Evaluation of Armrest and Handle design on Upper-body rehabilitation equipment

Objective: The aim of this study is evaluation of arm support and handle design in grahamizer(upper rehabilitation equipment). Background: In this recent, population of elderly has increased in Korea. So upper limb rehabilitation has been increased importance. The grahamizer is rehabilitation equipment of upper-body generally. However, Design research of Grahamizer by exercise Characteristics are nonexistent in development of upper limb rehabilitation. Method: In this study, usability evaluation of armrest and handle design was performed.; (1) The survey of leading research about handle design, (2) Motion analysis of armrest by variation of central axis, (3) Motion analysis of armrest by variation of armrest's length. Results: The leading research showed that diameter of handle: 3.5cm, shape of handle : D type. The results of this study showed that length of armrest: 220cm, Position of central axis: 3. Conclusion: In the results of this study, Method of design evaluation through handle's trajectory and motion analysis of upper body is very effective method. So evaluation method of this study is expected to be used in development of upper limb rehabilitation equipment.
Study on Deriving Design Improvement Factors by Quality Function Deployment

Objective: The purpose of this study is improving design of upper body ergometer by Quality Function Deployment (QFD). Background: Although many factors for usability including muscle activity, fatigue, breath and pressure were considered to improve upper body ergometer, design of ergometer that is one of the main factors for usability has not been fully investigated. Method: In this study, we asked to experts about requirement derived from users and gained weighting of valuation attributes using Expert choice 2000. From these data, we analyzed the correlation between weighting of valuation attributes weighting and engineering characteristics. Results: The results showed that the priorty improvement factors are armrest, link-structure of actuator and gap width of under frames. Conclusion: From this study, we expect that these quantified data can be proposed to develop the design of upper body ergometer Application: Priorty design improvement factors were derived by QFD method. We have a plan for developing additional design improvement factors of upper body ergometer based on this study.
Risk factors for mortality of severe trauma based on 3 years` data at a single Korean institution

Purpose: This study aimed to determine the mortality rate in patients with severe trauma and the risk factors for trauma mortality based on 3 years’ data in a regional trauma center in Korea. Methods: We reviewed the medical records of severe trauma patients admitted to Ajou University Hospital with an Injury Severity Score (ISS) > 15 between January 2010 and December 2012. Pearson chi-square tests and Student t-tests were conducted to examine the differences between the survived and deceased groups. To identify factors associated with mortality after severe trauma, multivariate logistic regression was performed. Results: There were 915 (743 survived and 172 deceased) enrolled patients with overall mortality of 18.8%. Age, blunt trauma, systolic blood pressure (SBP) at admission, Glasgow Coma Scale (GCS) at admission, head or neck Abbreviated Injury Scale (AIS) score, and ISS were significantly different between the groups. Age by point increase (odds ratio [OR], 1.016; P = 0.001), SBP ≤ 90 mmHg (OR, 2.570; P < 0.001), GCS score ≤ 8 (OR, 6.229; P < 0.001), head or neck AIS score ≥ 4 (OR, 1.912; P = 0.003), and ISS by point increase (OR, 1.042; P < 0.001) were significant risk factors. Conclusion: In severe trauma patients, age, initial SBP, GCS score, head or neck AIS score, and ISS were associated with mortality.

Background: We aimed to investigate the analgesic efficacy of an erector spinae plane block (ESPB) in immediate breast reconstruction (IBR) with a tissue expander. Methods: Adult women undergoing IBR with a tissue expander after mastectomy were randomly assigned to either intravenous patient-controlled analgesia (IV-PCA) alone (group P) or IV-PCA plus ESPB (group E). The primary outcome was the total amount of opioid consumption during 24 hours postoperatively between the two groups. Secondary outcomes were patient satisfaction, pain score at rest and on shoulder movement using numerical rating scale, incidences of postoperative nausea and vomiting (PONV), and a short form of the brief pain inventory (BPI-SF) at 3 and 6 months after surgery between the groups. Results: Fifty eight patients completed the study. At 24 hours postoperatively, total opioid consumption was significantly less in group E than in group P (285.0 ± 92.0, 95% confidence interval [CI]: 250.1 to 320.0 vs. 223.2 ± 83.4, 95% CI: 191.5 to 254.9, P = 0.005). Intraoperative and cumulative PCA fentanyl consumption at 3, 6, 9, and 24 hours were also less in group E than in group P (P = 0.004, P = 0.048, P = 0.020, P = 0.036, and P < 0.001, respectively). Patient satisfaction was higher in group E (6.9 ± 1.8 vs. 7.8 ± 1.4, P = 0.042). The incidences of PONV was similar. Conclusions: The ESPB decreased postoperative opioid consumption and increased patient satisfaction without significant complications after IBR with a tissue expander after mastectomy.
Evaluation on rehabilitation equipment of upper limb through motion analysis

Objective: The aim of this study is developing the upper limb model that could analyze motion of scapula and evaluation on rehabilitation equipment of upper limb through motion analysis. Background: In recent years, has been increasing musculoskeletal disorders of the shoulder because jobs using computer has been increasing. Method: The method of this study is separated of two categories : 1. Development of upper limb model. 2. Experiment by changing angle of moving using motion capture. Results: The range of angle change of Internal rotation, Anterior tilt, Upward rotation were showed greater value as follows : Internal rotation: 0º, Anterior tilt: 45º, Upward tilt: 90º. Conclusion: The results of scapula's motion were different by changing angle(tilting angle).
