Background: Duloxetine is an antidepressant that is also useful in chronic neuropathic and central origin pain. In this study, the role of duloxetine in decreasing acute postoperative pain after lumbar canal stenosis surgery is explored. Methods: In this single center, triple blinded, and placebo-controlled trial, 96 patients were randomized for statistical analysis. The intervention group received oral duloxetine 30 mg once a day (OD) for 2 days before surgery, 60 mg OD from the day of surgery to the postoperative second day and 30 mg OD for the next 2 days (a total duration of 7 days). A placebo capsule was given in the other group for a similar time and schedule. The same standard perioperative analgesia protocols were followed in both groups. Results: Total morphine consumption up to 24 hours was significantly decreased in the duloxetine group (P < 0.01). The time to the first analgesia requirement was similar in both groups but the time to the second and third dose of rescue analgesia increased significantly in the duloxetine group. The time to ambulation was decreased significantly (P < 0.01) in the duloxetine group as compared to the placebo group. Pain scores remained similar during most of the time interval. No significant difference was observed in the complication rate and patient satisfaction score recorded. Conclusions: Duloxetine reduces postoperative pain after lumbar canal stenosis surgery with no increase in adverse effects.

A review on surface deformation evaluation using multitemporal SAR interferometry techniques

Deformation monitoring is a crucial matter to avoid catastrophic failure due to extraction of resources. Radar data, mainly synthetic apreture radar (SAR) data and their diverse techniques are suitable for deformation monitoring. This paper provides a review of SAR data and their techniques for deformation monitoring. It also provides a brief description of Image acquisition, SAR geometry, SAR development, satellite specifications, and commercial software used for Interferometry and deformation monitoring. In the end, it is concluded that the existing SAR techniques are capable of accurate monitoring of land deformation. It is also concluded that persistant InSAR (PSInSAR) gives better results than differential InSAR (DInSAR).

We represent Paleoenvironmetal condition of Vestfold Hills region, based on the relative abundance of a single diatom species Fragilariopsis curta, a dominating sea-ice taxon. Increase in relative abundance of F. curta in a core from Vestfold Hills representing Holocene climatic history before 3820 years BP and 5560 year BP indicates the prevalence of colder climatic conditions, as this species is known to prefer the icier condition to thrive.

Backgrounds/Aims: En-bloc vein resection (VR) for pancreatic ductal adenocarcinoma (PDAC) of the head of pancreas adherent to the portomesenteric axis benefits patients when the vein wall is not infiltrated by tumour and an R0 resection is achieved, albeit at the expense of greater morbidity and mortality. Methods: A retrospective review of pancreaticoduodenectomy for PDAC over 6 years was conducted. Patients were divided into a standard resection group (Group SR) and simultaneous vein resection group (Group VR) and compared for outcome. Results: The study group consisted of 41 patients (Group SR 15, Group VR 26). VR was performed by end-to-end reconstruction in 12 patients and with interposition grafts in 13 cases (autologous vein in 10, PTFE in 3). R1 resections occurred in 49% patients, with the superior mesenteric artery margin most commonly involved. Patients with Ishikawa grade III and IV vein involvement were more likely to carry a positive SMA margin (p=0.04). Involvement of the splenoportal junction was associated with a significantly greater risk of pancreatic transection margin involvement. No difference in morbidity was seen between the groups. Median survival in the entire group of patients was 17 months and did not vary significantly between the groups. The only significant predictor of survival was lymph node status. Conclusions: Venous involvement by proximal PDAC is indicative of tumor location rather than tumor biology. VR improves outcomes in patients with tumor adhesion to the portomesenteric venous axis despite a high incidence of R1 resections and greater operative mortality.

High accuracy land use/land cover (LULC) mapping of Yamuna Chambal ravines for reclamation and conservation of these degraded/badlands is indispensable. Integration of freely available SAR datasets along with medium to high resolution optical data is one of the best approach for high accuracy LULC mapping. The objective of the presented study is to evaluate the fusion technique for Sentinel-1 SAR data and Sentinel-2 optical data for high accuracy LULC mapping in order to assess the area occupied by these negative landforms i.e., ravines. The VH-polarization fused image with Sentinel-2 optical data gives the best accuracy of 85% followed by VV-polarization fused image with same datasets of 84% accuracy whereas Sentinel-1 and Sentinel-2 provides the accuracy of 60 and 80%, respectively. The prepared LULC maps shown that bad land (Ravine class) occupied an area in the range of 600–700 km2 using combinations of different datasets as the wastelands in the area required immediate reclamation and conservation measures to be adopted. However, asymptotic performance of fusion technique for SAR and optical data further elucidate its successful implementation and dominancy over other datasets for improved LULC mapping.
Breast Cancer Awareness at the Community Level among Women in Delhi, India

Background: To assess women's awareness from diverse sections of society in Delhi regarding various aspects of breast cancer (BC) - perceptions, signs and symptoms, risk factors, prevention, screening and treatment. Materials and Methods: Community-level survey was undertaken in association with the Indian Cancer Society (ICS), Delhi during May 2013-March 2014. Women attending BC awareness workshops by ICS were given self-administered questionnaires before the workshop in the local language to assess BC literacy. Information provided by 2017 women was converted into awareness scores (aware=1) for analysis using SPSS. Awareness scores were dichotomized with median score=19 as cut off, create more aware and less aware categories. Bivariate and multivariate analysis provided P-values, odds ratios (ORs) and 95% confidence intervals (CIs). Results: Broadly, 53.4% women were aware about various aspects of BC. Notably, 49.1% women believed that BC was incurable and 73.9% women believed pain to be an initial BC symptom. Only 34.9% women performed breast self-examination (BSE) and 6.9% women had undergone clinical breast-examination/mammography. 40.5% women had higher awareness (awareness score > median score of 19), which was associated with education [graduates (OR=2.31; 95%CI=1.78, 3.16), post-graduates (OR=7.06; 95%CI=4.14, 12.05) compared to ${\leq}$ high school] and socio-economic status (SES) [low-middle (OR=4.20; 95%CI=2.72, 6.49), middle (OR=6.00; 95%CI=3.82, 9.42) and upper (OR=6.97; 95%CI=4.10, 11.84) compared to low SES]. Conclusions: BC awareness of women in Delhi was suboptimal and was associated with low SES and education. Awareness must be drastically increased via community outreach and use of media as a first step in the fight against BC.
