We report a 56-year-old female with symptomatic protrusion of the bony lesion in the trapezium. Excision and biopsy of the bony lesion revealed thickened and sclerotic bony trabecula with adjacent zone of fibrocartilage, which is compara- ble with melorheostosis. This lesion with unique radiologic and histologic find- ings may be important to differentiate with other bony lesions such as myositis ossifications and osteosarcoma. 유선상 과골증은 희귀한 경화성 골질환으로 수부에는 매우 드물게 발생한다. 저자들은 56세 여성의 좌측 대능형골에발생하여 증상을 유발한 유선상 과골증을 보고하고자 한다. 조직학적 검사상 두껍고 경화된 골주를 가지는 성숙골이 관찰되었으며, 환자는 수술 후 재발 소견이 없어 관찰 중이다. 이 병변은 특징적 방사선 소견 및 조직학적 소견을 통하여골화성 근염이나 골육종 등 다른 골 병변과의 감별이 필요하다.

유선상 과골증은 동통이나 관절의 강직을 유발할 수 있는 매우 드문 경화성 골질환으로 양초가 흘러내리는 듯한 특징적인 방사선적 양상을 보이는 것으로 알려져 있다. 치료는 보존적 치료를 시행하며, 압박 증상이나 변형 등을 예방하고 교정하기 위해 수술적 치료를 병행하기도 한다. 저자들은 수근골, 족근골, 쇄골, 경골 등 다양한 부위에서 발생한 유선상 과골증에 대해 보존적 치료와 수술적 치료를 병행하여 성공적인 치료 결과를 경험하였기에 보고하고자 한다. Melorheostosis is a very rare sclerosing bone disease with pain, joint stiffness, and limitation of motion. Its characteristic on radiography is a dense bone formation along the side of the bone, resembling the flow of candle grease. Various conservative or surgical methods have been practiced in treating pain and deformities. This is a report on the successful treatment result of six cases of melotheostosis, involving the metatarsal, metacarpal, clavicle, and tibia, which weretreated by conservative and operative treatment methods.
