        Evaluation of efficacy of Valsalva maneuver for attenuating propofol injection pain: a prospective, randomized, single blind, placebo controlled study

        Background: Pain on injection is a limitation with propofol use. The effect of the Valsalva maneuver on pain during propofol injection has not been studied. This maneuver reduces pain through the sinoaortic baroreceptor reflex arc and by distraction. We aimed to assess the efficacy of the Valsalva maneuver in reducing pain during propofol injection. Methods: Eighty American Society of Anesthesiologists class I adult patients undergoing general anesthesia were enrolled and divided into two groups of 40 each. Group I (Valsalva) patients blew into a sphygmomanometer tube raising the mercury column up to 30 mmHg for 20 seconds, while Group II (Control) patients did not. Anesthesia was induced with 1% propofol immediately afterwards. Pain was assessed on a 10-point visual analog scale (VAS), where 0 represented no pain, and 10, the worst imaginable pain, and a 4-point withdrawal response score, where 0 represented no pain, and 3, the worst imaginable pain. Scores were presented as median (interquartile range). Results: We analyzed the data of 70 patients. The incidence of pain was significantly lower in the Valsalva than in the control group (53% vs. 78%, P = 0.029). The withdrawal response score was significantly lower in the Valsalva group (1.00 [0.00–1.00] vs. 2.00 [2.00–3.00], P < 0.001). The VAS score was significantly lower in the Valsalva group (1.00 [0.00–4.00] vs. 7.00 [6.25–8.00], P < 0.001). Conclusions: A prior Valsalva maneuver is effective in attenuating injection pain due to propofol; it is advantageous in being a non-pharmacological, safe, easy, and time-effective technique.

        Human being are greatly inspired by nature. Nature has the ability to solve very complex problems in its own distinctive way. The problems around us are becoming more and more complex in the real time and at the same instance our mother nature is guiding us to solve these natural problems. Nature gives some of the logical and effective ways to find solution to these problems. Nature acts as an optimizer for solving the complex problems. In this paper, the algorithms which are discussed imitate the processes running in nature. And due to this these process are named as “Nature Inspired Algorithms”. The algorithms inspired from human body and its working and the algorithms inspired from the working of groups of social agents like ants, bees, and insects are the two classes of solving such Problems. This emerging new era is highly unexplored young for the research. This paper proposes the high scope for the development of new, better and efficient techniques and application in this area.

      • Utility of Parametric Curves in Image Processing Applications

        Image Processing is dealing with images containing lots of points. By Interpolation of these points to form different curves are taken under consideration in this paper. The curve modification and fitting in image processing can use Bezier Curve applications in various ways and by modifying these curves better result are generated. Bezier curve and its applications have vast scope for research in image processing like curve fitting algorithm, pattern recognition and computer aided geometric design. Application of Bezier curve are widely implemented in face detection, smile detection, hand writing and digit recognition which give wide view of Bezier curves. In this paper we go through various concepts and its application which can be used in image processing for providing extra edge in this field.

      • Automation of Segmentation of MRI Images in the Presence of Intensity Inhomogeneity by the Application of Probabilistic Neural Network

        This paper presents a different approach by employing probabilistic neural network (PNN) with level set method for segmentation of intensity inhomogeneous magnetic resonance images (MRI). The input image is classified by PNN, and initialise the image with initial zero level set contours. A cascade approach is used in which after processing image by PNN, the output of PNN is fed into Level Set Method (LSM) Algorithm. The LSM is usedto perform the segmentation and to estimate bias field.

      • Face Recognition Using Approximated Bezier Curve and Supervised Learning Approach

        Bezier curve are very strong for variety of application. Specifically in image processing it applies to object recognition, face recognition, and human gait recognition. It also works on fingerprint and other biometric system recognition. This paper presents a work to recognize digital images of human frontal faces using the approximated Bezier curve and an intelligent process of learning using neural network. The main structural features of faces like eye, eyebrows, nose, lips, and Face boundaries are extracted and using minimum of these features face is recognized efficiently.

        Palliative Radiotherapy in the Presence of Well-Controlled Metastatic Disease after Initial Chemotherapy May Prolong Survival in Patients with Metastatic Esophageal and Gastric Cancer

        Purpose We report the outcomes of patients treated with palliative radiotherapy (pRT) to the primarytumour in the context of well-controlled metastatic disease after initial chemotherapy. Materials and MethodsClinical records of 132 patients with metastatic esophago-gastric (OG) cancer treated withpalliative chemotherapy (pCT) between January 2009 and June 2013 were reviewed. Ninetysevenpatients had responding or stable disease after 3 months of chemotherapy, of whom53 patients received pRT to the primary tumour after initial chemotherapy in the presenceof well-controlled metastatic disease (group A, pCT-RT). The remaining 44 patients weretreated with pCT alone (group B, pCT). Treatment-related outcomes were assessed in abovegroups including time to local progression (TTLP), progression-free and overall survival. ResultsThe median overall survival for patients treated with pRT after initial chemotherapy (groupA) was 23.3 months (95% confidence interval [CI], 17.70 to 28.89 months) and significantlyhigher than the 14 months (95% CI, 10.91 to 17.08 months) in patients treated with pCTalone (group B) (p < 0.001). The use of pCT-RT was an independent predictor of OS in multivariateanalysis. Local recurrence was observed in 12/53 of patients (23%) in group Acompared to 16/44 (36%) in group B. The median TTLP was significantly higher in patientsafter pCT-RT at 17.3 months (5.23 months to 44.50 months) compared to 8.3 months(range, 4.10 to 25.23 months) in patients treated with pCT alone (p=0.006). ConclusionThe possibility of pRT influencing systemic disease in advanced OG cancer has not beenreported, and results from the present study present strong arguments for investigation ofthis therapeutic strategy in a randomized trial.
