        A Case of Hereditary Spherocytosis Initially Manifested as an Aplastic Crisis Caused by Parvovirus B19 Infection

        Hereditary spherocytosis (HS) is the most common inherited red cell membrane disorder. Its main laboratory finding is anemia with reticulocytosis. However, in the case of an aplastic crisis, there may be no reticulocytosis, making the diagnosis of HS difficult. We present the case of a 4-year-old boy who initially presented with persistent fever and sore throat. His 8-year old brother also had anemia of unknown etiology, and his father had a history of splenectomy in his 20s. Physical examination revealed anemic conjunctivae and hepatosplenomegaly, and laboratory findings showed anemia with decreased reticulocyte count and elevated ferritin and lactate dehydrogenase levels. A peripheral blood smear showed microcytic hypochromic anemia with severe poikilocytosis (spherocytes, acanthocytes, schistocytes), and bone marrow examination revealed decreased erythroid cells and increased hemophagocytosis. Increased osmotic fragility was observed, and parvovirus B19 was detected using polymerase chain reaction. Hence, we established the diagnosis of hereditary spherocytosis manifested as an aplastic crisis caused by parvovirus B19 infection.

      • 복합명사 분할과 명사구 합성을 이용한 통합 색인 기법

        본 논문에서는 명사구 색인과 복합명사 분할을 포함한 복합명사 처리를 위해 통계 정보와 자연언어 처리를 제한적으로 이용 가능하게 하는 통합적 색인 기법을 제안한다. 먼저 색인과 검색에서 복합명사 분할 및 합성 모두를 고려한 통합 기법을 제시하고, 이를 위해 통계 정보와 제한적인 자연언어 처리를 모두 이용하는 통합 색인 기법을 제안한다. 먼저 형태소 분석 및 태깅 과정에서 단일어를 색인어로 추출하고 구문분석의 결과에서 명사구를 합성해 낸다. 구문 분석 실패 시에는 형태소 분석 및 태깅의 결과만을 사용하게 된다. 또한 태깅의 결과에서 복합명사를 골라 통계 정보를 이용하여 단일 명사로 분할하고 재합성한다. 분할된 단일 명사와 합성된 명사구는 기존의 단일어로만 이루어진 색인어를 보완하기 위해 색인어로 사용된다. 실험은 한국어 정보검색의 실험 집합인 KTSET 2.0과 KRIST SET을 사용하여 통합색인 기법이 복합명사 처리에 효율적임을 보였다. In this paper, we propose an integrated indexing method with compound noun segmentation and noun phrase synthesis. Statistical information is used in the compound noun segmentation and natural language processing techniques are carefully utilized in the noun phrase synthesis. Firstly, we choose index terms from simple words through morphological analysis and part-of-speech tagging results. Secondly, noun phrases are automatically synthesized from the syntactic analysis results. If syntactic analysis fails, only morphological analysis and tagging results are applied. Thirdly, we select compound nouns from the tagging results and then segment and re-synthesize them using statistical information. In this way, segmented and synthesized terms are used together as index terms to supplement the single terms. We demonstrate the effectiveness of the proposed integrated indexing method for Korean compound noun processing using KTSET2.0 and KRIST SET which are a standard test collection for Korean information retrieval.

        A Case with Multiple Fungal Coinfections in a Patient who Presented with Pancoast Syndrome

        소아암 환자에서 발생하는 침습성 진균 감염은 사망과 후유증에 이르는 중대한 감염이다. 18세 남자 환자가 호중구감소 기간 동안 입원하여 치료받던 중 우측 견관절과 우측 팔에 감각이상과 신경쇠약을 호소하였고, 우측폐상엽의 폐렴이 진단되었다. 기관지의 조직학적 소견과 폐 수술검체에서 시행한 polymerase chain reaction (PCR)로 털곰팡이증을 확진하였으며, 흉수액의 PCR로 페니실리움 디쿰벤스 감염, 갈락토마난 항원법으로 아스페르길루스증을 추정하였다. 환자는 백혈병이 치료되지 못하고 Staphylococcus epidermidis 패혈증이 합병되어 사망하였다. 본 증례에서는 판코스트 증후군의 증상을 보인 환자에서 진단된 다발성 폐진균증을 보고 하는 바이다. Invasive fungal infection (IFI) is a serious threat to pediatric patients with cancer given high morbidity and mortality. We present an 18-year-old male with precursor T-cell lymphoblastic leukemia who developed Pancoast syndrome, presented with paresthesia and numbness in the right shoulder and arm during a neutropenic fever period. He was diagnosed with pneumonia in the right upper lung field. He was later found to have an invasive pulmonary fungal infection caused by multiple fungi species, including Rhizomucor, confirmed by histology and polymerase chain reaction (PCR) (proven infection), Penicillium decumbens diagnosed by PCR, and Aspergillus suspected from galactomannan assay (probable infection). Unfortunately, the patient's condition further worsened owing to the aggravation of leukemia, chemotherapy-induced neutropenia, and bacterial coinfection, leading to multiorgan failure and death. Here, we report a case of IFI caused by multiple fungal species that presented as Pancoast syndrome.

        Vivaldi: A Domain-Specific Language for Volume Processing and Visualization on Distributed Heterogeneous Systems

        <P>As the size of image data from microscopes and telescopes increases, the need for high-throughput processing and visualization of large volumetric data has become more pressing. At the same time, many-core processors and GPU accelerators are commonplace, making high-performance distributed heterogeneous computing systems affordable. However, effectively utilizing GPU clusters is difficult for novice programmers, and even experienced programmers often fail to fully leverage the computing power of new parallel architectures due to their steep learning curve and programming complexity. In this paper, we propose Vivaldi, a new domain-specific language for volume processing and visualization on distributed heterogeneous computing systems. Vivaldi's Python-like grammar and parallel processing abstractions provide flexible programming tools for non-experts to easily write high-performance parallel computing code. Vivaldi provides commonly used functions and numerical operators for customized visualization and high-throughput image processing applications. We demonstrate the performance and usability of Vivaldi on several examples ranging from volume rendering to image segmentation.</P>

        최근 몰입형 미디어(Immersive Media)의 등장과 수요의 증가에 따라 다중 시점 영상 관련 연구가 활발하게 진행되고 있다. 본 논문에서는 인스턴스 분할이 적용된 다중 시점 이미지의 객체들 간 ID 를 매칭하는 방법에 대해 분석한다. 기존 관련 연구들을 통해서 해당 작업을 수행하는 방법과 한계점에 대하여 설명하고, 스테레오 비전을 이용하여 ID 매칭 성능을 높인 관련 연구에 대해서 기술한다. 이후 실험을 통해 데이터 셋에 따른 알고리즘의 성능을 보이고, 이에 대한 한계점을 분석한다. 최종적으로 다중 시점 분할 객체 간 ID 매칭의 성능을 높이기 위해서 필요한 연구 방향에 대해서 제시한다.

        비급성 요통에 대한 위수혈 자락관법의 효과 탐색: 예비 무작위 대조군 시험

        Objectives This study was designed to investigate the effects of wet cupping on Wisu (BL12) in non-acute low back pain patients. Methods We recruited 30 participants for this study. Fifteen patients were randomly assigned to the Wisu (BL21) treatment group (WT group) and 15 were assigned to the non-acupoint treatment group (NT group). Both groups were treated with the pricking-cupping bloodletting method three times. Values at baseline and follow-up were analyzed by Wilcoxon signed rank test and the differences between the two groups were determined by Wilcoxon rank sum test. p-values less than 0.05 were considered significant. The primary outcome was the visual analogue scale (VAS), and secondary outcomes were the Oswestry disability index (ODI), Rolland-Morris disability questionnaire (RMDQ), Euroqol-5 dimension questionnaire (EQ-5D) and finger-to-ground distance (FTGD). These outcomes were measured on the day of first treatment before the procedure and on follow-up 7 days after the last treatment. Results Significant changes were identified in the VAS for pain and ODI in each group after wet cupping treatment on Wisu (p<0.05). However, no significant changes were found between groups. Meanwhile, RMDQ and EQ-5D were significantly decreased only in the NT group (p<0.05) without any differences between groups. FTGD was decreased in both groups, but not significantly. Conclusions Wet cupping with both Wisu treatment and non-acupoint had significant effects on non-acute low back pain, although there were no differences between the two groups. A large-scale study is needed to identify the effect of wet cupping on Wisu. (J Korean Med Rehabil 2018;28(4):21-32)

        MLI(Multi-Layer Insulation) is widely used to get highly insulating efficiency on cryogenic system in order to reduce heat loads. In this study, The heat loads from the environment for an annular cylinder cryostat such as HTS(High Temperature Superconducting) power cable system were measured by liquid nitrogen boil-off calorimetry for investigating the thermal characteristics on MLI. The cryostat consists of test vessel and two guard vessels which located at both end sides of test vessel. In order to prevent radiation heat load through the both end sides of the test vessel, the guard vessels are filled with liquid nitrogen. The measured heat loads are compared with the calculated them.

        Effectiveness and safety of Korean medicine for pain management after musculoskeletal surgery: a retrospective study

        To assess the effectiveness and safety of Korean medicine in managing postoperative pain. Retrospective, observational study. Medical records of 233 inpatients at a Korean medicine hospital who underwent Korean medicine treatments such as acupuncture, moxibustion, cuppping, bee-venom and herbal acupuncture, electrical therapy, herbal medicine, and carbon-beam therapy after musculoskeletal surgery were reviewed, and numeric ratings for pain were compared between baseline and follow-up. A correlation analysis between pain improvement and therapeutic factors was performed, and a regression analysis was conducted to determine the predictors of pain reduction. Serum laboratory test results at baseline and follow-up were subsequently compared to identify safety. Numeric rating scale scores for pain; presence of infection, drug-induced liver injury (DILI), and drug-induced kidney injury (DIKI) for safety. After Korean medicine treatment, the pain scores decreased from an average of 4.09 to 1.79 in the numeric rating scale (p < 0.001). Frequency of acupuncture therapy during hospitalization showed the strongest positive correlation with pain improvement (r = 0.341, p = 0.000) and was a predictor of pain reduction (adjusted R2 = 0.145). Improvements in serum laboratory data to reference limits were observed. All kappa coefficient values, except for white blood cell (WBC) count, were between 0.3 and 0.7, indicating that the follow-up data matched the baseline data. The low kappa coefficient value of WBC count was due to the high ratio of outliers. One (0.43%) and no cases of DILI and DIKI were identified, respectively. Korean medicine could be used for managing pain after musculoskeletal surgery.

        고관절 골절 수술 이후 광선치료를 포함한 한방치료 3예 보고

        This study is designed to report three cases of postoperative care after hip surgery using conventional Korean Medicine treatments including phototherapy. Three patients undergone hip fracture surgery were treated with acupuncture, herbal medicine, and phototherapy. We used Visual analogue scale (VAS), EuroQol five dimension scale (EQ-5D), and hip disability and osteoarthritis outcome score (HOOS) to evaluate treatment effectiveness. After the treatment, VAS score was improved in all three cases, while EQ-5D and HOOS showed different results by items. EQ-5D (pain/discomfort) and HOOS (pain, symptoms, activities of daily living) were improved in all three cases. EQ-5D (self-care) and HOOS (quality of life) showed improvements in two cases. EQ-5D (mobility, usual activities, anxiety/depression) and HOOS (sports/recreation) were improved in one case. Conventional Korean Medicine treatments including phototherapy can be effectively used to alleviate pain and symptoms after hip fracture surgery. (J Korean Med Rehabil 2018;28(4):113-124)
