        Age Differences in Health-Related Quality of Life Among the Elderly

        본 연구의 목적은 국내 노인의 연령별 건강관련 삶의 질의 차이를 이해하고, 연령대별로 적합한 건강프로그램을 계획하는 데 도움을 주고자 하는 것이다. 이 연구는 단면조사연구로 2010년-2012년에 실시된 국민건강영양조사의 일부 자료를 발췌하여 통계 처리하였다. 연구대상은 국내의 지역사회 내 거주하고 있는 1,666명의 노인이었다. 대상자의 일반적 특징조사를 위해 설문지를 사용하였고, 건강관련 삶의 질(3 level version of the EuroQoL 5 dimensional questionnaire; EQ-5D-3L)을 조사하기 위해 면담을 실시하였다. 통계처리는 복합표본조사의 선형회귀분석을 사용하였다. 연구결과, 각 연령별(65-69세, 70-74세, 75-79세, 80-84세, 85세 이상)에서 EQ-5D-3L은 차이가 있었다. EQ-5D-3L와 EQ-5D VAS의 이동, 자기관리, 일반적 활동영역에서 연령별로 통계학적인 차이가 있었다. 건강관련 삶의 질은 노인의 연령별로 차이가 있으므로 건강관련 프로그램을 개발 시 연령대에 따라 계획할 필요가 있다. A total of 1,666 elderly drawn from the non-institutionalized civilian population in South Korea were surveyed using a written questionnaire and an interview that incorporated the EQ-5D-3L. The study was conducted between 2010 and 2012. The data were analyzed using Linear regression analysis of complex sampling design. Scores of EQ-5D-3L differed according to each age groups. Mobility, self-care and usual activities domains of the EQ-5D-3L and EQ-5D VAS had significant differences among the age groups. The HRQoL showed differences according to age in older populations. Healthcare providers must consider the differences in HRQoL based on age when planning healthcare programs.

        Health-Related Quality of Life and its Associated Factors in COVID-19 Patients

        Objectives Health-related quality of life (HRQoL) is one of the most important outcome measures for patients. The purpose of this study was to evaluate HRQoL and related factors in Coronavirus disease 2019 (COVID-19) patients. Methods A total of 420 COVID-19 patients who had been discharged from hospital were selected using a systematic sampling. The EuroQol 5-dimensional-5 levels (EQ-5D-5L) questionnaire along with medical records of the patients were used to gather the data. The t test and analysis of variance were employed to test the difference between mean EQ-5D-5L scores, and the BetaMix model was used to investigate factors associated with EQ-5D-5L scores. Results The mean score for the patients who completed the EQ-5D-5L questionnaire (n = 409) was 0.6125. The EQ-5D-5L scores were significantly higher in males, patients with younger age, those with a low level of education, the employed, patients who worked in uncrowded workplaces, patients without diabetes, and those who were not admitted to intensive care unit. The BetaMix model showed that gender, age, education, employment status, having diabetes, heart failure, and admission to the intensive care unit were significant independent predictors of the EQ-5D-5L index values. Conclusion The mean score for EQ-5D-5L in COVID-19 patients was low in this study. Some of the factors, especially aging and having diabetes, should be considered in the aftercare of patients to improve their HRQoL.

        Health-related Quality of Life of Patients With Diabetes Mellitus Measured With the Bahasa Indonesia Version of EQ-5D in Primary Care Settings in Indonesia

        Objectives: Diabetes mellitus (DM) is a serious public health issue that places a heavy financial, social, and health-related burden on individuals, families, and healthcare systems. Self-reported health-related quality of life (HRQoL) is extensively used for monitoring the general population’s health conditions and measuring the effectiveness of interventions. Therefore, this study investigated HRQoL and associated factors among patients with type 2 DM at a primary healthcare center in Indonesia.Methods: A cross-sectional study was conducted in Klaten District, Central Java, Indonesia, from May 2019 to July 2019. In total, 260 patients with DM registered with National Health Insurance were interviewed. HRQoL was measured with the EuroQol Group’s validated Bahasa Indonesia version of the EuroQoL 5-Dimension 5-Level (EQ-5D-5L) with the Indonesian value set. Multivariate regression models were used to identify factors influencing HRQoL.Results: Data from 24 patients were excluded due to incomplete information. Most participants were men (60.6%), were aged above 50 years (91.5%), had less than a senior high school education (75.0%), and were unemployed (85.6%). The most frequent health problems were reported for the pain/discomfort dimension (64.0%) followed by anxiety (28.4%), mobility (17.8%), usual activities (10.6%), and self-care (6.8%). The average EuroQoL 5-Dimension (EQ-5D) index score was 0.86 (95% confidence interval [CI], 0.83 to 0.88). In the multivariate ordinal regression model, a higher education level (coefficient, 0.08; 95% CI, 0.02 to 0.14) was a significant predictor of the EQ-5D-5L utility score.Conclusions: Patients with diabetes had poorer EQ-5D-5L utility values than the general population. DM patients experienced pain/discomfort and anxiety. There was a substantial positive relationship between education level and HRQoL.

        Health-related Quality of Life Instrument With 8 Items for Use in Patients With Type 2 Diabetes Mellitus: A Validation Study in Korea

        Objectives: This study assessed the validity and reliability of the Health-related Quality of Life Instrument with 8 Items (HINT-8) in patients with diabetes. HINT-8 is a newly-developed, generic health-related quality of life (HRQoL) instrument. Methods: Three HRQoL instruments—HINT-8, EuroQoL 5-Dimension 5-Level (EQ-5D-5L), and Short Form Health Survey version 2.0 (SF-36v2)—were provided to 300 patients with diabetes visiting a tertiary hospital for follow-up visits in Korea. The HRQoL scores obtained using the HINT-8 were evaluated for subgroups with known differences based on demographics and diabetes-related characteristics (known-group validity). The mean scores of the instruments were compared between groups segmented by their responses to the HINT-8 (discriminatory ability). Correlation coefficients of the HINT-8 with other instruments were calculated (convergent and divergent validity). The Cohen kappa and intra-class correlation coefficient (ICC) were also evaluated (test-retest reliability). Results: The average HINT-8 indexes were lower among women, older, and less-educated subjects. Subjects who did not list any problems on the HINT-8 had significantly higher HRQoL scores than those who did. The correlation coefficients of the HINT-8 with the EQ-5D-5L index and EuroQoL visual analogue scale were 0.715 (p<0.001) and 0.517 (p<0.001), respectively. The correlation coefficients between the HINT-8 index and the scores of 8 domains of the SF-36v2 ranged from 0.478 (p<0.001) to 0.669 (p<0.001). The Cohen kappa values for the HINT-8 ranged from 0.268 to 0.601, and the ICC of the HINT-8 index was 0.800 (95% confidence interval [CI], 0.720 to 0.860).

        산후관리서비스가 산모의 건강관련 삶의 질에 미치는 영향

        Purposes: This study purposed to identify influence of postpartum care services on health-related quality of life in women after birth. Methodology: Korea Health Panel Survey 2009-2015 provided by the Korea Institute for Health and Social Affairs and National Health Insurance Service was used for the analysis. The health-related quality of life evaluate as a EuroQoL-5D(EQ-5D-3L, used the weight of the CDC) was used as dependent variables. Postpartum care services was used as independent variable. Demographic factors(education, economic activity, region, house income), health related variable(presence of chronic disease, self-rated health), birth related variable(birth-related problem, childbirth, pregnancy of advanced maternal age) used as covariates. Regression analysis was used. Findings: The rate of use of postpartum care services is increasing year by year. Postpartum care services and self-rated health positively influence on the health-related quality of life in women after birth and chronic disease and birth-related problem negatively influence on. Practical Implications: Therefore, it is necessary that the government’s policy of the postpartum care service be expanded and systematized to increase accessibility on. There are rare studies on the health-related quality of life of women after childbirth, adjusted for birth-related variables. So this study has significance.

      • 한국에서 경동맥 협착증 관련 건강상태별 삶의 질 조사

        Objectives: This study was carried out to investigate the impact of carotid artery stenosis on qualify of life in Korea. Methods: A health scenario for each health states after undergoing carotid endar- terectomy or carotid artery stent was developed, and Time Trade Off and EQ-5D-3L tools were used on 400 general population to examine health-related quality of life. In addition, the 5th Korea Na- tional Health and Nutrition Examination Survey (2012) and Stoke Registry of Seoul National Uni- versity Bundang Hospital were utilized to find out quality of life of stroke and myocardial infarction, which are major complications of carotid artery stenosis. Results: As a result of survey, utility weight for health states without major complications after undergoing carotid endarterectomy or carotid artery stent to treat carotid artery stenosis was 0.79 (±0.10), and utility weight was further decreased to 0.61 (±0.13) for health states after undergoing reoperation due to restenosis. These weight were similar to major complications, which showed utility of 0.77 (±0.05) for myocardial infarction and 0.79 (±0.06) for minor stroke. They were higher than weight of 0.41 (±0.48) for major stroke. Con- clusion: The results of this study showed that quality of life of carotid artery stenosis is similar to myocardial infarction and minor stroke. The meaning of this study is in that it produced Korean utility weight of each health states related to carotid artery stenosis which are regarded as critical data re- quired for cost-utility analysis in the future.
