        Study of Work-Related Musculoskeletal Pain and Health-related Quality of Life among Hospital Workers

        Purpose: This study examined the work-related musculoskeletal pain and quality of life of hospital workers. Methods: Self-reported questionnaires were sent to 350 hospital workers at Seoul and Gyeonggi-do, of which 341 were returned. The questionnaire had four items that covered the demographic information, areas of musculoskeletal problems, pain rating scale, and quality of life. The analysis was completed using descriptive statistic, and the differences between pain and demographic variables were identified using a chi-square test. The differences between the type of occupation and quality of life were analyzed by one-way analysis of variance and a Kruskal-Wallis test. Results: The 12-month prevalence of work-related musculoskeletal pain was 86.1% of physical therapists, 86.5% of occupational therapists, 77.1% of dental hygienists, and 75.8% of nurses. A significant difference in the general and work related variables was observed between the subject with a physical burden and type of occupation. The most affected pain sites of the physical therapist included low back, hand and wrist, shoulder, and neck. The occupational therapists included the hand and wrist, shoulder, neck, but the nurses and dental hygienists reported the shoulder, back, hand and wrist. A significant difference in the quality of life was noted between the subjects in physical therapists and dental hygienists and the subjects in the nurses and occupational therapists (p=0.00). Conclusion: These findings show that hospital workers appear to be high risk for work-related musculoskeletal pain, and the quality of life of physical therapists and nurses was higher than that of dental hygienists and occupational therapists.

        FDG-PET/CT as prognostic factor and surveillance tool for postoperative radiation recurrence in locally advanced head and neck cancer

        Purpose: To evaluate the prognostic value of metabolic tumor volume (MTV) and maximum standardized uptake value (SUVmax) on initial positron emission tomography-computed tomography (PET-CT) and investigate the clinical value of SUVmax for early detection of locoregional recurrent disease after postoperative radiotherapy in patients with locally advanced head and neck squamous cell carcinoma (HNSCC). Materials and Methods: A total of 100 patients with locally advanced HNSCC received primary tumor excision and neck dissection followed by adjuvant radiotherapy with or without chemotherapy. The MTV and SUVmax were measured from primary sites and neck nodes. The prognostic value of MTV and SUVmax were assessed using initial staging PET/CT (study A). Follow-up PET/CT scan available after postoperative concurrent chemoradiotherapy or radiotherapy were evaluated for the SUVmax value and correlated with locoregional recurrence (study B). A receiver operating characteristic (ROC) curve analysis was used to define a threshold value of SUVmax with the highest accuracy for recurrent disease assessment. Results: High MTV (>41 mL) is negative prognostic factor for disease free survival (p = 0.041). Postradiation SUVmax was significantly correlated with locoregional recurrence (hazard ratio 1.812; 95% confidence interval 1.361 to 2.413; p < 0.001). A cut-off value of 5.38 from follow-up PET/CT was identified as having maximal accuracy for detecting locoregional recurrence by ROC analysis. Conclusion: MTV at staging work-up was significantly associated with disease free survival. The SUVmax value from follow-up PET/CT showed high diagnostic accuracy for the detection of locoregional recurrence in postoperatively irradiated HNSCC.

This study was conducted to identify the relationship between work-related musculoskeletal pain and quality of life of physical therapists. Methods: A self-reported questionnaires was sent to 200 physical therapists at in Seoul and Kyoungido. The questionnaires was returned by 170 physical therapists. The questionnaire had included 4 items that coveringed demographic information, areas of musculoskeletal problems, pain rating scale, and WHOQOL-BREF. The analysis was completed using descriptive statistics, and differences between pain and demographic variables were identified using the chi-square test. The relationship between work-related musculoskeletal pain and quality of life was analyzed by t-test and Pearson's correlation. Results: The overall prevalence of work-related musculoskeletal pain was 76.8%. The most affected pain sites included the low back (48.8%), shoulder (45.,2%), hand and wrist (43.5%), and neck (33.3%). Pain ratings of subjects with pain was were moderate. There was a A significant difference for the subdomains of quality of life was observed between the subjects with musculoskeletal pain and those without pain. Weak negative correlations (r=-0.28) were observed between pain rating scale and QOL. Conclusion: These findings show that physical therapists appear to be at a higher risk for work-related musculoskeletal pain and physical domain of QOL. Therefore, Ffurther research is needed to investigate examine the effect of risk factors and ergonomics as physical load, general health status on prevalence of musculoskeletal pain.

      • A Content-Aware Video Encoding Scheme Based on Single-Pass Consistent Quality Control

        <P>In real-time video streaming applications based on battery-powered camera systems, it is crucial to capture and transmit critical information satisfying the storage capacity and channel bandwidth requirement. In this paper, we propose a single-pass content-aware video encoding scheme for such applications to achieve a consistent video quality while satisfying the bit-rate constraints. To achieve the objective we developed: 1) scene descriptor based on the discrete cosine transformation coefficients obtained from the video encoder; 2) a parameterized analytic distortion model for video encoding using the scene descriptor; 3) scene change detection based on the encoded MB mode ratio; and 4) a single-pass consistent video quality control scheme which exploits 1), 2), and 3). Experimental results show that the proposed method results in less deviation from the target distortion and less distortion variance than the up-to-date best result by up to 65.25% and 75.88%, respectively, while satisfying the given rate constraints. The computing time overhead in the proposed method was 0.560% and 0.571% for the variable and constrained variable bit-rate mode, respectively, which is negligible in nearly all embedded applications.</P>

        Motion Recognition for Kinect Sensor Data Using Machine Learning Algorithm with PNF Patterns of Upper Extremities

        Purpose: The purpose of this study was to investigate the availability of software for rehabilitation with the Kinect sensor by presenting an efficient algorithm based on machine learning when classifying the motion data of the PNF pattern if the subjects were wearing a patient gown. Methods: The motion data of the PNF pattern for upper extremities were collected by Kinect sensor. The data were obtained from 8 normal university students without the limitation of upper extremities. The subjects, wearing a T-shirt, performed the PNF patterns, D1 and D2 flexion, extensions, 30 times; the same protocol was repeated while wearing a patient gown to compare the classification performance of algorithms. For comparison of performance, we chose four algorithms, Naive Bayes Classifier, C4.5, Multilayer Perceptron, and Hidden Markov Model. The motion data for wearing a T-shirt were used for the training set, and 10 fold cross-validation test was performed. The motion data for wearing a gown were used for the test set. Results: The results showed that all of the algorithms performed well with 10 fold cross-validation test. However, when classifying the data with a hospital gown, Hidden Markov model (HMM) was the best algorithm for classifying the motion of PNF. Conclusion: We showed that HMM is the most efficient algorithm that could handle the sequence data related to time. Thus, we suggested that the algorithm which considered the sequence of motion, such as HMM, would be selected when developing software for rehabilitation which required determining the correctness of the motion.

        최근 네트워크가 점점 복잡해짐에 따라 많은 수의 미들박스를 동적으로 유연하게 관리할 필요성이 증가하고 있으며, 미들박스 설정이 사업자의 정책과 다르게 잘못 설정되는 경우가 빈번하기 때문에 미들박스를 효율적으로 관리할 수 있는 기법이 절실한 상황이다. 이러한 미들박스 관리의 어려움은 SDN (Software Defined Networking)의 중앙 집중화된 컨트롤러 구조와 유연한 프로그래밍 능력을 통해 해결할 수 있다. 즉, 네트워크 상황에 맞춰 동적으로 미들박스 정책을 적용하고, 데이터/컨트롤 평면의 분리를 통해 기존 미들박스 구조는 그대로 유지한 채 새로운 컨트롤 평면을 추가하는 것이 가능하다. 또한 클라우드와 분산 네트워크 기능 가상화 (NFV : Network Function Virtualization) 기술을 통해 보다 유연하게 미들박스를 관리하는 방안도 가능하다. 본 논문에서는 유선망과 무선망이 통합된 네트워크에서 SDN 기반의 미들박스 관리 기법과 클라우드 기반의 미들박스 관리 방안에 대해 알아보고 향후 연구 이슈에 대해서 살펴본다. Recently, middleboxes play a key role in many network settings such as firewalls, VPN gateways, proxies, intrusion detection and prevention systems, and WAN optimizers. However, achieving the performance and security benefits that middleboxes offer is highly complex, and therefore it is essential to manage middleboxes efficiently and dynamically. In this respect, Software-Defined Networking (SDN) offers a promising solution for middlebox policy enforcement by using logically centralized management, decoupling the data and control planes, and providing the ability to programmatically configure forwarding rules. Also, cloud computing and distributed Network Function Virtualization (NFV) can enable to manage middleboxes more easily. We introduce SDN-based middlebox management framework in integrated wired and wireless networks and discuss the further issues.
