        Are We Looking at a Paradigm Shift in the Management of Adolescent Idiopathic Scoliosis? Comprehensive Retrospective Analysis of 75 Patients of Nonfusion Anterior Scoliosis Correction with 2–5-Year Follow-up: A Single Center Experience

        Study Design: Retrospective cohort study.Purpose: This study aimed to evaluate the clinical and radiological outcomes of nonfusion anterior scoliosis correction (NFASC) in patients with idiopathic scoliosis and comprehensively analyze its principles.Overview of Literature: NFASC is a novel revolutionary motion-preserving surgery for idiopathic scoliosis. However, clinical data related to this procedure remain scarce, with no conclusive guidelines regarding case indications, proper technique, and possible complications.Methods: This study included patients with adolescent idiopathic scoliosis (AIS) who were treated with NFASC for a structural major curve (Cobb angle, 40°–80°) with more than 50% flexibility on dynamic X-rays. The mean follow-up was 26±12.2 months (range, 12– 60 months). Clinical and radiological data such as skeletal maturity, curve type, Cobb angle, surgery details, and Scoliosis Research Society-22 revised (SRS-22r) questionnaire were collected. Statistically significant trends were examined by post hoc analysis following repeated measures analysis of variance test.Results: A total of 75 patients (70 females, five males) were included, with a mean age of 14.96±2.69 years. The mean Risser and Sanders scores were 4.22±0.7 and 7.15±0.74, respectively. The mean main thoracic Cobb angles at the first and second follow-up (17.2°±5.36° and 16.92°±5.06°, respectively) were significantly lower than the preoperative Cobb angles (52.11°±7.74°) (<i>p</i> <0.05). Similarly, the mean thoracolumbar/lumbar Cobb angle significantly improved from the preoperative period (51.45°±11.26°) to the first follow-up (13.48°±5.11°) and last follow-up (14.24°±4.85°) (<i>p</i> <0.05). The mean preoperative and postoperative SRS-22r scores were 78.0±3.2 and 92.5±3.1, respectively (<i>p</i> <0.05). None of the patients had any complications until the most recent follow-up.Conclusions: NFASC offers promising curve correction and curve progression stabilization in patients with AIS, with a low risk for complications and preservation of spinal mobility and sagittal parameters. Thus, it proves to be a favorable alternative to fusion modality.

      • What is a Cancer Cell? Why does it Metastasize?

        This is a commentary on what a cancer cell is and why cancer cells metastasize. Normal cell get transformed to a cancer cell, with excessive production of free radicals that mutate the DNA of a normal cell. The immortality and malignant stage of transformed cell is maintained by higher GSH levels. With the faster rate of proliferation, when the cancer cell finds the place of origin is not conducive to its further growth, cancer cell chooses to take the metastatic course. We argue that if we can stop the exit of cancer cell from place of origin, cancer spread can be stopped or even cured.

        Synthesis and Antimicrobial Screening of Pyrimidine Annulated Dihydropyrano[2, 3-c]pyrazole Derivatives

        A series of pyrimidine annulated dihydropyrano[2, 3-c]pyrazole derivatives were synthesized and screened for their antimicrobial activity. The precursor, dihydropyrano[2, 3-c]pyrazole was synthesised under catalyst free condition using PEG-400 as reaction medium which facilitated improved yield compared to base catalyzed reaction. Wide scope of substrates, simple workup procedure and high yield even in the absence of catalyst are the major highlights of the protocol. Dihydropyrano[2, 3-c]pyrazoles on condensation with formic acid formed pyrimidine annulated dihydropyrano[2, 3-c]pyrazole derivatives. All the products are characterized using FTIR, $^1H-NMR$ and $^{13}C-NMR$ spectroscopic techniques. The molecules have shown good to moderate activity as antimicrobial agents when compared to the standard drug ciprofloxacin.

      • A Coherent Algorithm for Noise Revocation of Multispectral Images by Fast HD-NLM and its Method Noise Abatement

        Numerous spatial and transform-domain-based conventional denoising algorithms struggle to keep critical and minute structural features of the image, especially at high noise levels. Although neural network approaches are effective, they are not always reliable since they demand a large quantity of training data, are computationally complicated, and take a long time to construct the model. A new framework of enhanced hybrid filtering is developed for denoising color images tainted by additive white Gaussian Noise with the goal of reducing algorithmic complexity and improving performance. In the first stage of the proposed approach, the noisy image is refined using a high-dimensional non-local means filter based on Principal Component Analysis, followed by the extraction of the method noise. The wavelet transform and SURE Shrink techniques are used to further culture this method noise. The final denoised image is created by combining the results of these two steps. Experiments were carried out on a set of standard color images corrupted by Gaussian noise with multiple standard deviations. Comparative analysis of empirical outcome indicates that the proposed method outperforms leading-edge denoising strategies in terms of consistency and performance while maintaining the visual quality. This algorithm ensures homogeneous noise reduction, which is almost independent of noise variations. The power of both the spatial and transform domains is harnessed in this multi realm consolidation technique. Rather than processing individual colors, it works directly on the multispectral image. Uses minimal resources and produces superior quality output in the optimal execution time.

        Behaviour of geocell reinforced soft clay bed subjected to incremental cyclic loading

        The paper deals with the results of the laboratory cyclic plate load tests performed on the reinforced soft clay beds. The performances of the clay bed reinforced with geocells and geocells with additional basal geogrid cases are compared with the performance of the unreinforced clay beds. From the cyclic plate load test results, the coefficient of elastic uniform compression ($C_u$) was calculated for the different cases. The $C_u$ value was found to increase in the presence of geocell reinforcement. The maximum increase in the $C_u$ value was observed in the case of the clay bed reinforced with the combination of geocell and geogrid. In addition, 3 times increase in the strain modulus, 10 times increase in the bearing capacity, 8 times increase in the stiffness and 90% reduction in the settlement was observed in the presence of the geocell and geogrid. Based on the laboratory test results, a hypothetical case of a prototype foundation subjected to cyclic load was analyzed. The results revealed that the natural frequency of the foundation-soil system increases by 4 times and the amplitude of the vibration reduces by 92% in the presence of the geocells and the geogrids.

        Effect of vibration during local anesthesia administration on pain, anxiety, and behavior of pediatric patients aged 6-11 years: A crossover split-mouth study

        Background: Uncooperative behavior of children due to dental anxiety may interfere with the effective delivery of dental care and compromise the quality of treatment provided. Injection of local anesthesia is one of the most anxiety-inducing stimuli in pediatric dentistry. This study aimed to compare the efficacy of a child-friendly device, having a combined effect of vibration and distraction, with the conventional method of injection on pain, anxiety, and behavior of pediatric patients aged 6-11 years. Methods: This randomized, crossover, split-mouth study included 30 children requiring a bilateral inferior alveolar nerve block. The children were equally divided into two groups: group 1, aged 6-8 and group 2, aged 9-11 years. All children were injected with anesthesia using the conventional and device method in two separate sessions. They were assessed for anxiety by measuring the pulse rate before and during the administration of local anesthesia. Behavior was assessed using Faces, Legs, Activity, Cry, Consolability (FLACC) scale, and the child's experience while receiving anesthesia was assessed using the Wong Bakers Pain Rating Scale. Results: Results showed that the children who received local anesthesia using the device method had a lower mean pulse rate, FLACC scores, and pain rating scores than those who received local anesthesia using the conventional method. Conclusion: The device method was more effective than the conventional method in managing pain, anxiety, and behavior of patients aged 6-11 years. The device is a cost effective, simple, and child-friendly product for administrating local anesthesia in pediatric patients.

        A Facile Solvent and Catalyst Free Synthesis of New Dihydro Pyrimidinones as Antimicrobial Agents

        An efficient one pot multicomponent synthesis of pyrimidinone derivatives of Biginelli type is described. 4-amino-6-aryl-pyrimidine-5-carbonitrile molecules were synthesized efficiently via three-component Biginelli-type condensation of aldehyde, malononitrile, and semicarbazone as urea substituent in the presence of a catalytic amount of PEG-400 as green medium under microwave irradiation. The reactions proceeded efficiently in the presence of microwave radiation to afford the desired products in good to excellent yields. Products have been confirmed by IR, and NMR spectral analysis. All the molecules were tested for their antimicrobial activity against E. coli, S. aureus, P. aeruginosa and C. tropicalis. Some of the compounds have shown moderate to good inhibition efficiency against both gram-positive and gram-negative bacteria. The potent activity was observed against the fungal species with minimum inhibition concentration 12.5 ㎍/mL.

        Multifunctional lipidic nanocarriers for effective therapy of glioblastoma: recent advances in stimuli-responsive, receptor and subcellular targeted approaches

        Background Glioblastoma, or glioblastoma multiforme (GBM), remains a fatal cancer type despite the remarkable progress in understanding the genesis and propagation of the tumor. Current treatment modalities, comprising mainly of surgery followed by adjuvant chemoradiation, are insufficient for improving patients' survival owing to existing hurdles, including the blood–brain barrier (BBB). In contemporary practice, the prospect of long-term survival or cure continues to be a challenge for patients suffering from GBM. This review provides an insight into the drug delivery strategies and the significant efforts made in lipid-based nanoplatform research to circumvent the challenges in optimal drug delivery in GBM. Area covered Owing to the unique properties of lipid-based nanoplatforms and advancements in clinical translation, this article describes the application of various stimuli-responsive lipid nanocarriers and tumor subcellular organelle-targeted therapy to give an idea about the strategies that can be applied to enhance site-specific drug delivery for GBM. Furthermore, active targeting of drugs via surface-modified lipid-based nanostructures and recent findings in alternative therapeutic platforms such as gene therapy, immunotherapy, and multimodal therapy have also been overviewed. Expert opinion Lipid-based nanoparticles stand out among the other nanocarriers explored for GBM drug delivery, as they support both passive and active drug targeting by crossing/bypassing the BBB at the same time minimizing toxicity and projects better pharmacological parameters. Although these nanocarriers could be a plausible choice for treating GBM, indepth research is essential to advance neuro-oncology research and enhance outcomes in patients with brain tumors.

        Acacia mangium Willd. - A Fast Growing Tree for Tropical Plantation

        Acacia mangium is an evergreen fast-growing tropical tree, which can grow up to 30 m tall and 50 cm thick, under favorable conditions. It is a low-elevation species associated with rain forest margins and disturbed, well-drained acid soils. It is native to Papua, Western Irian Jaya and the Maluku islands in Indonesia, Papua New Guinea and north-eastern Queensland in Australia. Due to its rapid growth and tolerance of very poor soils, A. mangium was introduced into some Asian, African and western hemisphere countries where it is used as a plantation tree. A. mangium has good quality wood traits, such as a comparatively low proportion of parenchymatous cells and vessels, white and hard wood, and high calorific value. Therefore, it is useful for a variety of purposes, such as furniture, cabinets, turnery, floors, particleboard, plywood, veneer, fence posts, firewood, and charcoal. It is also being used in pulp and paper making because it has good pulp traits, with high yields of pulp, quality of kraft, and produces paper with good optical, physical and surface properties. Because there are significant provenance differences in growth rate, stem straightness, heartwood formation and frequency of multiple leaders, the productivity and quality also varies depending upon environmental conditions, so genetic improvement programmes have been undertaken in countries like Australia, India, Indonesia, Malaysia, the Philippines, Taiwan and Thailand. The programme includes provenance identifications and testing, plus tree selection and clonal multiplication, establishment of seed orchards and hybridization. The phenology, reproductive biology, fruit characteristics, silvicultural practices for cultivation, pest and diseases problems, production of improved planting stock, harvesting, wood properties and utilization have been discussed in this paper.
