      • Prostate Specific Antigen and Gleason Score in Men with Prostate Cancer at a Private Diagnostic Radiology Centre in Western Jamaica

        Background: Prostate cancer is the most common malignant tumour in men and the second most common cause of male cancer death. The study examines the clinicopathological features of patients with prostate cancer consecutively diagnosed at a private Diagnostic Radiology Centre in Western Jamaica over a 6-year period. Method: The medical records, including the pathology reports of 423 consecutive patients who had transrectal ultrasonography (TRUS) - guided prostate biopsy between January 2006 and December 2011 were reviewed. Results: The mean age at diagnosis of the 191 men with prostate adenocarcinoma was $68.5{\pm}0.59$ years with the majority in the 70 - 79 year age group (43.5%). Moderately differentiated carcinomas (Gleason score of 6) comprised the largest group with 72 cases (37.9%); poorly differentiated cancers with Gleason scores of 8 - 10 comprised 49 cases (25.8%). The PSA levels increased with Gleason score. The mean PSA levels for men with Gleason score of 6 was $50.1{\pm}30.0$ ng/mL compared with $136.5 {\pm}59.9$ ng/mL in patients with Gleason score of 8 and $140.5{\pm}31.8$ ng/mL in patients with Gleason score of 9. Perineural invasion was present in 7.85% of the cases overall; high-grade prostatic intraepithelial neoplasia (HGPIN) was present in 4.71% of the biopsies. Conclusion: Although the majority of patients had moderate, and moderate to poor differentiated carcinomas, the number with poorly differentiated carcinoma was high. This is a reflection of the patients' late clinical presentation at the time of diagnosis.

      • Management Earnings Forecasts and the Credibility of Earnings Information

        This paper examines earnings credibility for firms that issue earnings forecasts compared to firms that do not. We measure credibility using the earnings response coefficient (ERC) and we find a significantly higher ERC for forecast firms. We find firms issue biased forecasts in order to meet or beat analysts`` forecasts but, nevertheless, forecast firms have more credible earnings information than non-forecast firms. We also find the higher ERC of forecast firms is most evident among firms with positive earnings surprises, indicating that firms have stronger incentives to signal earnings credibility when they have good news.

        A Co-Creative Approach to Integrated Water Resource Management through Fostering Shared Value

        Water is an important resource necessary for the survival of the human race and the ecosystem which we are apart of. Today many countries globally are facing the challenge of water scarcity resulting in a constant struggle for the economic and social development of these countries. Integrated Water Resource Management(IWRM) through fostering shared value is the solution to these water issues countries are currently facing particularly since it is cost effective and can be achieved in a sustainable manner. Therefore the goal of this paper is to present a viable solution to solve the challenges of managing water resource for future use in developing countries through creating shared value. The design of this paper is to provide a conceptual overview of IWRM through the use of co-creation with stakeholders. In addition a qualitative study employing a grounded theory approach to derive a conceptual model for a co-creative approach to IWRM in developing countries through fostering shared value will be employed. The findings for this research suggested that co-creation is the new solution to IWRM within developing countries. Particularly, since it harness and integrates knowledge, and viable solutions from various stakeholders within a country and worldwide to solve critical problems, while generating shared value for all stakeholders. This paper contributes to the practical, social and academic literature that currently exists regarding IWRM. This is an original paper that presents a viable strategy, through the use of co-creation, for the IWRM in developing countries.

      • Prostate Cancer and Metabolic Syndrome: Is there a link?

        Metabolic syndrome has become quite prevalent within our society. Over the past two decades, the prevalence of metabolic syndrome has sharply increased worldwide and it has become a major public health problem in several countries. It is associated with the global epidemic of obesity and diabetes mellitus and imposes numerous cardiovascular risks. Prostate cancer is the second most common cancer among men, surpassed only by non-melanoma skin cancer. A considerable body of evidence exists suggesting that some components of the metabolic syndrome have been associated with the risk of prostate cancer. These components include obesity, an abdominal fat distribution, and hyperinsulinemia. Androgen deprivation therapy (ADT) is the most widely used therapeutic modality in prostate cancer. It changed the body composition and lipid profile of men with prostate cancer. Androgen deficiency is associated with increased levels of total cholesterol, low-density lipoprotein (LDL)-cholesterol, increased production of proinflammatory factors, and increased thickness of the arterial wall and contributes to endothelial dysfunction. The aim of this review is to evaluate the association between metabolic syndrome and prostate cancer and to discuss the implications of androgen deficiency in men with cardiovascular risk factors. A comprehensive literature search was carried out with the use of PubMed from 1980 through 2011, and relevant articles pertinent to metabolic syndrome and prostate cancer are evaluated and discussed.

        Radiation tolerance of a small COTS single board computer for mobile robots

        As robotics become more sophisticated, there are a growing number of generic systems being used forroutine tasks in nuclear environments to reduce risk to radiation workers. The nuclear sector has calledfor more commercial-off-the-shelf (COTS) devices and components to be used in preference to nuclearspecific hardware, enabling robotic operations to become more affordable, reliable, and abundant. Toensure reliable operation in nuclear environments, particularly in high-gamma facilities, it is importantto quantify the tolerance of electronic systems to ionizing radiation. To deliver their full potential to endusers, mobile robots require sophisticated autonomous behaviors and sensing, which requires significantcomputational power. A popular choice of computing system, used in low-cost mobile robots for nuclearenvironments, is the UP Core single board computer. This work presents estimates of the total ionizingdose that the UP Core running the Robot Operating System (ROS) can withstand, through gamma irradiation testing using a Co-60 source. The units were found to fail on average after 111.1 ± 5.5 Gy, due tofaults in the on-board power management circuitry. Its small size and reasonable radiation tolerancemake it a suitable candidate for robots in nuclear environments, with scope to use shielding to enhanceoperational lifetime.

      • GARCH-X(1, 1) model allowing a non-linear function of the variance to follow an AR(1) process

        GARCH-X(1,1) model specifies that conditional variance follows an AR(1) process and includes a past exogenous variable. This study proposes a new class from that model by allowing a more general (non-linear) variance function to follow an AR(1) process. The functions applied to the variance equation include exponential, Tukey's ladder, and Yeo–Johnson transformations. In the framework of normal and student-t distributions for return errors, the empirical analysis focuses on two stock indices data in developed countries (FTSE100 and SP500) over the daily period from January 2000 to December 2020. This study uses 10-minute realized volatility as the exogenous component. The parameters of considered models are estimated using the adaptive random walk metropolis method in the Monte Carlo Markov chain algorithm and implemented in the Matlab program. The 95% highest posterior density intervals show that the three transformations are significant for the GARCH-X(1,1) model. In general, based on the Akaike information criterion, the GARCH-X(1,1) model that has return errors with student-t distribution and variance transformed by Tukey's ladder function provides the best data fit. In forecasting value-at-risk with the 95% confidence level, the Christoffersen’s independence test suggest that non-linear models is the most suitable for modeling return data, especially model with the Tukey's ladder transformation.

        Radiation testing of low cost, commercial off the shelf microcontroller board

        The impact of gamma radiation on a commercial off the shelf microcontroller board has been investigated. Three different tests have been performed to ascertain the radiation tolerance of the device from a nuclear decommissioning deployment perspective. The first test analyses the effect of radiation on the output voltage of the on-board voltage regulator during irradiation. The second test evaluated the effect of gamma radiation on the voltage characteristics of analogue and digital inputs and outputs. The final test analyses the functionality of the microcontroller when using an external, shielded voltage regulator instead of the on-board voltage regulator. The results suggest that a series of latch-ups occurs in the microcontroller during irradiation, causing increased current drain which can damage the voltage regulator if it does not have short-circuit protection. The analogue to digital conversion functionality appears to be more sensitive to gamma radiation than digital and analogue output functionality. Using an external, shielded voltage regulator can prove beneficial when used for certain applications. The collected data suggests that detaching the voltage regulator can extend the lifespan of the platform up to approximately 350 Gy.

        Botox combined with myofascial release physical therapy as a treatment for myofascial pelvic pain

        Purpose: To report the effects of combined onabotulinumtoxinA (Botox) injections and myofascial release physical therapy on myofascial pelvic pain (MFPP) by comparing pre- and posttreatment average pelvic pain scores, trigger points, and patient self-reported pelvic pain. Secondary outcomes were to examine posttreatment complications and determine demographic differences between patients with/without an improvement in pain. Materials and Methods: This was an Institutional Review Board approved retrospective case series on women over 18 years with MFPP who received Botox and physical therapy between July 2006 and November 2014. Presence of trigger points and pelvic pain scores were determined by digital palpation of the iliococcygeus, puborectalis, obturator internus, and rectus muscles. Average pelvic pain scores (0–10) reflected an average of the scores obtained from palpation of each muscle. Self-reported improvement in pain was recorded as yes/no. Results: Fifty women met the inclusion/exclusion criteria. Posttreatment, patients had lower average pelvic pain scores (3.7±4.0 vs. 6.4±1.8, p=0.005), and fewer trigger points (44% vs. 100%, p<0.001). Fifty-eight percent of patients (95% confidence interval, 44–72) noted an improvement in self-reported pain. Patients most likely to report no improvement in pain had chronic bowel disorders, while those most likely to report an improvement in pain had a history of past incontinence sling (p=0.03). Posttreatment complications included: constipation (8%), worsening urinary retention (2%), and urinary tract infection (4%). Conclusions: Botox combined with soft tissue myofascial release physical therapy under anesthesia can be effective in treating women with chronic pelvic pain secondary to MFPP.
