      • Cost-Effectiveness of Intensive Vs. Standard Follow-Up Models for Patients with Breast Cancer in Shiraz, Iran

        Background: Breast cancer is the most common type of cancer amongst women throughout the world. Currently, there are various follow-up strategies implemented in Iran, which are usually dependent on clinic policies and agreement among the resident oncologists. Purpose: A cost-effectiveness analysis was performed to assess the cost-effectiveness of intensive follow-up versus standard models for early breast cancer patients in Iran. Materials and methods: This cross sectional study was performed with 382 patients each in the intensive and standard groups. Costs were identified and measured from a payer perspective, including direct medical outlay. To assess the effectiveness of the two follow-up models we used a decision tree along with indicators of detection of recurrence and metastasis, calculating expected costs and effectiveness for both cases; in addition, incremental cost-effectiveness ratios were determined. Results: The results of decision tree showed expected case detection rates of 0.137 and 0.018 and expected costs of US$24,494.62 and US$6,859.27, respectively, for the intensive and standard follow-up models. Tornado diagrams revealed the highest sensitivity to cost increases using the intensive follow-up model with an ICER=US$148,196.2. Conclusion: Overall, the results showed that the intensive follow-up method is not cost-effective when compared to the standard model.

      • Cost-Utility of "Doxorubicin and Cyclophosphamide" versus "Gemcitabine and Paclitaxel" for Treatment of Patients with Breast Cancer in Iran

        Purpose: A cost-utility analysis was performed to assess the cost-utility of neoadjuvant chemotherapy regimens containing doxorubicin and cyclophosphamide (AC) versus paclitaxel and gemcitabine (PG) for locally advanced breast cancer patients in Iran. Materials and Methods: This cross-sectional study in Namazi hospital in Shiraz, in the south of Iran covered 64 breast cancer patients. According to the random numbers, the patients were divided into two groups, 32 receiving AC and 32 PG. Costs were identified and measured from a community perspective. These items included medical and non-medical direct and indirect costs. In this study, a data collection form was used. To assess the utility of the two regimens, the European Organization for Research and Treatment of Cancer Quality of Life Questionnaire-Core30 (EORTC QLQ-C30) was applied. Using a decision tree, we calculated the expected costs and quality adjusted life years (QALYs) for both methods; also, the incremental cost-effectiveness ratio was assessed. Results: The results of the decision tree showed that in the AC arm, the expected cost was 39,170 US$ and the expected QALY was 3.39 and in the PG arm, the expected cost was 43,336 dollars and the expected QALY was 2.64. Sensitivity analysis showed the cost effectiveness of the AC and ICER=-5535 US$. Conclusions: Overall, the results showed that AC to be superior to PG in treatment of patients with breast cancer, being less costly and more effective.

        On the elastic parameters of the strained media

        The changes of parameters of pressure and velocity of propagation of elastic pressure and shear waves in uniformly deformed solid compressible media are studied within the nonclassically linearized approach (NLA) of nonlinear elastodynamics to create a new theoretical basis of the geomechanical interpretation of various groups of geophysical observational and experimental data. The cases of small and large deformations are considered while their describing by various elastic potentials, i.e., problems considering the physical and geometric nonlinearity. Convenient analytical formulae are obtained to calculate the indicated parameters in the deformed isotropic media within the nonclassical linear and nonlinear solution in the NLA. Specific numerical experiments are conducted in case of overall compression of various materials. It is shown that the method (generally accepted in the studies of mechanics of standard constructional materials) of additional linearization (relative to the pressure parameter) in the basic correlations of the NLA introduces substantial quantitative and qualitative errors into the results at significant preliminary deformations. The influences of the physical and geometric nonlinearity on the studied characteristics of the medium are large in various materials and differ qualitatively. The contribution of nonlinear components to the values of the considered parameters prevails over linear components at large deformations. When certain critical values of compression deformations in the medium are achieved, elastic waves with actual velocity cannot propagate in it. The values of the critical deformations for pressure and shear waves differ within different elastic potentials and variants of the theory of initial deformations.

      • Modeling Hydrogen Peroxide Bleaching Process to Predict Optical Properties of a Bleached CMP Pulp

        In this paper, the possibility of statistical modeling from the pulp and peroxide bleaching condition variables to predict optical properties of a bleached chemimechanical pulp used in a newsprint paper machine at Mazandaran Wood and Paper Industries Company (MWPI) was studied. Due to the variations in the opacity and the brightness of the bleached pulp at MWPI and to tackle this problem, it was decided to study the possibility of modeling the bleaching process. To achieve this purpose, Multi-variate Regression Analysis was used for model building and it was found that there is a relationship between independent variables and pulp brightness as well as pulp opacity, consequently, two models were constructed. Then, model validation was carried out using new data set in the bleaching plant at MWPI to test model predictive ability and its performance.

        Vibration Energy Harvesting Using Piezoelectric Elements by Multi-switch Circuit with Adaptive Inductance

        Mechanical vibration energy harvesting using multi-switch circuit with adaptive inductance is a new method based on impedance matching between electrical circuit and mechanical vibrating system in order to maximize the harvested energy. Considering a vibrating piezoelectric element in parallel with a load resistance, the whole converted energy is not dissipated or flowed into the load resistance and a remarkable portion of this energy is stored on the internal capacitor of the piezoelectric. By connecting the vibrating piezoelectric to a special interface circuit, approximately the whole stored energy portion on the piezoelectric flows into the circuit. The proposed interface circuit contains a set of inductors in series with electronic switches that controlled by a microcontroller which always provides resonance condition in the designed frequency range. In the proposed technique, for a simulated piezoelectric generator with a constant current source in the design frequency range of 100 to 700 Hz, the harvested power of numerical analysis is 2 to 10 times (depending on frequency) more than standard method (simple resistive circuit). Harvested power in the exprimental test for a cantilever piezoelectric plate in the design frequency of 100 to 350 Hz, is 4.5 times more than standard method.

        Hydroalcoholic Extract of Scrophularia Striata Attenuates Hypertrophic Scar, Suppresses Collagen Synthesis, and Stimulates MMP2 and 9 Gene Expression in Rabbit Ear Model

        Objectives: Hypertrophic scars (HSs) are caused by abnormal wound healing. To date, no standard treatment has been made available for HSs. Scrophularia striata has been reported to accelerate wound healing and has the potential to prevent HS formation. In this study, we investigated the anti-scarring effects of S. striata extract (SSE) in a rabbit ear model of scarring. Methods: In this study, New Zealand white rabbit (weight: 2.3-2.5 kg) were used. In the prevention phase of the study, three test groups received 5%, 10%, and 15% ointments of SSE in the Eucerin base, the fourth group received Eucerin, and the fifth group received no treatment. The samples were obtained on day 35 after wounding. In the treatment phase of the study, the test groups received an intralesional injection of SSE (5%, 10%, and 15%), the fourth group received an intralesional injection of triamcinolone, the fifth group received a solvent (injection vehicle), and the sixth group received no treatment. To evaluate the anti-scarring effects of SSE, the scar elevation index (SEI), epidermis thickness index (ETI), collagen deposition, and MMP2 and MMP9 gene expression were evaluated. Results: A significant reduction in SEI, ETI, and collagen deposition was noted in animals treated with SSE compared with the control groups. In addition, topical SSE stimulated MMP2 and MMP9 gene expression. Conclusion: The findings of this study demonstrate the potential for SSE in the prevention and treatment of HS. SSE could be prepared as an appropriate formulation to treat wounds and prevent abnormal scarring.

        Geomechanical analysis of elastic parameters of the solid core of the Earth

        It follows from the basic principles of mechanics of deformable solids relating to the strength, stability and propagation of elastic waves that the Earth's inner core cannot exist in the form of a spherical structure in the assumed thermobaric conditions and calculation values of physico-mechanical parameters. Pressure level reaches a value that is significantly greater than the theoretical limit of medium strength in the model approximations at the surface of the sphere of the inner core. On the other hand, equilibrium state of the sphere is unstable on the geometric forming at much lower loads under the influence of the "dead" surface loads. In case of the action of "follower" loads, the assumed pressure value on the surface of the sphere is comparable with the value of the critical load of "internal" instability. In these cases, due to the instability of the equilibrium state, propagation of homogeneous deformations becomes uneven in the sphere. Moreover, the elastic waves with actual velocity cannot propagate in such conditions in solid medium. Violation of these fundamental conditions of mechanics required in determining the physical and mechanical properties of the medium should be taken into account in the integrated interpretations of seismic and laboratory (experimental) data. In this case, application of the linear theory of elasticity and elastic waves does not ensure the reliability of results on the structure and composition of the Earth's core despite compliance with the required integral conditions on the mass, moment of inertia and natural oscillations of the Earth.

        Determination of elastic parameters of the deformable solid bodies with respect to the Earth model

        The study of behavior and values of deformations in the geological medium makes the scientific basis of the methodology of synthesis of true values of parameters of its physico-mechanical and density properties taking into account the influence of geodynamic impacts. The segments of continuous variation of homogeneous elastic uniform deformations are determined under overall compression of the medium. The limits of these segments are defined according to the criteria of instability (on geometric form changes and on "internal" instability). Analytical formulae are obtained to calculate current and limiting (critical) values of deformations within the framework of various variants of small and large initial deformations of the non-classically linearized approach of non-linear elastodynamics. The distribution of deformation becomes non-uniform in the medium while the limiting values of deformations are achieved. The proposed analytical formulae are applicable only within homogeneous distribution of deformations. Numerical experiments are carried out for various elastic potentials. It is found that various forms of instability can precede phase transitions and destruction. The influence of these deformation phenomena should be removed while the physico-mechanical and density parameters of the deformed media are determined. In particular, it is necessary to use the formulae proposed in this paper for this purpose.

        The Spread of Insertion Sequences Element and Transposons in Carbapenem Resistant Acinetobacter baumannii in a Hospital Setting in Southwestern Iran

        Background Acinetobacter baumannii is one of the most important hospital pathogenic bacteria that cause infectious diseases. The present study aimed to determine the frequency of carbapenem resistance genes in association with transposable elements and molecular typing of carbapenem-resistant A. baumannii bacteria collected from patients in Shiraz, Iran. Materials and Methods A total of 170 carbapenem-resistant A. baumannii isolates were obtained from different clinical specimens in two hospitals. The minimum inhibitory concentrations (MIC) of imipenem were determined and the prevalence of OXA Carbapenemases, Metallo-β-lactamases genes, insertion sequences (IS) elements, and transposons were evaluated by the polymerase chain reaction (PCR) method. Finally, molecular typing of the isolates was performed by the Enterobacterial Repetitive Intergenic Consensus-PCR method. Results The MICs ranged from 16 to 1,024 µg/mL for imipenem-resistant A. baumannii isolates. Out of the 170 carbapenem resistant A. baumannii isolates, blaOXA-24-like (94, 55.3%) followed by blaOXA-23-like (71, 41.7%) were predominant. In addition, A. baumannii isolates carried blaVIM (71, 41.7%), blaGES (32, 18.8%), blaSPM (4, 2.3%), and blaKPC (1, 0.6%). Moreover, ISAba1 (94.2%) and Tn2009 (39.2%) were the most frequent transposable elements. Furthermore, (71, 44.0%) and (161, 94.7%) of the ISAba1 of the isolates were associated with blaOXA-23 and blaOXA-51 genes, respectively. Besides (3, 1.7%), (1, 0.6%) and (5, 2.9%) of blaOXA-23 were associated with IS18, ISAba4, and ISAba2, respectively. Considering an 80.0% cut off, clusters and four singletons were detected. Conclusion According to the results, transposable elements played an important role in the development of resistance genes and resistance to carbapenems. The results also indicated carbapenem-resistant A. baumannii bacteria as a public health concern.
