Recently reduction of allophone of motors for automobiles has received a great deal of attention. For this background, the final goal of our research is to develop a simulator for transfer characteristics of allophone of motors in automobiles. In this paper at the preparation phase for the development of a simulator, we investigated the applicability of the transfer function synthesis method (TFSM) experimentally. Additionally, we examined some problems to be solved for practical use of the TFSM by using the finite element method (FEM). As a result, it was found that the derivation of the transfer functions H22 and H33 at the connecting point was difficult and that the modeling accuracy of H22 and H33 had a large influence on the modeling accuracy of the total transfer function H41_sim. Moreover, the method to improve the modeling accuracy was proposed on the basis of the results by FEM analysis. It was confirmed that we could apply the TFSM to the simple test body with the contact area by using the proposed method and the adjustment for damping coefficient of the transfer function obtained by the curve fitting technique (CFT).
Ultrasonically-assisted Hydrothermal Method for Ferroelectric Material Synthesis

The hydrothermal method enables the production of high-quality piezoelectric materials. In this study, we propose to irradiate the reaction solutions with ultrasonic power during the hydrothermal method to obtain a shorter reaction time and a smooth film surface. A high-pressure reaction container for the ultrasonic transducer was newly developed, and the ultrasonically-assisted hydrothermal method was examined by using this container. The effect of ultrasonic assist on the synthesis of lead-zirconate-titanate (PZT) thin films and (K,Na)NbO3 powders was verified. Thicker PZT film, thickness around 10 µm, could be obtained in one process, and (K,Na)NbO3 powder was synthesized in half the previous reaction time.

Vertebral artery stump syndrome (VASS) is a rare condition that can cause posterior circulation ischemic stroke due to occlusion of the ipsilateral vertebral artery (VA) orifice, resulting in blood flow stagnation and embolus formation. Although there is no established treatment for this condition, we observed 3 cases of VASS out of 326 acute ischemic stroke cases at a single institution from April 2021 to October 2022. Despite the best possible antithrombotic treatment, all 3 patients had recurrent ischemic strokes. One patient underwent drug-eluting stenting of the VA orifice to relieve occlusive flow. The other 2 patients received coil embolization, which resulted in the disappearance of their culprit collateral flow. None of the patients had recurrent ischemic strokes after endovascular intervention. Based on our observations, stenting and coil embolization are effective methods for preventing future recurrences of VASS.
Ultrasonic Transducer for the Hydrothermal Method

Direct ultrasound irradiation is advantageous for increasing the efficiency of the hydrothermal method, which can be used to produce piezoelectric thin films and lead-free piezoelectric ceramics. To apply ultrasound directly to the process, transducer prototypes were developed regarding the boundary conditions of the hydrothermal method. LiNbO3 and PIC 181 were proven to be feasible materials for high-temperature-resistant transducers (≥200 ℃). The resistance of the transducer’s horn against a corrosive mineralizer was achieved by using Hastelloy C-22. The efficiency of the ultrasound-assisted hydrothermal method depends on the generated sound field.The impedance and the sound field measurements have shown that the sound field depends on the filling level and on the position and design of the transducer.

Daikenchuto (TJ-100) is a Japanese traditional herbal medicine that enhances postoperative enterokinesis and increases the intestinal blood flow. Daikenchuto has historically played a positive role in the perioperative management of gastrointestinal surgery. However, the effect of Daikenchuto on improving nutrition after cardiac operation has never been reported. This study evaluated the effect of Daikenchuto administration after cardiac operation. Eighty-four patients who underwent isolated first aortic valve replacement (AVR) at our department from May 2008 to December 2014 were enrolled in this study and retrospectively examined. We administered Daikenchuto when patients started their water intake. Blood tests were performed before and seven days after AVR. We prescribed Daikenchuto for 21 patients (group D). The reductions in the albumin level and prognostic nutritional index (p = 0.045, p = 0.008, respectively, multivariate analysis of variance) after the operation were significantly lower in group D (3.75 ± 0.3 to 3.33 ± 0.4 and 44.7 ± 4.8 to 41.3 ± 6.7 mg/dl, respectively) than in the group without Daikenchuto (group N, 63 patients) (3.74 ± 0.5 to 3.13 ± 0.4 and 44.6 ± 6.3 to 37.7 ± 5.3 mg/dl, respectively). There were no significant differences in the incidence rates of postoperative complications or hospitalization duration between the two groups. Postoperative Daikenchuto administration is considered to be safe and effective for improving the postoperative nutritional status after simple AVR as well as digestive surgery.

Objective: To examine the association between the mobilization level during intensive care unit (ICU) admission and independence in activity of daily living (ADL), defined as Barthel Index (BI)≥70. Methods: This was a post-hoc analysis of the EMPICS study involving nine hospitals. Consecutive patients who spend >48 hours in the ICU were eligible for inclusion. Mobilization was performed at each hospital according to the shared protocol and the highest ICU mobility score (IMS) during the ICU stay, baseline characteristics, and BI at hospital discharge. Multiple logistic regression analysis, adjusted for baseline characteristics, was used to deter-mine the association between the highest IMS (using the receiver operating characteristic [ROC]) and ADL. Results: Of the 203 patients, 143 were assigned to the ADL independence group and 60 to the ADL dependence group. The highest IMS score was significantly higher in the ADL independence group than in the dependence group and was a predictor of ADL independence at hospital discharge (odds ratio, 1.22; 95% confidence interval, 1.07–1.38; adjusted p=0.002). The ROC cutoff value for the highest IMS was 6 (specificity, 0.67; sensitivity, 0.70; area under the curve, 0.69). Conclusion: These results indicate that, in patients who were in the ICU for more than 48 hours, that patients with good function in the ICU also exhibit good function upon discharge. However, prospective, multicenter trials are needed to confirm this conclusion.
A Two-Dimensional Study of Transonic Flow Characteristics in Steam Control Valve for Power Plant

A steam control valve is used to control the flow from the steam generator to the steam turbine in thermal and nuclear power plants. During startup and shutdown of the plant, the steam control valve is operated under a partial flow conditions. In such conditions, the valve opening is small and the pressure deference across the valve is large. As a result, the flow downstream of the valve is composed of separated unsteady transonic jets. Such flow patterns often cause undesirable large unsteady fluid force on the valve head and downstream pipe system. In the present study, various flow patterns are investigated in order to understand the characteristics of the unsteady flow around the valve. Experiments are carried out with simplified two-dimensional valve models. Two-dimensional unsteady flow simulations are conducted in order to understand the experimental results in detail. Scale effects on the flow characteristics are also examined. Results show three types of oscillating flow pattern and three types of static flow patterns.
