Wake Up Time, Light, and Mood in a Population Sample Age 40–64 Years

        Takuro Endo,Daniel F. Kripke,Sonia Ancoli-Israel 대한신경정신의학회 2015 PSYCHIATRY INVESTIGATION Vol.12 No.2

Objective-Concern that disturbances of sleep and light exposures at night might increase cancer risks have been expressed, but little actual exposure data has been collected. Measurements from a representative population sample were examined to understand the magnitude of in-bed light exposure at night and possible correlates. Methods-From 1990 to 1994, a home survey of sleep disorders among adults ages 40–64 was conducted in the City of San Diego California, using stratified representative sampling techniques. Along with questionnaires, sleep logs, and 3-night wrist activity and pulse oximetry measures, bedside illumination was measured with a computer recording system. Questionnaires included the CESD depression scale and a scale of symptoms typical of winter depression. Results-Complete data were available from 286 men and women, whose mean in-bed intervals averaged 7 hours and 42 minutes. The mean room illumination during the first part of the night was mean 12.7 lux (median 3.2 lux) and during the last 2 hours in bed averaged 28.7 lux (median 18.9 lux). Nocturnal light exposure was positively correlated with age, male gender, summer season, time in bed, wake-up time, and depressive symptoms. Conclusion-Complex bi-directional interactions may take place between sleep disturbances, depression, time in bed, wake-up-time, and in-bed illumination. The most crucial light exposures appear to occur in the last 2 hours in bed, largely after dawn, so daylight exposure may be an important factor.

Technological Innovation and Multiple- and Single-Sourcing Policies In the Automobile Parts Trade

Obayashi, Atsuomi,Endo, Takuro Korean Institute of Industrial Engineers 2005 Industrial Engineeering & Management Systems Vol.4 No.2

The single sourcing policy, in which an automobile manufacturer purchases identical or similar parts from one supplier, has an advantage of scale economy. Meanwhile, multiple sourcing policy, which allows procuring similar parts from multiple suppliers, has benefits of dispersing risks and promoting competition among suppliers. This paper analyzes the procurement policies by presenting a model of the Japanese automobile parts trade. It concludes that maturity of technology involved should be taken into account besides above-mentioned factors which have traditionally been recognized. For parts produced using evolving technologies, the single sourcing enhances purchaser’s benefits because of the scale economy in learning process. In the meantime, multiple sourcing is more beneficial to the purchaser if the parts are based on mature technologies. In either policy, if the technology involved is evolving, motivating suppliers by returning a great part of cost reduction as a reward to them may eventually increase profit for the purchaser. The conclusion supports the situation where the number of suppliers is being cut down as the trend of modularization and system deliveries of parts progresses in the auto parts industry, and suggests that returning part of benefits to parts suppliers may be encouraged from the viewpoint of auto manufacturers’ own interest.

Surgical Outcomes of Full Endoscopic Posterior Cervical Foraminotomy for Proximal Cervical Spondylotic Amyotrophy

        Lee Deokcheol,Ohmori Kazuo,Yoneyama Reiko,Endo Takuro,Endo Yasuhiro 대한척추외과학회 2024 Asian Spine Journal Vol.18 No.1

Study Design: Retrospective analysis of case series. Purpose: This study aimed to clarify the effects of full endoscopic posterior cervical foraminotomy (FPCF) on cervical spondylotic amyotrophy (CSA). Overview of Literature: The method for decompressing the ventral nerve root and anterior horn (AH) in CSA is controversial. Methods: Patients without myelopathy who underwent FPCF for proximal CSA between 2017 and 2022 were analyzed. The outcome measure was the results of the manual muscle testing (MMT) of the deltoid and biceps. Preoperative nerve root and AH compression were evaluated by magnetic resonance imaging. The intervertebral foramen morphology and bony decompression extent were evaluated by computed tomography. Results: FPCF was performed at the C4/5 level and at the C4/5 and C5/6 levels in 14 and 11 patients, respectively. The width of the narrowest intervertebral foramen was significantly narrower on the affected side than on the healthy side at the C4/5 (2.5 mm vs. 3.6 mm) and operated C5/6 (1.9 mm vs. 3.1 mm) levels. AH compression occurred at the C4/5 and C5/6 levels in 28% and 21% of the patients, respectively. Bony decompression was performed laterally beyond the narrowest foramen at the C4/5 and C5/6 levels in 96% and 91% of the patients, respectively. Compared with patients without AH compression, in those with AH compression, the lamina was resected medially by an average of >1.7 mm and >3.6 mm at the C4/5 and C5/6 levels, respectively. Furthermore, 76% and 81% of the facet joint surfaces were preserved at the C4/5 and C5/6 levels, respectively. Postoperative MMT grade improvement was excellent, good, and fair in 64%, 20%, and 16% of the patients, respectively. Conclusions: FPCF was effective for treating proximal CSA.
