Treatment of giant sialolithiasis on wharton`s duct

Sialolithiasis is one of the most common diseases occurring in salivary glands. Sialoliths are characterized by duct closure due to calculus deposits in the salivary gland and ducts, causing edema and pain. Sialolithiasis is most prevalent in the submandibular gland. Most of the sialoliths are small in size (less than 10 mm), and giant sialoliths (greater than 15 mm) are rarely reported. This case reports the successful removal of giant sialolithiasis of 45 mm or more, by an intraoral approach.
Removal of a fractured needle during inferior alveolar nerve block: two case reports

The inferior alveolar nerve block is the most common method of local anesthesia for intraoral surgery at the posterior mandibular region. However, unexpected complications may occur when administering the local anesthesia. One of these uncommon complications is the fracture of the needle. If the injection needle is broken during the surgery, it should be removed immediately. However, this is one of the most difficult procedures. In this report, we present two cases of needle fracture during the procedure, and its successful removal under general/local anesthesia administration.
Histomorphometric evaluation for osseointegration after particulated-dentin coated implant placement

Purpose: The purpose of this study was to evaluate osseointegration after placement of a machined surface implant and particulated-dentin-coated implant in a canine model. Materials and Methods: Four dogs were randomly assigned to two groups, and each group was further divided into two subgroups 4 and 8 weeks after implantation. The implant was placed under different conditions. Group 1 included the machined surface implant and group 2 included the particulated-dentin-coated implant. Histologic sections and histomorphometric analysis were obtained 4 and 8 weeks after surgery. Results: Group 2 showed a higher bone-implant contact rate and bone formation rate than group 1 in the 8 week group. In group 2, the 8 week group revealed significant elevation of both bone-implant contact rate and new bone formation rate versus the 4 week group. Conclusion: According to these results, particulate-dentin-coated implants can provide satisfactory stability and increase the quantity and maturity of new bones in the later stages of implant placement.
Paranasal augmentation using Medpor implant and changes in lateral profile

Purpose: The purpose of this study was to simultaneously evaluate soft tissue changes induced by bilateral sagittal split osteotomy of the mandible as well as paranasal augmentation using a Medpor® implant based on cephalometric radiography before and 6 months after surgery. Materials and Methods: We analyzed the increase in soft tissue volume in 20 patients diagnosed with mandibular advancement and midfacial depression between Feburary 2005 and Feburary 2009. Paranasal augmentation was performed using a 4.5mm-thick Medpor® implant, and lateral head radiographs were analyzed before and 6 months after surgery. Results: The increase in soft tissue volume was 3.27±1.16 mm, which was 72.67% of the actual thickness. After surgery, no particular complication was detected, and the results were acceptable to the patients and surgeons. Based on these results, the Medpor®implant is a satisfactory artificial graft material for use in facial augmentation. Conclusion: Paranasal augmentation using the Medpor® implant can be considered a good surgical option that can maximize the aesthetics of a patient`s face.

Maxillary sinus membrane elevation and bone graft have been performed routinely in alveolar bone with insufficient residual bone height. There are a number of causes for development of maxillary sinusitis after these procedures. When maxillary sinusitis is caused by sinus membrane elevation, bone graft, and implant placement, various treatment such as medication, incision and drainage (I&D), implant removal, and the Caldwell-Luc procedure can be considered. Removal of an implant or the Caldwell-Luc procedure can be harmful if inflammation is not present in the oral cavity and survival of grafted bone and implant osseointegration can be expected despite the presence of maxillary sinusitis. In this case, functional endoscopic sinus surgery, which was often used in the otorhinolaryngology department, was performed without removal of the implant for a patient with maxillary sinusitis after one month following implant placement. Thus, we report on this case with a review of the literature.
다양한 Laser를 이용한 Peri-Implantitis의 치료

Peri-implantitis is the inflammatory process, such as edema, bleeding, pus, of the mucosa surrounding dental implants. As the symptoms become severe, the surrounding bone is absorbed causing the implant surface to be exposed. Clinicians treat periimplantitis in various ways since a gold standard for the treatment of peri-implantitis has not been established. Various treatment methods include mechanical, chemical surface treatment and surgical excision, and recently decontamination of the implant surface using various types of lasers has been proposed. Thus, this study reviews the types of lasers and its effects that can be used for the treatment of peri-implantitis.
