        Human Acellular Dermis versus Submuscular Tissue Expander Breast Reconstruction: A Multivariate Analysis of Short-Term Complications

        Background Acellular dermal matrix (ADM) allografts and their putative benefits have been increasingly described in prosthesis based breast reconstruction. There have been a myriad of analyses outlining ADM complication profiles, but few large-scale, multi-institutional studies exploring these outcomes. In this study, complication rates of acellular dermis-assisted tissue expander breast reconstruction were compared with traditional submuscular methods by evaluation of the American College of Surgeon's National Surgical Quality Improvement Program (NSQIP) registry. Methods Patients who underwent immediate tissue expander breast reconstruction from 2006-2010 were identified using surgical procedure codes. Two hundred forty tracked variables from over 250 participating sites were extracted for patients undergoing acellular dermis-assisted versus submuscular tissue expander reconstruction. Thirty-day postoperative outcomes and captured risk factors for complications were compared between the two groups. Results A total of 9,159 patients underwent tissue expander breast reconstruction; 1,717 using acellular dermis and 7,442 with submuscular expander placement. Total complications and reconstruction related complications were similar in both cohorts (5.5% vs. 5.3%, P=0.68 and 4.7% vs. 4.3%, P=0.39, respectively). Multivariate logistic regression revealed body mass index and smoking as independent risk factors for reconstructive complications in both cohorts (P<0.01). Conclusions The NSQIP database provides large-scale, multi-institutional, independent outcomes for acellular dermis and submuscular breast reconstruction. Both thirty-day complication profiles and risk factors for post operative morbidity are similar between these two reconstructive approaches.

        La migración coreana en Yucatán. Procesos de integración y movilidad social y geográfica

        A boat with just over a thousand Koreans arrived in 1905 in Yucatán. They were hired to work as laborers in 32 haciendas henequeneras located throughout the state. From a historical vision in which documentary, oral and bibliographic sources were used, this text aims to reveal the main characteristics of Korean immigration, analyzing the integration process and the social and geographical mobility experienced by Koreans throughout the twentieth century, including in the analysis the experiences of their descendants who are still living in Yucatan and more particularly in the town of Motul. The geographic dispersion, lack of Korean women and the absence of chain migration were some of the factors that turned particular this historical migration. En 1905 llegó a Yucatán un barco con poco más de mil coreanos que fueron contratados para trabajar como jornaleros en 32 haciendas henequeneras distribuidas por todo el estado. Desde una visión histórica en la que se recurrió a fuentes documentales, orales y bibliográficas, este texto tiene el objetivo de desvelar las principales características de la migración coreana, analizando los proceso de integración, así como la movilidad social y geográfica que experimentaron los coreanos a lo largo del siglo XX, incluyendo en el análisis las vivencias de sus descendientes que aún se encuentran viviendo en Yucatán y más particularmente en el municipio de Motul. La dispersión geográfica, la falta de mujeres coreanas y la ausencia de una cadena migratoria fueron algunos de los factores que volvieron particular esta migración histórica.

      • Human Papilloma Virus Detection by INNOLiPA HPV in Prostate Tissue from Men of Northeast Mexico

        Background: Prostatic adenocarcinoma by Prosate cancer (PCa) is the most prevalent cancer and the second cause of cancer-related death among men in the Western world. Human papilloma virus (HPV) may be considered as a preventable risk factor. In this study, we assessed the frequencies of HPV infection in prostatic adenocarcinoma and benign prostatic hyperplasia (BPH) cases in Northeast Mexico. Materials and Methods: A total of 87 paraffin-embedded blocks (from 25 and 62 patients with definite diagnoses of BPH and adenocarcinoma, respectively) were selected and subjected to INNOLiPA HPV Genotyping to detect 28 high- and low-risk HPV types. The rates of infection were compared in the two studied groups. Results: INNOLiPA HPV demonstrated great sensitivity for HPV detection on paraffin-embedded tissue. Global prevalence was 14.9% (13/87). HPV infection was positive in 19.4% (12/62) of patients with adenocarcinoma and 4.0% (1/25) of patients with BPH. HPV-11, which is considered to be low risk, was more prevalent. Interestingly, one patient with BPH and six with prostate cancer showed examples considered to be high risk (HPV-18, -51, -52, and -66). Conclusion: A higher rate of HPV infection among Mexican patients with prostatic carcinoma than among those with BPH was observed. HPV infections may thus contribute to the risk of prostate cancer. Further studies are required to elucidate any roles of HPV infection in prostate disease in Mexico and the effect of prevention and treatment of HPV infection on prostatic adenocarcinoma.

        Tunnelling on terrace soil deposits: Characterization and experiences on the Bogota-Villavicencio road

        Terrace deposits are often encountered in portal areas and tunnels with low overburden. They are challenging to excavate considering their great mechanical and spatial heterogeneity and a very high stiffness contrast within the ground. Terrace deposits are difficult to characterize, considering that samples for laboratory testing are almost unfeasible to obtain, and laboratory tests may not be representative due to scale effects. This paper presents the approach taken for their characterization during the design stage and their posterior validation performed during construction. Lessons learned from several tunnels excavated on terrace deposits on the Bogota-Villavicencio road (central-east Colombia), suggest that based on numerical simulations, laboratory testing and tunnel system behaviour monitoring, an observational approach allows engineers to optimize the excavation and support methods for the encountered ground conditions, resulting in a more economic and safe construction.

        A fecalith mimicking a bladder calculus secondary to an appendicovesical fistula: a case report

        An appendicovesical fistula is defined as an abnormal communication between the appendix and the urinary bladder, with only a few cases reported in the literature. It is very challenging to make an early diagnosis, due to the inability of conventional imaging modalities to detect this unique pathology. The symptoms are often mild, and there are not any specific signs or symptoms that might suggest this type of anomalous communication. We report a case of a 27-year-old male patient who presented difficulty for initiating urination, dysuria, and persistent urinary tract infections. An abdominal x-ray showed a large calculus inside the bladder. A cystoscopy was performed, where the tip of the appendix was seen protruding inside the bladder with a large fecalith adhered to the bladder wall. An appendectomy and partial cystectomy with primary repair were auspiciously achieved. A review of the literature is also presented.

      • eIF4E phosphorylation by MST1 reduces translation of a subset of mRNAs, but increases lncRNA translation

        Post-transcriptional gene regulation is an important step in eukaryotic gene expression. The last step to govern production of nascent peptides is during the process of mRNA translation. mRNA translation is controlled by many translation initiation factors that are susceptible to post-translational modifications. Here we report that one of the translation initiation factors, eIF4E, is phosphorylated by Mammalian Ste20-like kinase (MST1). Upon phosphorylation, eIF4E weakly interacts with the 5' CAP to inhibit mRNA translation. Simultaneously, active polyribosome is more associated with long noncoding RNAs (lncRNAs). Moreover, the linc00689-derived micropeptide, STORM (Stress- and TNF-α-activated ORF Micropeptide), is triggered by TNF-α-induced and MST1-mediated eIF4E phosphorylation, which exhibits molecular mimicry of SRP19 and, thus, competes for 7SL RNA. Our findings have uncovered a novel function of MST1 in mRNA and lncRNA translation by direct phosphorylation of eIF4E. This novel signaling pathway will provide new platforms for regulation of mRNA translation via post-translational protein modification.

        CORONAL TEMPERATURE, DENSITY AND NONTHERMAL VELOCITY DERIVED FROM SERTS EUV SPECTRA

        To derive coronal temperature, electron density and nonthermal velocity, we have analyzed high resolution spectra (e.g., Fe XII 338.3, Fe XII 352.1, Fe XIV 334.2, Fe XIV 353.8, Fe XV 284.2, Fe XV 321.8, Fe XV 327.0, Fe XVI 335.4, and Fe XVI 360.8) taken from AR 6615 by SERTS (Solar Extreme Ultraviolet Rocket Telescope and Spectrograph). Important findings emerging from the present study are as follows: (1) Temperature estimated from Fe XVI 335.4 and Fe XIV, 334.2 is $\~2.4\times10^6 K$ and no systematic difference in temperature is found between the active region and its adjacent quiet region; (2) Mean electron density estimated from Fe XV is $\~3\times10^9 cm^{-3}\;and\;\~10^{10} cm^{-3}$ from Fe XII and Fe XIV; (3) Mean density of the active region is found to be higher than that of the quiet region by a factor of 2; (4) Nonthermal velocity estimated from Fe XV and Fe XVI is $20\times25 km\;s^{-l}$ which decreases with increasing ionization temperatures. This supports the notion that the nonthermal velocity declines outwards above the transition region.
