Physical therapy students’ perception of their ability of clinical and clinical decision-making skills enhanced after simulation-based learning courses in the United States: a repeated measures design

        Fabian Bizama,Mansoor Alameri,Kristy Jean Demers,Derrick Ferguson Campbell 한국보건의료인국가시험원 2022 보건의료교육평가 Vol.19 No.-

Purpose It aimed to investigate physical therapy students’ perception of their ability of clinical and clinical decision-making skills after a simulation-based learning course in the United States. Methods Survey questionnaires were administered to voluntary participants, including 44 second and third-year physical therapy students of the University of St. Augustine for Health Sciences during 2021–2022. Thirty-six questionnaire items consisted of 4 demographic items, 1 general evaluation, 21 test items for clinical decision-making skills, and 4 clinical skill items. Descriptive and inferential statistics evaluated differences in students’ perception of their ability in clinical decision-making and clinical skills, pre- and post-simulation, and post-first clinical experience during 2021–2022. Results Friedman test revealed a significant increase from pre- to post-simulation in perception of the ability of clinical and clinical decision-making skills total tool score (P<0.001), clinical decision-making 21-item score (P<0.001), and clinical skills score (P<0.001). No significant differences were found between post-simulation and post-first clinical experience. Post-hoc tests indicated a significant difference between pre-simulation and post-simulation (P<0.001) and between pre-simulation and post-first clinical experience (P<0.001). Forty-three students (97.6%) either strongly agreed (59.1%) or agreed (38.5%) that simulation was a valuable learning experience. Conclusion The above findings suggest that simulation-based learning helped students begin their first clinical experience with enhanced clinical and clinical decision-making skills.

Quantitative expression and localization of cysteine and aspartic proteases in human abdominal aortic aneurysms

        Fabian Lohoefer,Christian Reeps,Christina Lipp,Martina Rudelius,Felix Haertl,Edouard Matevossian,Alma Zernecke,Hans-Henning Eckstein,Jaroslav Pelisek 생화학분자생물학회 2014 Experimental and molecular medicine Vol.46 No.-

Cysteine and aspartic proteases possess high elastolytic activity and might contribute to the degradation of the abdominal aortic aneurysm (AAA) wall. The aim of this study was to analyze, in detail, the proteases (cathepsins B, D, K, L and S, and inhibitor cystatin C) found in human AAA and healthy aortic tissue samples. The vessel walls from AAA patients (n¼36) and nonaneurysmal aortae (n¼10) were retrieved using conventional surgical repair and autopsy methods. Serum samples from the same AAA patients and 10 healthy volunteers were also collected. Quantitative expression analyses were performed at the mRNA level using real-time reverse transcriptase-PCR (RT–PCR). Furthermore, analyses at the protein level included western blot and immunoprecipitation analyses. Cellular sources of cysteine/aspartic proteases and cystatin C were identified by immunohistochemistry (IHC). All cysteine/aspartic proteases and cystatin C were detected in the AAA and control samples.Using quantitative RT–PCR, a significant increase in expression was observed for cathepsins B (P¼0.021) and L (P¼0.018),compared with the controls. Cathepsin B and cystatin C were also detected in the serum of AAA patients. Using IHC, smooth muscle cells (SMCs) and macrophages were positive for all of the tested cathepsins, as well as cystatin C; in addition, the lymphocytes were mainly positive for cathepsin B, followed by cathepsins D and S. All cysteine/aspartic proteases analyzed in our study were detected in the AAA and healthy aorta. The highest expression was found in macrophages and SMCs. Consequently, cysteine/aspartic proteases might play a substantial role in AAA.

U6 is unsuitable for normalization of serum miRNA levels in patients with sepsis or liver fibrosis

        Fabian Benz,Christoph Roderburg,David Vargas Cardenas,Mihael Vucur,Jeremie Gautheron,Alexander Koch,Henning Zimmermann,Jorn Janssen,Lukas Nieuwenhuijsen,Mark Luedde,Norbert Frey,Frank Tacke,Christian 생화학분자생물학회 2013 Experimental and molecular medicine Vol.45 No.9

MicroRNA (miRNA) levels in serum have recently emerged as potential novel biomarkers for various diseases. miRNAs are routinely measured by standard quantitative real-time PCR (qPCR); however, the high sensitivity of qPCR demands appropriate normalization to correct for nonbiological variation. Presently, RNU6B (U6) is used for data normalization of circulating miRNAs in many studies. However, it was suggested that serum levels of U6 themselves might differ between individuals. Therefore, no consensus has been reached on the best normalization strategy in ‘circulating miRNA’. We analyzed U6 levels as well as levels of spiked-in SV40-RNA in sera of 44 healthy volunteers, 203 intensive care unit patients and 64 patients with liver fibrosis. Levels of U6 demonstrated a high variability in sera of healthy donors, patients with critical illness and liver fibrosis. This high variability could also be confirmed in sera of mice after the cecal ligation and puncture procedure. Most importantly, levels of circulating U6 were significantly upregulated in sera of patients with critical illness and sepsis compared with controls and correlated with established markers of inflammation. In patients with liver fibrosis, U6 levels were significantly downregulated. In contrast, levels of spiked-in SV40 displayed a significantly higher stability both in human cohorts (healthy, critical illness, liver fibrosis) and in mice. Thus, we conclude that U6 levels in the serum are dysregulated in a disease-specific manner. Therefore, U6 should not be used for data normalization of circulating miRNAs in inflammatory diseases and previous studies using this approach should be interpreted with caution. Further studies are warranted to identify specific regulatory processes of U6 levels in sepsis and liver fibrosis.

Learning Deep Representation by Increasing ConvNets Depth for Few Shot Learning

Fabian, H.S. Tan,Kang, Dae-Ki The Institute of Internet 2019 International journal of advanced smart convergenc Vol.8 No.4

Though recent advancement of deep learning methods have provided satisfactory results from large data domain, somehow yield poor performance on few-shot classification tasks. In order to train a model with strong performance, i.e. deep convolutional neural network, it depends heavily on huge dataset and the labeled classes of the dataset can be extremely humongous. The cost of human annotation and scarcity of the data among the classes have drastically limited the capability of current image classification model. On the contrary, humans are excellent in terms of learning or recognizing new unseen classes with merely small set of labeled examples. Few-shot learning aims to train a classification model with limited labeled samples to recognize new classes that have neverseen during training process. In this paper, we increase the backbone depth of the embedding network in orderto learn the variation between the intra-class. By increasing the network depth of the embedding module, we are able to achieve competitive performance due to the minimized intra-class variation.

Annexin A6 is highly abundant in monocytes of obese and type 2 diabetic individuals and is downregulated by adiponectin in vitro

        Fabian Stögbauer,Johanna Weigert,Markus Neumeier,Josef Wanninger,Daniela Sporrer,Markus Weber,Andreas Schäffler,Carlos Enrich,Peta Wood,Thomas Grewal,Charalampos Aslanidis,Christa Buechler 생화학분자생물학회 2009 Experimental and molecular medicine Vol.41 No.7

Adiponectin stimulates cholesterol efflux in macrophages and low adiponectin may in part contribute to disturbed reverse cholesterol transport in type 2 diabetes. Monocytes express high levels of annexin A6 that could inhibit cholesterol efflux and it was investigated whether the atheroprotective effects of adiponectin are accompanied by changes in annexin A6 levels. Adiponectin reduces annexin A6 protein whereas mRNA levels are not affected. Adiponectin-mediated activation of peroxisome proliferator-activated receptor α (PPARα) and AMP-activated protein kinase (AMPK) does not account for reduced annexin A6 expression. Further, fatty acids and lipopolysaccharide that are elevated in obesity do not influence annexin A6 protein levels. Annexin A6 in monocytes from overweight probands or type 2 diabetic patients is significantly elevated compared to monocytes of normal- weight controls. Monocytic annexin A6 positively correlates with body mass index and negatively with systemic adiponectin of the blood donors. Therefore, the current study demonstrates that adiponectin reduces annexin A6 in monocytes and thereby may enhance cholesterol efflux. In agreement with these in vitro finding an increase of monocytic annexin A6 in type 2 diabetes monocytes was observed.

Vol.30 No.3

In Chile, since the restitution of democracy (1989), the distribution of parliamentary seats is governed by a binomial system, the only one of its kind in the world. In this electoral system, two senators and two deputies are elected for each senatorial constituency and electoral district. By the end of 2017, this system will be replaced by the well-known D’Hondt method, which entails a significant increase in the number of parliamentarians and the redistribution of seats in the legislative chambers. In this study, we discuss the problem of centralism and analyze how this redistribution of seats will affect the voting power of the different regions of the country. To measure the hypothetical voting power of each region we use classical power indices of cooperative game theory. The study shows that although the voting power of the regions in the Senate will remain relatively unchanged, the centralization of power in the Metropolitan Region will be increased in the Chamber of Deputies. Finally, we propose a method to obtain a feasible redistributions of seats, which would effectively increase the voting power of the other regions in the Chamber of Deputies.
