How to Prevent Anastomotic Leak in Colorectal Surgery? A Systematic Review

Anastomosis leakage (AL) after colorectal surgery is an embarrassing problem. It is associated with poor consequence. This review aims to summarize published evidence on prevention of AL after colorectal surgery and provide recommendations according to the Oxford Centre for Evidence-Based Medicine. We conducted bibliographic research on January 15, 2020, of PubMed, Cochrane Library, Embase, Scopus, and Google Scholar. We retained meta-analysis, reviews, and randomized clinical trials. We concluded that mechanical bowel preparation did not reduce AL. It seems that oral antibiotic or oral antibiotic with mechanical bowel preparation could reduce the risk of AL. The surgical approach did not affect the AL rate. The low ligation of the inferior mesenteric artery could reduce the AL rate. The mechanical anastomosis is superior to hand-sewn anastomosis only in case of right colectomies, with similar results in rectal surgery between the 2 anastomosis techniques. In the case of right colectomies, this anastomosis could be performed intracorporeally or extracorporeally with similar outcomes. The air leak test did not reduce AL. There is no interest of external drainage in colonic surgery but drains reduced the rate of AL and rate of reoperation after low anterior resection. The transanal tube reduced the rate of AL.
Co‑simulation Improvement for Uncertain Flexible Robot Arm

Flexible robot arm driven by Brushless DC Motor (BDCM) under uncertainties represents one of the most complex and heterogeneous system. Indeed, the verifcation phase becomes a great challenge for designers. Avoid and predict risks accurately at earlier stage represents the main purpose of the Computer-Aided Design (CAD) feld. This paper treats the case of robotics system for tracking trajectory problem and attempts to improve the verifcation phase by identifying the most suitable co-simulation technique. For the system analyzed in this paper, the fexible robot arm driven by Brushless DC actuator is verifed using the Model In the Loop (MIL) technique, the Software In the Loop (SIL) technique and CODIS+technique. Each one verifes the system according to a particular abstraction level. The performance of each technique is determined by measuring the accuracy and time simulation. Experimental results have revealed that CODIS+is the most adequate technique for fexible robot arm, outperforming MIL and SIL by 4–5 times.

Rosa canina is a well-known medicinal plant used in folk remedy that alleviates various disorders, including inflammation, gastritis, and diarrhea. The objective of this investigation was to identify and quantify the phenolic components of R. canina methanolic extract (RCME) and to determine its protective action with dextran sulfate sodium (DSS)-generated mice colitis model. RCME chemical analysis was done using Liquid Chromatography–Electrospray Ionization–Tandem Mass Spectrometry, and experimental animals received RCME at different doses before colitis induction by oral DSS administration during 7 days. Another group received sulfasalazine as a positive control. Colitis damages and RCME benefits were assessed using histopathological and biochemical changes and improvements. Many phenolic compounds have been identified. In addition, the DSS intoxication induced an alteration of colonic epithelium associated with an oxidative stress state. DSS administration led to an increase or decrease of intracellular mediators such as free iron and ionizable calcium. RCME consumption effectively protected against colonic histological/biochemical alterations induced by DSS intoxication providing support for the traditional use of this plant.
