        Approaching Knowledge, Attitudes, and Practices Model for Elderly with Dementia Who are Suspected to Have Hearing Impairment in Korea

        Because the psychophysical symptoms of hearing loss and dementia in the elderly are very similar, untrained healthcare professionals in dementia facilities can easily overlook a severity of hearing loss in their patients. The present study identifies their knowledge, attitudes, and practices (KAP) on hearing loss using a survey whether they may help hearing problem of the patients with dementia. A total of 29 health-care professionals responded to the KAP survey. Also, 2 family members participated. The results showed that most of the nurses and caregivers in elderly medical welfare facilities who worked with dementia patients did not have knowledge of their hearing loss. Even the facility managers did not know how to conduct hearing tests for their patients although they did recognize that some of their patients had a hearing loss. Eventually, actual practice was not possible at this moment by the professionals. However, our respondents did have a positive attitude toward screening for hearing loss and help their patients with dementia wear hearing aids if a clinical guideline was provided. We suggest to develop clear and precise clinical guidelines of the hearing screening test for the dementia patients due to the interrelationship between dementia and hearing loss. When these guidelines apply to elderly residents in a medical welfare facility, early diagnosis and treatment of their sensory loss will help alleviate their dementia as well.

        Application of Acceptable Noise Level to Confirm Fitting Strategy for Bone-Anchored Hearing Aid

        Since there is no standardized fitting procedure for bone-anchored hearing aid (BAHA) patients, many clinics have failed to perform their successful fitting. The present case study tried to identify whether acceptable noise level (ANL) was clinically applied as one of fitting procedures for the BAHA users. Four hearing-impaired adults with BAHA (3 female and 1 male) who had a period of hearing deprivation for at least 8 years and the BAHA experience of more than 1 year participated. They conducted ANL in their unaided and aided BAHA conditions after being familiar with the ANL instruction. Also, the subjects responded to a self-report of Korean version of Profile of Hearing Aid Benefit (K-PHAB) to subjectively check amount of satisfaction with BAHA. Compared to the unaided condition, two out of four BAHA users showed a decreased ANL in the aided condition whereas the other users had a slightly increased ANL. Regardless, their levels were examined within the permissible range except for one subject. Their K-PHAB scores were similar to the ANL results in the aided condition. Although measured ANLs were slightly differed from the users, the current results suggest that ANL might be a tool to measure individual’s inherent value when the BAHA fitting being appropriated while also supported by K-PHAB scores. In the following study, we need to confirm a typical pattern of many BAHA users with longitudinal measures of ANL.

        소음성 난청의 효율적인 예방을 위한 첨도 측정법의 이해

        Purpose: In general, energy metrics, i.e., LAeq 8 hours, have been applied as a risk indicator of noise exposure. However, there are some limitations of LAeq measurement since it considers only the amount of average energy of noise and is not appropriate for measuring the complex noise. In light of this, the kurtosis metrics have been introduced by many contemporary researchers to be applied as an alternative measure. Methods: In the main body, we introduced the weakness of current noise exposure criteria, compared between Gaussian and non-Gaussian noise, briefly explained the concepts of kurtosis metrics, and scrutinized more applications of the kurtosis metrics by summarizing previous 7 research papers. Results: Compare to the Gaussian noise, the non-Gaussian noise has been found to have stronger cochlear damage effects on the outer and inner hair cells and led to permanent hearing loss in the higher frequencies. Noise exposure trauma levels were dramatically increased as the kurtosis levels increased. Although kurtosis metrics are a good indicator or measure, it still need to be adjusted in terms of frequency-specificity. Conclusion: Since the kurtosis metrics are not implemented in Korea, especially in the industrial field, we need to develop a new measurement approach using the kurtosis metrics on workers and then provide scientific based criteria for effectively preventing their potential risk of developing noise-induced hearing loss.
