Topical minoxidil has been used to treat male pattern hair loss and alopecia areata multiplex. It is generally well tolerated, but common adverse events, including irritation, pruritus, dryness, scaling and unwanted hair growth, have been reported. A 34-year-old woman presented with numerous pinhead- to matchhead-sized papules and vellus hair growth on the entire face, especially on the forehead. She had applied 3% minoxidil solution on the scalp for 4 years for the treatment of female pattern hair loss. Surgical incision had been performed to remove the cysts. Eruptive vellus hair cysts remitted after surgical removal. On histopathological examination, laminated keratinous material and vellus hairs (diameter) were shown. There was no stimulatory effect of minoxidil on the expression of CK17 and Ki-67 in cultured hair organs when compared with control. Eruptive vellus hair cysts show asymptomatic follicular papules and they are caused by a developmental abnormality of vellus hair follicles. Hypertrichosis is one possible adverse effect of minoxidil. Hypertrichosis is restricted to the face and exposed areas, possibly due to inadvertent application. Rarely, systemic hypertrichosis caused by higher sensitivity of the hair follicles to minoxidil has been reported. We report this unusual case with facial eruptive vellus hair cysts occurred by the application of 3% minoxidil solution on the scalp for the treatment of hair loss.

Eruptive vellus hair cysts (EVHCs) are small, red- or brown-colored cystic papules that usually occur on the chest and proximal extremities of children and adults. Histologically, these small cysts are observed in the mid-dermis and contain multiple hair shafts and keratinous material. Pathogenesis of EVHC has not been completely understood. A 40-year-old man with mental retardation presented with multiple bluish papules on his back. A biopsy specimen showed features of eruptive vellus hair cysts. We report a case of EVHC that occurred at an unusual site in a patient with mental retardation. (Korean J Dermatol 2017;55(5):290∼292)

Eruptive vellus hair cysts(EVHCs) are benign dermal cysts that develop as a consequence of occlusion of the infundibulum of vellus hairs. The primary cause of occlusion is unknown. There are no standard treatments to date, but surgical removal, laser therapy, and various topical treatments have been reported. To our knowledge, there are 4 English literatures that treated EVHCs with retinoic acid with variable outcomes. Herein, we present a 4 year-old male. Papules appeared on his chest 1 year ago and spread continuously. Biopsy was done and it was diagnosed as EVHCs. Topical tretinoin 0.01% cream was applied on the lesion for 6 months and lesions markedly improved. Our case shows tretinoin is an effective and safe treatment modality of EVHCs.
A case of eruptive vellus hair cysts in an 8-year girl with precocious puberty

Eruptive vellus hair cysts (EVHCs) are uncommon developmental anomaly of vellus hair follicles that are observed in young patients. EVHCs are either sporadic, in which case they appear in the first or second decade of life, or inherited in an autosomal dominant fashion with lesions appearing at birth or in early infancy. They manifest clinically with multiple small dome shaped papules with colors ranging from skin-colored, yellow, and blue to red. Lesions commonly involve anterior chest, abdomen, and extremities. Histological examination typically shows a cystic structure lined with squamous epithelium, filled with laminated keratin and multiple vellus hairs. About 25% of cases resolve spontaneously and for the remaining cases, there is no definite treatment but topical retinoic acid, pulsed carbon dioxide, and erbium: Yttrium aluminum garnet-laser ablation can be tried. An 8-year-old girl presented with multiple dot-like macules on the back. She was diagnosed with precocious puberty and Hashimoto’s thyroiditis, treated with leuprolide and levothyroxine, respectively. Histopathologic findings from the macules showed small follicular cysts lined by squamous epithelium containing laminated keratinous material and vellus hair shafts. The patient was diagnosed with EVHCs and observed without treatment. Herein we report a case of EVHCs in an 8-year girl with precocious puberty.

An eruptive vellus hair cyst is an uncommon, benign, epithelial tumor with hair differentiation. Clinically, it presents as asymptomatic, flesh-colored to yellowish, follicular papules, 1 to 4 ㎜ in diameter, most commonly on the chest. It is usually sen in children and young adults but may be congenital. Histologically, it shows a middermal cyst lined by squamous epithelium containing keratinous material and vellus hairs. It has a tendency to clear spontaneously but may be persistent. We report a case of congenital eruptive vellus hair cyst in an 11-year-old girl who presented multiple pinhead-sized brown to black papules on her anterior chest since birth. A case of coexistence of steatocystoma multiplex and eruptive vellus hair cysts

Steatocystoma multiplex and eruptive vellus hair cysts are uncommon benign tumors of the pilosebaceous unit. They share a clinical resemblance, but have distinctive histologic features. It has been proposed that these clinical entities are a spectrum of the same disease process. A 23-year-old man presented with multiple papules on the neck and chest for 10 years. On physical examination, there were flesh-colored smooth papules on the neck and follicular papules on the chest. Histologic findings of the flesh-colored smooth papules revealed cysts lined by wavy eosinophilic cuticle and lack of a granular layer. In contrast, histologic findings of the follicular papules revealed cysts lined by mature squamous cells with a granular layer, and containing vellus hairs and layered laminated keratinous material. Based on these findings, diagnoses of steatocystoma multiplex and eruptive vellus hair cysts were made. Herein, we report a case of coexistence of steatocystoma multiplex and eruptive vellus hair cysts in a patient which is rarely reported in the Korean literature. This case possibly support the idea that steatocystoma multiplex and eruptive vellus hair cysts are within the spectrum of the same disease process.

Eruptive vellus hair cysts are characterized by asymptomatic, skin colored to bluish, of brownish, 1-4㎜ sized, follicular papules and nodules located on mainly anterior chest, axilla, groin, and thigh. Histopathologic examination reveals cysts containing multiple vellus hairs and the laminated keratinous materials at dermis. We reported 2 cases of eruptive vellus hair cysts developed on the unusual sites, forehead and buttock, respectively. (Korean J Dermatol 2004;42(1):74~76)

Eruptive vellus hair cysts and steatocystoma multiplex have clinical similarities in terms of age of onset, location and appeararice of the lesions, but histopathologic feitures are distinctively differen-tiated. We present herein a 37-year-old woman with eruptive vellus hair cyst and steatocystoma multiplex as well. The histopathologic sections from 2 different lesions showed features of both eruptive vellus hair cyst with sebaceous gland in and near the cyst wall, and steatocy toma multiplex with rudimentary hair follicle near the cyst wall. This case may support the premie that eruptive vellus hair cyst and steatocystoma multiplex have a common developmental origin. (Kor J Dermatol 1994; 32(1): 149-152)

We report a 48-year-old female who had eruptive vellus hair cysts associated with milia on the face. Histopathologic examination of lesions on the forehead and cheek showed typical findings of eruptive vellus hair cysts and milia. These findings suggested that two entities are in some way related. Eruptive vellus hair cysts and milia can be considered as subtypes of multiple pilosebaceous cysts that may all present overlapping histologic features. (Korean J Dermatol 2003;41(6) : 780~783)

Both eruptive vellus hair cyst and steatocystoma multiplex are uncommon conditions. There are clinical similarities between these two entities, but histological features are distinctive. We describe a patient with multiple subcutaneous nodules on the chest and both axilla, showing combined histological features of both an eruptive vellus hair cyst and steatocystoma multiplex. This case suggests that eruptive vellus hair cyst and steatocystoma multiplex are variants of one disorder which originate in the pilosebaceous duct. (Korean J Dermatol 1998;36(1): 116-119)
