chapters is that in the 『Lingshu·Guanneng』 the standard for Won Bang is the needler’s movements, while in the 『Suwen·Bazhengshenminlun』, it is the patient’s breathing. Moreover, while in the former the subjects of supplementation and draining are clearly divided into healthy qi and exterior pathogenic qi, in the latter the subject of manipulation is the deficiency and excessiveness caused by deviation of the healthy qi, thus making the subject of both supplementation and draining healthy qi. Based on these findings, we can conclude that the supplementation and draining of needling is divided into two methods; separating the healthy and pathogenic qi and manipulating the deficiency and excessiveness of healthy qi within the body.

        한국 침구 전문서 『영소침구경』 연구

        Objectives : Previous research on texts specializing in acupuncture and moxibustion in Korea have mostly been focused on those written in the Joseon period, leaving an academic void in the modern and contemporary eras. This research on the "Yeongsochimgugyeong" was undertaken to help readers' understanding of the text and to provide basic material for further research on modern-contemporary education of acupuncture and moxibustion. Methods : The original text of "Yeongsochimgugyeong" was examined along with books and news articles on related people. Information that was difficult to obtain through written documents was supplemented by interviews with people who knew about the situation at the time. The findings were organized as 1) the author and related people and 2) introduction of "Yeongsochimgugyeong". Results and Conclusions : The original text of "Yeongsochimgugyeong" was written by teacher Jeon, Gwangok, interpreted and annotated by his disciple Gwon, Yeongjun. It is presumed that they wrote this book as educators of Korean medicine for educational purposes on acupuncture and moxibustion before and after the Japanese colonial era. The book is consisted of an overview of meridianology, details on points and meridians, supplementing and draining methods, similar to the current text book. The contents and expressions of "Yeongsochimgugyeong" reflect the state of acupuncture and moxibustion education of the early and mid $20^{th}$ century, making it a valuable resource.
