        Simplified Method for Lateral Distribution Factor of the Live Load of Prefabricated Concrete Box-Girder Bridges with Transverse Post-tensioning

        Transverse post-tensioning arrangement in prefabricated prestressed concrete (PC) box-girder bridge can effectively improve the transverse connection's performance to avoid stress concentration. The number of transverse post-tensioning tendons and the transverse post-tensioning force are crucial factors determining the live load's lateral distribution. Four prefabricated PC box-girder model tests were carried out to calculate the lateral distribution of the live load. Based on experimental results, the recommended values of the reduction factors of contact interfaces considering the effective longitudinal transfer length of transverse post-tensioning were given, and a simplified method of lateral distribution factor was proposed. The proposed simplified method results were compared with the PCI code, experiments, finite element method, articulated plate method, and rigid-plate method. The results showed that the lateral distribution factor calculated by the simplified method agreed with the results of experiments and the finite element method. The average relative differences were less than 8%. In comparison, the relative differences were more than 15% for the PCI code, articulated plate method, and rigid plate method. Thus, the proposed method could meet the engineering application and be adopted to calculate the lateral distribution factor of bridges with transverse post-tensioning.

        Circ_0074027 regulates the progression of non-small cell lung cancer via miR-1304-5p/GJB2 axis

        Background Non-small cell lung cancer (NSCLC) is one of the most common malignant tumors; circular RNA (circRNA) has been shown to be involved in the advance of NSCLC. However, the function of circ_0074027 in NSCLC has not been fully studied. Objective Quantitative real-time polymerase chain reaction (qRT-PCR) was used to detect the expression of circ_0074027, microRNA-1304-5p (miR-1304-5p) and gap junction protein beta 2 (GJB2) in NSCLC. Cell counting kit-8 assay, thymidine analog 5-ethynyl-2′-deoxyuridine (EdU) assay, transwell assay, and flow cytometry were designed to explore cell function. Western blot examined the protein levels of Bax, Bcl-2 and GJB2. The relationship between miR-1304-5p and circ_0074027 or GJB2 was verified by dual-luciferase reporter assay and RNA immunoprecipitation (RIP) assay. The xenotransplantation model was established to study the role of circ_0074027 in vivo. Results circ_0074027 and GJB2 are significantly high-expressed in NSCLC tissues and cells. Silencing circ_0074027 reduced the proliferation, migration and invasion of NSCLC cells, and promoted cell apoptosis. MiR-1304-5p is a target of circ_0074027. Silencing circ_0074027 inhibited the malignant behavior of NSCLC cells by releasing miR-1304-5p. In addition, GJB2 is a target of miR-1304-5p. Overexpression of GJB2 partially restored the inhibitory effect of miR-1304-5p on cell malignant behavior. Animal experiments confirmed the anti-tumor effect of silence circ_0074027 in vivo. Conclusion Circ_0074027 regulates the expression of GJB2 by regulating miR-1304-5p, thereby promoting the development of NSCLC. Our study provides a novel targeted therapy for NSCLC.

        Atrial fibrillation in critically ill patients who received prolonged mechanical ventilation: a nationwide inpatient report

        Background/Aims: To evaluate temporal trends of atrial fibrillation (AF) prevalence in critically ill patients who received prolonged mechanical ventilation (MV) in the United States. Methods: We used the 2008 to 2014 National Inpatient Sample to compute the weighted prevalence of AF among hospitalized adult patients on prolonged MV. We used multivariable-adjusted models to evaluate the association of AF with clinical factors, in-hospital mortality, hospitalization cost, and length of stay (LOS). Results: We identified 2,578,165 patients who received prolonged MV (21.27% of AF patients). The prevalence of AF increased from 14.63% in 2008 to 24.43% in 2014 (p for trend < 0.0001). Amongst different phenotypes of critically ill patients, the prevalence of AF increased in patients with severe sepsis, asthma exacerbation, congestive heart failure exacerbation, acute stroke, and cardiac arrest. Older age, male sex, white race, medicare access, higher income, urban teaching hospital setting, and Western region were associated with a higher prevalence of AF. AF in critical illness was a risk factor for in-hospital death (odds ratio, 1.13; 95% confidence interval, 1.11 to 1.15), but in-hospital mortality in critically ill patients with AF decreased from 11.6% to 8.3%. AF was linked to prolonged LOS (2%, p < 0.0001) and high hospitalization cost (4%, p < 0.0001). LOS (-1%, p < 0.0001) and hospitalization cost (-4%, p < 0.0001) decreased yearly. Conclusions: The prevalence of comorbid AF is increasing, particularly in older patients. AF may lead to poorer prognosis, and high-quality intensive care is imperative for this population.
