Dental management of Lowe syndrome under general anesthesia in a child: A case report

Lowe syndrome, also known as oculocerebrorenal syndrome, is a rare X-linked recessive disorder characterized by a combination of abnormalities associated with ocular (congenital cataracts), nervous system (mental impairment and hypotonia), and renal dysfunction. Dental crowding, over-retained primary teeth, ectopic and delayed eruption of permanent dentition, and taurodontism have been reported as oral findings. Dental care for young patients with Lowe syndrome is complicated by difficulties in behavior management and other systemic conditions of these patients. General anesthesia may be an appropriate and effective option to eliminate the need for repeated visits to the hospital and to provide improved treatment quality for the patient. However, this approach should be used with caution because general anesthesia may induce malignant hyperthermia and metabolic acidosis in patients with Lowe syndrome. This case study reports the dental management of Lowe syndrome in a 4-yearold male patient who presented with multiple caries in his primary dentition under general anesthesia.

본 연구의 목적은 노동시장에 참여하고 있는 한국 청년의 불안정성이 얼마나 심각하게, 어떤 양상으로 나타나고 있는지 분석하는 것이다. 이를 위해 불안정성을 개념화하고 고용관계, 소득 그리고 사회보험 세 가지 측면의 불안정성을 모두 고려하는 통합적(holistic) 접근을 통해 노동시장의 불안정성을 측정하였다. 본 연구는 만 19세~34세의 청년 중 순수비경제활동인구만을 제외하고 임금근로자, 비임금근로자, 비경제활동인구 중 잠재실업자, 취업준비생, 니트족, 구직포기자까지 포함한 모든 청년들을 대상으로 하여 이들이 노동시장에서 겪는 불안정성을 분석하였다. 분석 결과, ‘매우 불안정한’ 청년과 ‘불안정하지 않은’ 청년의 규모가 증가하여 청년노동시장의 불안정성이 양극화 되는 경향을 볼 수 있었고 청년들의 불안정성에는 특히 소득 불안정성이 큰 영향을 미치고 있는 것으로 분석되었다. 패널로짓 분석을 통해 청년들이 불안정 집단에 속할 확률을 분석한 결과, 청년노동시장에서 가장 불안정하고 열악한 환경에 노출될 가능성이 큰 집단은 남성, 저연령, 저학력 집단이었고 직업적으로는 단순노무종사자, 서비스 종사자, 판매 종사자, 산업적으로는 교육 서비스업, 숙박ㆍ음식점업의 불안정 가능성이 가장 높게 나타났다. 따라서 본 연구는 중소기업 인턴제 등과 같은 고용 및 취업촉진 정책을 면밀히 재고해 볼 필요가 있으며 서비스업 일자리가 증가하는 작금의 상황에서 일자리 질 개선 논의가 선차적으로 필요함을 주장한다. This research aims to analyze precariousness of youth in labour market in Korea. To study how severe their precariousness is and how it appears, we measure the precariousness in the labour market by using holistic approach which considers all three attributes -employment relationship, wage or income, and social insurance. We study all youth aged 19 to 34 years old, including wage workers, non-wage workers, and involuntary unemployed, but excluding voluntarily economically inactive. As a result, we find polarization of precariousness of youth as ‘highly precarious’ and ‘not precarious’ youth have been increased. In addition, according to the result of panel logit anlaysis, the most precarious group consists of male, younger and under-educated youth with elementary jobs, service jobs or sales jobs and with education service, housing and food service sectors. Therefore, policies which aims to promote just employment rate such as small-medium firms internship should be reconsidered and especially for service workers, quality of jobs should be improved in the service economy that the number of jobs in the sector keeps increasing.

PRS는 소하악증, 설하수증, 구개열의 세가지 임상적 특징을 갖는 질환으로 상기도 폐쇄로 인한 호흡곤란, 섭식장애를 나타낸다. PRS 환아는 치과 치료 시 호흡 관리에 대한 고려가 필요하고 또한 다양한 증후군이 동반될 수 있어 치과 치료시 전신적인 상태에 대한 고려가 필요하다. 본 증례에서는 다발성 치아우식증을 주소로 내원한 PRS 환자에 대한 보고로 기도 확보 및 유지의 어려움이 예상되었으나 치과적 치료를 전신마취하 성공적으로 시행되었다. Pierre Robin sequence(PRS) is characterized by a triad of clinical signs: micrognathia, glossoptosis and cleft palate. These anatomical deformities of PRS predispose patients to respiratory problems and feeding difficulties at birth. Maintaining oral hygiene and enduring dental treatment are complicated by their general conditions of PRS patients. We present a case of dental treatment of PRS patient under general anesthesia. A 3-year-old boy with PRS visited Seoul National University Dental Hospital for caries treatment. Clinical and radiographic examinations revealed multiple carious lesions. Considering the patient’s medical condition and compliance of treatment, dental treatment under general anesthesia was decided. Despite expected challenges of managing the airway of the patient, intubation was performed successfully. The patient was treated with pulp treatments and restorations using composite resin and stainless steel crowns. No complications were observed during and after the procedure.
