        성경상담: 돌봄에서 성경사용을 위한 유기적 그리고 "성례전적" 접근

        목회적 돌봄에서 명백한 전달 매개로서 성경을 사용하는 것은(Louw 1998 참조) 다음과 같은 치료적 차원을 가정한다: 구원은 인간의 행동에 있어 매우 특별한 치료적 효과을 가지고 있다, 더 넓은 의미에서 구원은 목회적 만남의 상황내에서 성경 텍스트에 의해 보여지고 전달되어 진다고 가정된다, 만약 목사들이 근심가진 사람에게 하나님의 약속 안에서 소망을 불러일으키고, 그 사람을 고양시키려 한다면, 그때 그들은 하나님의 말씀의 빛 안에서 삶의 이슈들을 그리고 삶의 이슈들의 빛 안에서 하나님의 말씀을 해석할 수 있어야 한다, 이러한 노력은 목회적 돌봄을 성경적 상담의 도전에 직면시키고 문제적 상황들에서 하나님의 약속들을 적용해야 함에 직면시킨다. Using Scripture (See Louw 1998) as the distinctive communicative medium in pastoral care presumes a therapeutic dimension: that salvation has a very specific therapeutic impact on human behaviour. A further assumption is that salvation is communicated and revealed by the scriptural text, within the context of the pastoral encounter. If pastors are going to be able to generate hope in God`s promises and to edify the person concerned, then they must be able to interpret life issues in the light of the Word of God and the Word of God in the light of life issues. This endeavour faces pastoral care with the challenge of biblical counselling and the application of God`s promises to problematic situations.

        Combat-Related Intradural Gunshot Wound to the Thoracic Spine: Significant Improvement and Neurologic Recovery Following Bullet Removal

        The vast majority of combat-related penetrating spinal injuries from gunshot wounds result in severe or complete neurological deficit. Treatment is based on neurological status, the presence of cerebrospinal fluid (CSF) fistulas, and local effects of any retained fragment(s). We present a case of a 46-year-old male who sustained a spinal gunshot injury from a 7.62‑mm AK-47 round that became lodged within the subarachnoid space at T9–T10. He immediately suffered complete motor and sensory loss. By 24–48 hours postinjury, he had recovered lower extremity motor function fully but continued to have severe sensory loss (posterior cord syndrome). On post-injury day 2, he was evacuated from the combat theater and underwent a T9 laminectomy, extraction of the bullet, and dural laceration repair. At surgery, the traumatic durotomy was widened and the bullet, which was laying on the dorsal surface of the spinal cord, was removed. The dura was closed in a water-tight fashion and fibrin glue was applied. Postoperatively, the patient made a significant but incomplete neurological recovery. His stocking-pattern numbness and sub-umbilical searing dysthesia improved. The spinal canal was clear of the foreign body and he had no persistent CSF leak. Postoperative magnetic resonance imaging of the spine revealed contusion of the spinal cord at the T9 level. Early removal of an intra-canicular bullet in the setting of an incomplete spinal cord injury can lead to significant neurological recovery following even high-velocity and/or high-caliber gunshot wounds. However, this case does not speak to, and prior experience does not demonstrate, significant neurological benefit in the setting of a complete injury.

        Subtotal calvarial vault reconstruction utilizing a customized polyetheretherketone (PEEK) implant with chimeric microvascular soft tissue coverage in a patient with syndrome of the trephined: A case report

        The syndrome of the trephined is a neurologic phenomenon that manifests as sudden decline in cognition, behavior, and sensorimotor function due to loss of intracranial domain. This scenario typically occurs in the setting of large craniectomy defects, resulting from trauma, infection, and/or oncologic extirpation. Cranioplasty has been shown to reverse these symptoms by normalizing cerebral hemodynamics and metabolism. However, successful reconstruction may be difficult in patients with complex and/or hostile calvarial defects. We present the case of a 48-year-old male with a large cranial bone defect, who failed autologous cranioplasty secondary to infection, and developed rapid neurologic deterioration leading to a near-vegetative state. Following debridement and antibiotic therapy, delayed cranioplasty was accomplished using a polyetheretherketone (PEEK) implant with free chimeric latissimus dorsi/serratus anterior myocutaneous flap transfer for vascularized resurfacing. Significant improvements in cognition and motor skill were noted in the early postoperative period. At 6-month follow-up, the patient had regained the ability to speak, ambulate and self-feed-correlating with evidence of cerebral/ventricular re-expansion on computed tomography. Based on our findings, we advocate delayed alloplastic implantation with total vascularized soft tissue coverage as a viable alternative for reconstructing extensive, hostile calvarial defects in patients with the syndrome of the trephined.

        Social media as an enabler of women’s entrepreneurial empowerment during the pandemic

        Entrepreneurs utilize social media to market their products, create value, innovate, and reach their customers. However, recent literature reviews emphasize the need for further investigating the role of social media in small businesses or individual owners. This study explores the enabling empowerment of women pursuing small businesses using social media platforms in Southern Philippines. A thematic analysis, following the grounded theory approach, was conducted to derive themes that explain social media as an enabler of empowerment as experienced by individual women business owners or sellers. The themes indicate that social media enabled them to start or continue their own business and expand their market reach through family and close friends. Moreover, it became an avenue for expressing their autonomy and learning to use the features of social media in an innovative way to support their business.
