<P>We have developed a simple and sensitive system for detecting AGG trinucleotide repeats through the formation of intermolecular G-quadruplexes using a fluorescent oligonucleotide. The fluorescence signal increased rapidly and dramatically by 44.7-fold with respect to the low background signal in the presence of RNA agg repeats and by 35.0-fold in the presence of DNA AGG repeats.</P>

Background: Following the coronavirus disease 2019 (COVID-19) outbreak in Wuhan, China, a total of 637 patients had been diagnosed with the disease in Seoul as of May 2, 2020. Our study aimed to describe the impact of the 3T strategies (preemptive testing, prompt tracing and proper treatment) on the epidemiological characteristics of COVID-19 in Seoul. Methods: The descriptive and explanatory analysis was carried out on critical indicators such as epidemiological characteristics and key duration of patient status change from January 24 to May 2 in Seoul before and after preemptive testing for patients under investigation associated with COVID-19 clusters. Results: Preemptive testing increased the positive test rate (3.9% to 4.2%), an asymptomatic case at diagnosis (16.9% to 30.6%), and reduced the time from symptom onset to quarantine (4.0 to 3.0 days). Prompt tracing decreased unknown sources of infection (6.9% to 2.8%), the mean number of contacts (32.2 to 23.6), and the time-varying reproduction number R(t) (1.3 to 0.6). With proper treatment, only 2 cases of mortality occurred, resulting in a fatality rate of just 0.3%. Conclusion: In the first wave of the COVID-19 pandemic lasting 100 days, the effect of the 3T strategies flattened the curve and decreased the time during which infected individuals were contagious, thereby lowering the R(t) below 1 in Seoul.
Facile conversion of ATP-binding RNA aptamer to quencher-free molecular aptamer beacon

<P><B>Abstract</B></P> <P>We have developed RNA-based quencher-free molecular aptamer beacons (RNA-based QF-MABs) for the detection of ATP, taking advantage of the conformational changes associated with ATP binding to the ATP-binding RNA aptamer. The RNA aptamer, with its well-defined structure, was readily converted to the fluorescence sensors by incorporating a fluorophore into the loop region of the hairpin structure. These RNA-based QF-MABs exhibited fluorescence signals in the presence of ATP relative to their low background signals in the absence of ATP. The fluorescence emission intensity increased upon formation of a RNA-based QF-MAB·ATP complex.</P> <P><B>Graphical abstract</B></P> <P>[DISPLAY OMISSION]</P>

Misinterpreting radiographic findings can lead to unnecessary interventions and potential patient harm. The urgency required when responding to the compromised health of trauma patients can increase the likelihood of misinterpreting chest x-rays in critical situations. We present the case report of a trauma patient whose skin fold artifacts were mistaken for pneumothorax on a follow-up chest x-ray, resulting in unnecessary chest tube insertion. We hope to help others differentiate between skin folds and pneumothorax on the chest x-rays of trauma patients by considering factors such as location, shape, sharpness, and vascular markings.

OBJECTIVES: Because effective decolonization options are not available, and treatment options are limited, carbapenem-resistant Enterobacterales (CRE) constitute increasingly threatening nosocomial pathogens. To prevent CRE-associated transmission and ensure patient safety, healthcare personnel and everyone in contact with CRE-infected patients must implement stringent infection control practices. This report describes a CRE outbreak, possibly related to a caregiver at a long-term care facility (LTCF), and presents a new surveillance model to improve the infection control of CRE in Seoul, Korea. METHODS: The Seoul Metropolitan Government surveillance system identified an outbreak of CRE in an LTCF in 2022. We obtained data on the demographic characteristics and contact histories of the inpatients, medical staff, and caregivers. To isolate the inpatients and employees exposed to CRE, we used rectal swab samples and environmental sampling during the study period (May-December 2022). RESULTS: We identified 18 cluster cases (1 caregiver and 17 inpatients) and 12 sporadic cases with CRE, and conducted a complete 197-day follow-up of all cases in the LTCF’s isolation wards. CONCLUSIONS: This investigation demonstrated that our surveillance model and targeted intervention, based on the cooperation of the municipal government, public health center, and infection control advisory committee, effectively contained the epidemic at the LTCF. Measures to improve the compliance of all employees in LTCFs with infection control guidelines should also be adopted.

대도시 중심의 생활공간 확대로 초고층 건축물이 일반화되었다. 건축물이 거대해지고 복잡해짐에 따라 화재와 같은 재난 시 긴급 피난이 더 중요해진 것이다. 대형 건물 화재 시 인명피해는 대부분 연기로 인해 발생한다. 화재 현장을 빨리 벗어나야 하는 이유이다. 계단이 막혀 피난이 어려운 긴급 상황에서 쉽게 탈출하는 시설이 피난 기구이다. 피난 기구는 화재 현장에서 마지막으로 사용하는 비상 탈출수단이다. 현재 소방시설법은 11층 이상에는 피난 기구 설치를 제외하고 있다. 이는 고층에서 안전하게 사용할 수 있는 피난 기구가 없기 때문이다. 최근 기술의 발달로 고층에서도 얼마든지 사용 가능한 피난기구가 개발되었다. 국가화재안전기준에도 이러한 피난 기구 설치기준을 제시하고 있다. 하지만 11층 이상에는 법적인 예외규정에 따라 설치하지 않기 때문에 무용지물이다. 본 연구에서는 소방청 고시로 운영되고 있는 승강식피난기의 성능인증 기술기준을 분석하였다. 승강식피난기가 성능인증 기술기준의 개선으로 고층건축물에 사용할 수 있는지 연구하였다. 관계 기관에서는 고층에서 안전하게 사용할 수 있는 피난 기구가 없어서 11층 이상의 피난 기구 설치 제외조항을 바꿀 수 없다는 논리이기 때문이다. 현 기술기준이 규정하고 있는 기준 외에 몇 가지 기준을 추가하면 고층건축물에도 사용할 수 있다는 결론을 얻었다. 새로운 기술이 개발되면서 관련 기준도 개정되어 현장에 개발품이 설치되어야 관련 산업도 발전하고, 국민안전도 실현될 것이다.

The low-frequency vibration of a passenger car determines the “hardness” and “harshness” in the ride comfort quality of a luxury passenger car. Many studies on the objective evaluation of ride quality have been performed, and diverse methods for such evaluation have been proposed. The vibration metrics that have been mainly used in the industrial field are the power spectrum density(PSD) and the vibration dose values(VDV). PSD has been used for evaluating the stationary ride comfort quality in the frequency domain, and VDV has been used for evaluating the transient ride comfort quality. These vibration metrics, however, cannot determine which condition is stationary and which is transient. This paper thus proposes a new vibration metric that can distinguish the stationary condition from the transient condition, and presents the developed novel ride comfort index for the objective assessment of ride comfort quality based on the new vibration metric.
