Purpose: General surgeons at regional hospitals should have the primary trauma care skills necessary to treat critically ill trauma patients to withstand transfer. This study was conducted to identify a consensus on primary trauma care skills for general surgeons. Methods: An initial list of acute care surgical skills was compiled, and revised by six trauma experts (acute care surgeons); 33 skills were nominated for inclusion in the Delphi consensus survey. Participants (councilors of the Japanese Society for Acute Care Surgery) were presented with the list of 33 trauma care skills and were asked (using web-based software) to rate how strongly they agreed or disagreed (using a 5-point Likert scale) with the necessity of each skill for a general surgeon. The reliability of consensus was predefined as Cronbach’s α ≥ 0.8, and trauma care skills were considered as primarily required when rated 4 (agree) or 5 (strongly agree) by ≥ 80% participants. Results: There were 117 trauma care specialists contacted to participate in the Delphi consensus survey panel. In the 1st round, 85 specialists participated (response rate: 72.6%). In the 2nd round, 66 specialists participated (response rate: 77.6%). Consensus was achieved after two rounds, reliability using Cronbach’s α was 0.94, and 34 items were identified as primary trauma care skills needed by general surgeons. Conclusion: A consensus-based list of trauma care skills required by general surgeons was developed. This list can facilitate the development of a new trauma training course which has been optimized for general surgeons.

<P>Summary</P><P>Most aerial parts of the plant body are products of the continuous activity of the shoot apical meristem (SAM). Leaves are the major component of the aerial plant body, and their temporal and spatial distribution mainly determines shoot architecture. Here we report the identification of the rice gene <I>PLASTOCHRON3</I> (<I>PLA3</I>)/<I>GOLIATH</I> (<I>GO</I>) that regulates various developmental processes including the rate of leaf initiation (the plastochron). <I>PLA3</I>/<I>GO</I> encodes a glutamate carboxypeptidase, which is thought to catabolize small acidic peptides and produce small signaling molecules. <I>pla3</I> exhibits similar phenotypes to <I>pla1</I> and <I>pla2</I>– a shortened plastochron, precocious leaf maturation and rachis branch-to-shoot conversion in the reproductive phase. However, in contrast to <I>pla1</I> and <I>pla2</I>, <I>pla3</I> showed pleiotropic phenotypes including enlarged embryo, seed vivipary, defects in SAM maintenance and aberrant leaf morphology. Consistent with these pleiotropic phenotypes, <I>PLA3</I> is expressed in the whole plant body, and is involved in plant hormone homeostasis. Double mutant analysis revealed that <I>PLA1</I>, <I>PLA2</I> and <I>PLA3</I> are regulated independently but function redundantly. Our results suggest that PLA3 modulates various signaling pathways associated with a number of developmental processes.</P>

This is the first comparative study to examine two different background monitoring sites in Japan to compare differences in the concentration and composition of long-range-transported polycyclic aromatic hydrocarbons (PAHs) from the Asian continent. This study chose the Noto Peninsula (the Kanazawa University Wajima Air Monitoring Station; WAMS) and Fukue Island (the Fukue-jima Atmosphere and Aerosol Monitoring Station; FAMS) to compare 10 weekly periods in 2009 and 2010 from autumn to spring, seasons that are strongly influenced by the Asian continent. The PAHs concentration differed significantly for most periods at these two sites. The backward trajectory analysis found that the low height of the air mass may cause the low concentration and the similar air mass condition could result in the similar concentration. The concentration of long-range-transported PAHs depended more on the source of the coal combustion areas such as Northeast China in heating period.
