        Short-term effects of joint mobilization with versus without voluntary movement in patients with chronic ankle instability: A single-blind randomized controlled trial

        Objective: Joint mobilization for arthrokinematics altered by the positional fault of chronic ankle instability (CAI) is an effective intervention for stabilization. In this study, we compared the effects of ankle dorsi flexion range of motion (DFROM) and dynamic balance ability (DBA) in CAI patients via passive joint mobilization (PJM), a method traditionally performed in previous studies, and active joint mobilization (AJM), a method that can have a greater effect on cortical excitability with spontaneous movements. Design: Single-blind two-arm randomized controlled trial Methods: A total of 30 participants were registered: 15 each to the PJM and AJM groups. Each participant received a total of 10 intervention sessions, 10 minutes per session, 5 times a week for 2 weeks. PJM used Maitland s mobilization method to apply joint mobilization with talus in the posterior direction and AJM used an angular joint motion to induce patient s voluntary motion of medial malleolus anterior gliding and lateral malleolus posterior gliding, respectively. DFROM of the ankle was measured by using tape and DBA was evaluated by using the balance system. Results: Significant improvement was observed after intervention in both the PJM and AJM groups except for the DBA-anterior and DBA-right variables of the PJM group. There were statistically significant differences between the AJM and PJM groups in the DFROM, DBA-anterior, DBA-posterior, and DBA-right variables. Conclusions: The overall improvement of DFROM and DBA was found to be more effective in joint mobilization including voluntary movement. When it is accompanied by voluntary movement, it further affects the neuromuscular system of the ankle.

      • 점탄성 특성을 반영한 수치해석에 의한 아스팔트 노반궤도의 거동분석

        노반의 지지력 및 내구성, 주행쾌적성을 개선 시키기 위하여 아스팔트 강화노반의 도입이 고려되고 있다. 본 연구에서는 다층탄성해석 프로그램인 KENTRACK과 유한요소해석 프로그램인 ABAQUS를 이용하여 아스팔트 강화노반의 계절별 온도변화에 따른 변위 및 거동 특성에 대한 분석을 실시하였다. ABAQUS에 의한 분석시 궤도의 기하학적인 실제구조를 반영하기 위하여 3-D로 모델링 하였으며, 특히 최적 아스팔트 강화노반의 거동분석을 위하여 점탄성(Visco-Elastic) 모델을 적용하였다. 수치해석을 통하여 아스팔트의 물성변화에 따른 아스팔트층 하면에서의 인장변형률과 흙노반 상면에서의 소성변형 등 파손에 대한 민감도를 추적, 조사하여 설계수명 예측모델의 수립가능성을 탐색하였으며 아스팔트 강화노반의 적용성을 평가하였다. In order to increase bearing stiffness and durabilty of, and drivability on the track structure, asphalt trackbed foundation is introduced in design. In this study, behavior of the asphalt track structure is analyzed using a commercial finite element program, ABAQUS, and KENTRACK with changing temperature. A 3-D modeling with visco-elastic constitutive model for asphalt mix is considered in analyzing the structure in case of ABAQUS FE analysis. Critical points such as vertical sresses on the sugrade and tensile stresses at the bottom of the asphalt trackbed foundation is investigated in order to get information and design paramters in this kind of new trackbed structure.

        Effect of Cytoskeletal Manual Therapy, a Novel Soft Tissue Mobilization Technique, on Axillary Web Syndrome after Axillary Lymph Node Dissection: A Case Report

        Objective: Axillary web syndrome (AWS) is a condition comprising fibrous band-like cords that appear in the axilla of patients after axillary lymph node dissection (ALND) during breast cancer surgery and result in pain and reduced mobility. The cords appearing with AWS are hardened veins or lymphatic vessels. Manual therapy and stretching are recommended for pain control and mobility improvement. Therefore, this study investigated the effect of cytoskeletal manual therapy (CMT), which is a new soft tissue mobilization technique. Design: A case reportMethods: A 41-year-old woman with AWS after breast cancer surgery and ALND visited a physical therapy clinic because of shoulder pain, decreased function, and decreased mobility. The cords were palpable and pain occurred 2 weeks after surgery. CMT was performed three times per week for a total of 6 weeks. Her pain intensity, range of motion (ROM), and shoulder function were measured. Results: Measurements were performed after 2 weeks and 6 weeks of CMT and evaluated using the numeric pain rating scale (NPRS). Her pain intensity largely decreased after 2 weeks (4-point score reduction) and after 6 weeks (5-point score reduction) of CMT. After CMT, her full ROM was restored and her shoulder function was improved (7-point score reduction). Conclusions: CMT is effective for pain control, mobility improvement, and functional improvement of patients with AWS.

      • 3D 유한요소해석에 의한 직결 아스팔트 노반 궤도의 설계수명 예측

        본 연구에서는 직결 아스팔트 노반 궤도의 최적 두께 결정을 위한 설계차트를 개발하기 위하여 주요 설계인자로서 궤도구조형식, 열차 하중(윤중) 및 속도, 년간통과톤수(MGT), 재료 물성(흙노반 및 강화노반 탄성계수), 각 층구조별 두께(강화노반 및 아스팔트노반) 등을 설정하였으며 이들 설계인자의 조합을 달리하면서 윤중 재하시 아스팔트 노반하면에서의 인장변형률 및 흙노반 상면에서의 수직변형률을 ABAQUS 프로그램을 이용한 3D 구조해석을 통해 분석하였다. 구조해석 분석결과와 손상도 이론에 의한 Miner법칙을 활용하여 임의 직결 아스팔트 궤도구조에 대응하는 설계수명(내구년한) 결정을 위한 설계차트를 개발하였다. In this study, primary design parameters such as types of track sections, wheel load size, velocity, MGT, elastic modulus and thickness of each layers are defined. Full 3D Finite element analyses were performed using ABAQUS program in order to calculate tensile strains under asphalt layer and vertical stresses at top of subgrade with different values of design parameters defined. Using Miners" theory with computed structural analyses results, design charts to decide design life of the asphalt trackbed foudnation are produced.
