Irreversible electroporation for the treatment of pancreatic neuroendocrine tumors

Backgrounds/Aims: Resection or enucleation is currently the treatment of choice for small pancreatic neuroendocrine tumors (NETs). Irreversible electroporation is a novel ablative method that is used for locally advanced pancreatic adenocarcinoma, but little data exists for its use for pancreatic NETs. We report an early experience of IRE for early pancreatic NETs. Methods: Between April 2014 and March 2015, 3 patients with small (<2 cm) pancreatic NETs were treated with percutaneous IRE. Results: There were no adverse effects during the procedure. Mean hospital stay was 2.6 days. All patients remained disease free on 12-19 months follow up. One patient developed recurrent pancreatitis with pseudocyst formation. Conclusions: IRE for small tumors of the pancreas is practical and may offer advantages over other thermal ablative techniques, since it preserves vital structures such as blood vessels, bile and pancreatic ducts. Further data regarding the long term disease free interval is required to establish efficacy.

The incidence of complications after liver resection is closely related to functional future liver remnant (FLR). The standard approach to augment FLR is surgical or radiological occlusion of the artery or portal vein on the tumor side. Associated liver partition and portal vein ligation for staged hepatectomy (ALLPS) has been introduced as an alternative method to augment FLR. It offers rapid and effective hypertrophy for resecting liver metastases. However, data regarding its application in patients with hepatocellular carcinoma (HCC) with a background of chronic liver disease are limited. Here we describe the use of ALPPS procedure to manage a large solitary HCC with a background of chronic liver disease. The rising incidence of HCC has increased the number of surgical resections in patients with advanced stage liver disease not considered for liver transplantation. We reviewed reported experience of ALPPS in established chronic liver disease and current therapeutic modalities for HCC on a background of chronic liver disease in patients with potential liver insufficiency where tumor burden is beyond liver transplant criteria.

This paper presents a survey of controller design techniques aimed at autonomous navigation and control of Unmanned Aerial Vehicles (UAVs), focusing on the challenge of aerodynamic uncertainty. Although many roadblocks exist, the most significant and challenging task for UAV navigation and control is the one of aerodynamic/model uncertainty. Current autopilots and controller designs for autonomous airplanes are mainly concerned with the feature of constant, unknown aerodynamic parameters, i.e., control and stability derivatives of the platform. This research focuses on a thorough investigation of the related theory and its applicability, centering on specific techniques that are able to control UAVs with rapidly changing, time-varying aerodynamic characteristics during flight. The scientific merit of this work is the comprehensive overview provided and the technical study that is performed, highlighting the advantages and disadvantages for each technique with respect to its efficiency and performance.

Single-to-three-phase direct AC/AC Modular Multilevel Converters have gained a significant interest in the field of railway interties. The use of such an MMC-based structure implies more advanced control techniques as well as design requirements, especially in the case where the three-phase side experiences grid asymmetries. This paper proposes a global control scheme, which is capable of ensuring high performance in such operating conditions. In addition, the impact of grid unbalances on the MMC capacitive energy storage requirements is evaluated by using different line current control techniques with current amplitude limitation as well as taking into consideration a three-phase reactive power injection. The proposed concepts are validated by means of simulation results. Finally, preliminary experimental results from a down-scaled laboratory prototype are provided.
Containership new-building orders and freight rate shocks: A “wait and see” perspective

This paper studies the behaviour of shipping investors following an unexpected shock in the freight rates, while accounting for costs (fuel), and the macro environment (stock prices and trade). The estimates firstly confirm the existence of a long-term relationship between the macroeconomic environment and freight rates, as well as between that and newbuilding orders. Most importantly, we find that when the source of the shock is less clear but still causes an increase in freight rates, shipping investors respond with a delay, which could last almost a year. The thinking behind this “inaction period” is rational, given that the only way to observe whether a shock is permanent or transitory is to wait it out. The above findings have important policy implications not only for shipping investors but also for countries that rely heavily on their ship-building industries.
Fracture of the Second Rib: An Indirect Sign of Serious Trauma Like Fracture of the First Rib?

Background: The first 3 ribs are anatomically well-protected, shielded by the large thoracic muscles, the shoulder girdle, and the arm. A significant force is required to fracture these ribs; thus, such fractures suggest a high-energy trauma and are associated with injuries to vital organs of the thorax, such as the aorta, the heart, the lungs and the great vessels. Methods: A retrospective analysis was conducted over a 10-year period at a single hospital. The study assessed patients with fractures of the second rib, including their concurrent injuries and the overall severity of their trauma. Results: Among the 76 patients included in the study, the average age was 47.35 years, 81.5% were men, and 19.5% were women. Thirteen patients (17.1%) survived their injuries. The most common causes of injury were road traffic accidents (63%) and pedestrian injuries (22%). The patients who did not survive sustained injuries to an average of 5 additional organs, while survivors had injuries to an average of 2.07 additional organs. Left rib fractures were the most frequently observed (46%). The most serious concurrent injuries reported were to the aorta (5.26%), heart (10.52%), lung (52.36%), head (57.89%), liver (30.2%), spleen (26.31%), and kidney (17.1%). Conclusion: As indicators of serious injury to vital endothoracic organs, isolated fractures of the second rib should be considered equal to first rib fractures in clinical importance.
Covid-19 and the energy trade: Evidence from tanker trade routes

We employ a cointegration setup to explore route-specific off-equilibrium deviations related to Covid-19 that have affected clean (petroleum products) and dirty (crude oil) tanker freight rates, over and above the expected macroeconomic reactions. We find that the additional deviation caused by Covid-19 is routespecific. In particular, deviation caused by Covid-19 is found to be more significant for clean tankers, with an average impact of 0.15, an expected outcome given that these products are more reliant on economic developments because of their uses. The clean tanker impact is more evident in Japan-related routes, while no specific pattern can be extracted with regards to the additional off-equilibrium Covid-19 deviation for dirty tanker routes. Results suggest that time-charters and hedging against the stock markets can help shipowners ameliorate demand-driven shocks. © 2022 The Authors.
