Backgrounds/Aims: Pancreaticoduodenectomy (PD) is the only radical treatment for periampullary malignancies. Superior mesenteric artery (SMA) first approach combined with total meso-pancreas (MP) excision was conducted to improve the oncological results. There has not been any previous research of a technique that combines the SMA first approach and total MP excision with a detailed description of the MP macroscopical shape. Methods: We prospectively assessed 77 patients with periampullary malignancies between October 2020 and March 2022 (18 months). All patients had undergone PD with SMA first approach combined total MP excision. The perioperative indications, clinical data, intra-operative index, R0 resection rate of postoperative pathological specimens (especially mesopancreatic margin), postoperative complications, and follow-up results were evaluated. Results: The median operative time was 289.6 min (178−540 min), the median intraoperative blood loss was 209 mL (30−1,600 mL). Microscopically, there were 19 (24.7%) cases with metastatic MP, and five cases (6.5%) with R1-resection of the MP. The number of lymph nodes (LNs) harvested and metastatic LNs were 27.2 (maximum was 74) and 1.8 (maximum was 16), respectively. Some (46.8%) patients had pancreatic fistula, but mostly in grade A, with 7 patients (9.1%) who required re-operations. Some 18.2% of cases developed postoperative refractory diarrhea. The rate of in-hospital mortality was 1.3%. Conclusions: The PD with SMA first approach combined TMpE for periampullary malignancies was effective in achieving superior oncological statistics (rate of MP R0-resection and number of total resected LNs) with non-inferior short-term outcomes. It is necessary to evaluate survival outcomes with long-term follow-up.

Backgrounds/Aims: Parenchymal-sparing anatomical hepatectomy (Ps–AH) based on portal ramification of the right anterior section (RAS) is a new technique to avoid unnecessarily transecting too much liver parenchyma, especially in cases of major anatomical hepatectomy. Methods: We prospectively assessed 26 patients with primary hepatic malignancies having undergone major Ps–AH based on portal ramification of the RAS from August 2018 to August 2022 (48 months). The perioperative indications, clinical data, intra-operative index, pathological postoperative specimens, postoperative complications, and follow-up results were retrospectively evaluated. Results: Among the 26 patients analyzed, there was just one case that had intrahepatic cholangiocarcinoma The preoperative level of α– Fetoprotein was 25.2 ng/mL. All cases (100%) had Child–Pugh A liver function preoperatively. The ventral/dorsal RAS was preserved in 19 and 7 patients, respectively. The mean surgical margin was 6.2 mm. The mean surgical time was 228.5 minutes, while the mean blood loss was 255 mL. In pathology, 5 cases (19.2%) had microvascular invasion, and in the group of HCC patients, 92% of all cases had moderate or poor tumor differentiation. Six cases (23.1%) of postoperative complications were graded over III according to the Clavien–Dindo system, including in three patients resistant ascites or intra-abdominal abscess that required intervention. Conclusions: Parenchymal-sparing anatomical hepatectomy based on portal ramification of the RAS to achieve R0-resection was safe and effective, with favorable short-term outcomes. This technique can be used widely in clinical practice.

Purpose The aim of this study was to investigate whether standard uptake values (SUVs) of pretreatment 18F-FDG PET/CT were the surrogate parameters for predicting the outcomes in locally advanced esophageal squamous cell carcinoma patients treated with definitive chemoradiotherapy. Materials and Methods Sixty patients with esophageal squamous cell carcinoma underwent pretreatment 18F-FDG PET/CT and received definitive chemoradiotherapy. 18F-FDG metabolic parameters including SUVmax, SUVmean, SULpeak, total lesion glycolysis (TLG), and metabolic tumor volume (MTV) of primary tumor were calculated. The receiver-operating characteristic (ROC) curve was used to determine the optimal cutoff value of FDG PET/CT-derived parameters that associated with treatment response. Estimating progression-free survival (PFS) and overall survival (OS) was analyzed by using Kaplan–Meier methods. Univariate and multivariate analysis for PFS and OS was performed using Cox regression. Results Complete response was achieved in 38.3%. The 4-year OS and PFS rates were 48.6% and 44.4%, respectively. SUVmean with a cutoff value of 6.1 could predict complete response with sensitivity of 69.6%, specificity of 78.4%, and accuracy of 75%. Cox multi-factor regression analyses revealed SUVmean > 6.1 as an independent prognostic factor for OS (HR = 6.74, p = 0.02) and PFS (HR = 6.53, p < 0.001). Conclusions Our study suggests that SUVmean of the primary tumor in pretreatment 18F-FDG PET/CT may be used as an independent predictor in esophageal squamous cell carcinoma patients treated with definitive chemoradiotherapy.

In this paper, an air collector with sensible heat storage is numerically solved using the finite difference method. The change in temperatures of the glass, airflow, and absorber plate is assumed to be unidirectional. Meanwhile, heat conduction in the storage material is considered in two dimensions. Six storage materials containing brick, granite, slag, limestone, sandstone, and concrete are simulated unsteadily. Numerical techniques, including an implicit method, an upwind scheme, the Thomas algorithm, and the ADI method, are used to solve a system of differential equations describing the collector. The charging and discharging mechanisms are effectively explained by the two-dimensional temperature distribution of the storage material. Results indicate that the charge and discharge periods are up to 16:00 PM and 24:00 PM, respectively, demonstrating an air temperature difference of 4.5 °C for the granite thickness of 5 cm. Granite and slag are suitable storage materials due to their superior thermal properties. The air channel height has a significant effect on the performance of the collector. The air temperature difference doubles when the channel height is reduced from 5 cm to 2 cm.

<P><B>Abstract</B></P><P>In this article, thermally conductive and relatively low dielectric constant polymer matrix composites of an aluminum nitride filler (AlN) and a novel liquid crystalline copoly(ester amide) (LCP) were prepared via a solution blending method in the presence of a phosphate containing dispersant. The viscosities, thermal conductivities, and dielectric properties of the prepared AlN/LCP composites were investigated as a function of AlN loading. Our experimental results demonstrated that the AIN/LCP composite with AlN concentration of 50 wt% had 2.5 times higher thermal conductivity than pure LCP (2.020 and 0.817 W/mK for composite with 50 wt% of AlN and pure LCP, respectively), but its dielectric constant remained at low level, i.e., < 9.0 at frequency of 900 Hz. In addition, viscosities of AlN/LCP pastes in the <I>N</I>‐methyl‐2‐pyrrolidinone solvent remained at acceptable levels with the high AlN loading of 50 wt%. Liver venous deprivation (LVD) is considered to be a safe and effective method that induces more rapid and important liver hypertrophy before major hepatectomy in comparison with portal vein embolization (PVE) alone. Bleeding complications after LVD or PVE are rare, but can be a life-threatening event. Herein, we report a case of perihepatic and extended subcapsular hematoma of the liver after LVD. Transarterial embolization was performed using gelfoam to control the hemorrhage; however, the patient developed irreversible liver failure and passed away after 38 days of treatment. In patients with LVD or PVE, severe post-procedural bleeding poses a substantial challenge for treatment. Arterial embolization should be carefully considered to avoid liver failure and even death, regardless of whether temporary embolization is used.

This study examines the impact of CSR and its authenticity on customer loyalty through the brand image in the chain coffee shop industry. Using qualitative and quantitative methods, 601 survey questionnaires were distributed, of which 491 were analyzed. The results show that CSR has a positive impact on brand image and customer loyalty. In addition, authenticity does not moderate the impact of CSR on brand image and loyalty, as it is difficult for customers to verify the authenticity of CSR action programs. This study emphasizes the importance of CSR action programs and authenticity for businesses to sustainably enhance their distinctive brand image and customer loyalty. Therefore, for sustainable development in the future, managers of coffee shop chains need to focus on the following issues. First, the most important thing is the right awareness of businesses regarding CSR and CSR authenticity. Third, businesses should strengthen the transparency of CSR action programs in various media so that consumers can easily verify authenticity, to increase brand image and improve customer loyalty.
