      • Prevalence of Abnormal Urodynamic Study Results in Patients with Congenital and Idiopathic Scoliosis and Its Predictive Value for the Diagnosis of Tethered Cord Syndrome: A Single Institution Clinical Study

        Study Design: A total of 110 patients with scoliosis were enrolled in this analytical cross-sectional study.Purpose: We aimed to compare the urodynamic study (UDS) results of patients with idiopathic scoliosis (ISC) and congenital scoliosis (CSC) and to evaluate the clinical significance of abnormal UDS findings in predicting underlying tethered cord syndrome (TCS).Overview of Literature: An abnormal UDS finding is commonly found in patients with CSC. However, there is no consensus regarding its prevalence in patients with ISC.Methods: Using the STROBE checklist for cross-sectional studies, 110 patients with scoliosis were selected based on our inclusion and exclusion criteria. Among the patients, 76 presented with ISC and 34 with CSC. Demographic data and other details, such as the results of spine radiography, UDS, and magnetic resonance imaging of the spine in both supine and prone positions, were recorded and analyzed.Results: Approximately 50% of patients with CSC had normal UDS findings; 8.8%, mild impairment; and 41.2%, significant abnormalities. Moreover, 67.1% of patients with ISC had normal UDS findings; 9.2%, mild impairment; and 23.7%, significant abnormalities (p =0.166). TCS was identified in 38.2% and 26.3% of patients with CSC and ISC, respectively (p =0.571). In patients with ISC, a significantly abnormal UDS finding indicated that the risk of TCS increased from 26.3% to 50% (odds ratio [OR], 4.2; p =0.009). Meanwhile, in patients with CSC, the risk was almost similar (OR, 0.8; p =0.8).Conclusions: Even with the absence of subjective urinary symptoms, subclinical urologic impairments can be observed in a significant number of patients with ISC. An abnormal UDS finding can be a sign of underlying spinal cord tethering in a patient with ISC who is a candidate for corrective spine surgery even though it is an independent variant and is not exclusive to candidates for surgery. This finding has high clinical utility for neuro- and ortho-spine surgeons who aim to correct scoliosis (OR, 4.2; p =0.009).

        Gut microbiome is an integral part of the Gut-Brain axis. It is becoming increasingly recognized that the presence of a healthy and diverse gut microbiota is important to normal cognitive and emotional processing. It was known that altered emotional state and chronic stress can change the composition of gut microbiome, but it is becoming more evident that interaction between gut microbiome and central nervous system is bidirectional. Alteration in the composition of the gut microbiome can potentially lead to increased intestinal permeability and impair the function of the intestinal barrier. Subsequently, neuro-active compounds and metabolites can gain access to the areas within the central nervous system that regulate cognition and emotional responses. Deregulated inflammatory response, promoted by harmful microbiota, can activate the vagal system and impact neuropsychological functions. Some bacteria can produce peptides or short chain fatty acids that can affect gene expression and inflammation within the central nervous system. In this review, we summarize the evidence supporting the role of gut microbiota in modulating neuropsychological functions of the central nervous system and exploring the potential underlying mechanisms.         Quantitative and Semiquantitative Health Risk Assessment of Occupational Exposure to Styrene in a Petrochemical Industry

        Background: Styrene is one of the aromatic compounds used in acetonitrile-butadiene-styrene (ABS) producing petrochemicals, which has an impact on health of workers. Therefore, this study aimed to investigate the health risks of styrene emitted from the petrochemical industry in Iran. Methods: Air samples were collected based on NIOSH 1501 method. The samples were analyzed by the Varian-cp3800 gas chromatograph. Finally, risk levels of styrene's health effects on employees were assessed by the quantitative method of the U.S. Environmental Protection Agency (U.S. EPA) and the semiquantitative way by the Singapore Occupational Safety and Health Association. Results: Based on the results, the employees had the highest average exposure to styrene vapors (4.06 × 10<sup>-1</sup>mg.(kg - day)<sup>-1</sup>) in the polybutadiene latex (PBL) unit. Therefore, the most top predictors of cancer and non-cancer risk were 2.3×10<sup>-4</sup> and 7.26 × 10<sup>-1</sup>, respectively. Given that the lowest average exposure (1.5 × 10<sup>-2</sup>mg.(kg - day)<sup>-1</sup>) was in the dryer unit, the prediction showed a moderate risk of cancer (0.8 × 10<sup>-6</sup>) and non-cancer (2.3 × 10<sup>-3</sup>) for the employees. The EPA method also predicted that there would be a definite cancer risk in 16% and a probable risk in 76% of exposures. However, according to the semiquantitative approach, the rate of risk was at the "low" level for all staff. The results showed that there was a significant difference (p < 0.05) between the units in exposure and health risk of styrene (p < 0.05). Conclusion: Given the high risk of styrene's health effects, appropriate control measures are required to reduce the exposure level.

        Effect of Different Types of Diagnostic Criteria for Gestational Diabetes Mellitus on Adverse Neonatal Outcomes: A Systematic Review, Meta-Analysis, and Meta-Regression

        Background: Evidence supporting various diagnostic criteria for diagnose gestational diabetes mellitus (GDM) are consensus-based, needs for additional evidence related to outcomes. Therefore, the aim of this systematic-review and meta-analysis was to assess the impact of different GDM diagnostic-criteria on the risk of adverse-neonatal-outcomes.Methods: Electronic databases including Scopus, PubMed, and Web of Sciences were searched to retrieve English original, population-based studies with the universal GDM screening approach, up to January-2020. GDM diagnostic criteria were classified in seven groups and International Association of the Diabetes and Pregnancy Study Groups (IADPSG) was considered as reference one. We used the Mantel–Haenszel method to calculate the pooled odds of events. The possibility of publication bias was examined by Begg’s test.Results: A total of 55 population-based studies consisting of 1,604,391 pregnant women with GDM and 7,770,855 non-GDM counterparts were included. Results showed that in all diagnostic-criteria subgroups, the risk of adverse neonatal outcomes including macrosomia, hyperbilirubinemia, respiratory distress syndrome, neonatal hypoglycemia, neonatal intensive care unit admission, preterm birth, and birth-trauma were significantly higher than the non-GDM counterparts were significantly higher than non-GDM counterparts. Meta-regression analysis revealed that the magnitude of neonatal risks in all diagnostic-criteria subgroups are similar.Conclusion: Our results showed that the risk of adverse-neonatal-outcome increased among women with GDM, but the magnitude of risk was not different among those women who were diagnosed through more or less intensive strategies. These findings may help health-care-providers and policy-makers to select the most cost-effective approach for the screening of GDM among pregnant women.

        Effect of production level and source of fat supplement on performance, nutrient digestibility and blood parameters of heat-stressed Holstein cows

        The interactive effect of dietary fat supplementation and milk yield level on dairy cows performance under heat stress has not been thoroughly investigated. The purpose of this study was to evaluate the effect of production level, the source of fat supplements and their interaction on dairy cows performance under heat stress. In this study, 64 Holstein multiparous cows were divided into 2 groups and received one of two rations having either calcium salts of fatty acids (Ca-FA) or high-palmitic acid (PA) supplements (2.8% of DM; dry matter). After completing the experiment and based on maturity-equivalent milk, cows were divided into two groups of high-yielding (14,633 kg) and medium-yielding (11,616 kg). Average temperature humidity index (THI) was 71 during the trial period. Apparent digestibility of dry matter (p = 0.04), organic matter (p = 0.05), and neutral detergent fiber (NDF; p = 0.04) for cows fed Ca-FA were greater than cows fed PA. The milk fat content in high-producing cows was 0.3% greater than medium-producing cows (p = 0.03). The milk protein content in cows fed Ca-FA was greater than cows fed PA (p < 0.01). High-producing cows had greater serum cholesterol (p = 0.02) than medium-producing cows. The cows fed PA tended to have a greater BUN than cows fed Ca-FA (p = 0.06). Alanine aminotransferase and aspartate aminotransferase tended to be increased by PA, which indicates that cows in PA treatment may have experienced more adverse effect on the liver function than cows on Ca-FA. Therefore, under heat stress and in 90 d trial, milk production level does not affect the cows’ response to PA or Ca-FA. Although cows fed Ca-FA received lower energy than those fed PA, they compensated for this shortage likely with increasing the digestibility and produced a similar amount of milk.
