Since 1970s, energy-saving design criteria of buildings has been improved through numerous revisions. The purpose of this research is to show how energy saving design criteria affects heating energy consumption of apartments using district heating. Heating energy consumption has been measured in 4 apartments located in Gyeonggi province, Korea from Nov. 1, 2007 through Oct. 31, 2008. Collected data was regressed to linear correlations. Heating energy consumptions were calculated for past, present and future energy-saving design criteria, which are outdoor temperature, ventilation and insulation. The results show that present design criteria has reduced heating energy consumption by 15%, and the future criteria will reduce the energy consumption by 42% compared to the criteria before 2001.

OBJECTIVE: The objective of this study was to define the radiological characteristics of 2-phase computed tomography (CT) of parotid gland Warthin tumors (WTs) with a pathologic basis for these findings. METHODS: We prospectively enrolled 116 patients with parotid gland tumor who underwent preoperative 2-phase CT scans(scanning delays of 30 and 120 seconds). The attenuation changes and enhancement patterns were analyzed according to pathology. We also evaluated size-matched samples of WTs and pleomorphic adenoma by staining CD31, vascular endothelial growth factor-receptor 2, collagen IV, and smooth muscle actin. RESULTS: Computed tomography numbers in WTs were significantly higher than those in other tumors in early-phase scans and lower in delayed scans. Pathologically, CD31(+) blood vessel area was significantly higher in WTs than in pleomorphic adenomas. In addition, WTs had an extensive capillary network and many leaky blood vessels. CONCLUSIONS: The enhancement pattern of early fill-in and early washout is the typical finding of WTs on 2-phase CT scans, which may be attributed pathologically to abundant blood vessel and extensive capillary network.

BACKGROUND AND AIM: The endoscopic techniques for treating a small rectal neuroendocrine tumor (NET) are performed in most large centers; however, the endoscopic management of this condition is not well established. This study was designed to determine the long-term prognosis of endoscopically resected NET. METHODS: We prospectively studied patients with endoscopically treated rectal NET in Seoul St Mary’s hospital, between January 2000 and June 2010. The long-term outcomes were analyzed in association with the pathological complete resection and resection procedures. RESULTS: Seventy-seven patients (48 men and 29 women; mean age, 52.3 years; range, 23–77 years) were included. The average NET size was 7.0±2.8 mm (range, 3–16 mm). There was no procedure-related complication. En-bloc removal was achieved for all lesions, and the rate of histological complete resection was 75.3% (58/77). Histological complete resection rates were 71.4% (10/14) by conventional endoscopic mucosal resection (EMR), 74.1% (43/58) by a two-channel EMR, and 100.0% (5/5) by endoscopic submucosal dissection (ESD). Among six patients with incomplete histological resection, two underwent additional EMR, two underwent transanal endoscopic microsurgery, and two underwent low anterior section with lymph node dissection. The remaining 13 patients with ‘possible’ remnant NET underwent regular endoscopic surveillance without additional resection. In the latter group, only one patient had local recurrence, detected on regular colonoscopic surveillance, after 56 months and was treated with additional EMR. All of the patients are alive and 98.7% (76/77) of the patients are free from disease during the follow-up periods. CONCLUSION: Endoscopic resection is a safe and effective modality and may potentially be used for the treatment of NETs smaller than 15 mm in diameter, those confined to the submucosal layer, and those without metastasis. Local treatment was believed to be curative in cases with complete histological resection. In addition, this treatment may have an excellent prognosis in patients with ‘possible’ remnant NET.

<P><B>Summary</B></P><P>The present study investigated the clinical usefulness of plasma real‐time polymerase chain reaction (PCR) (plasma‐PCR) in the prevention of BK virus‐associated nephropathy (BKVAN). First, we investigated the diagnostic value of plasma BK‐PCR, urine BK‐PCR, and urine cytology for the prediction of BKVAN retrospectively. Then we designed a prospective study of regular plasma‐PCR monitoring and pre‐emptive immunosuppression (IS) reduction based on the result. In the retrospective cohort, the prevalence of BKVAN was 3.7% (14/379) and the positive rate of decoy cells, urine‐PCR (>1 × 10<SUP>10</SUP> copies/ml), and plasma‐PCR (>1 × 10<SUP>4</SUP> copies/ml) was 18.6%, 11.1%, and 5.5%, respectively. Plasma‐PCR was superior to urine‐PCR or urine cytology in specificity and positive predictive value for detection of BKVAN. In prospective study, regular monitoring of plasma‐PCR detected significant BKV viremia in 8.3% (12/145) and BKVAN in 1 patient (0.6%). After IS reduction, BKV viremia was eliminated in 91.6% (11/12) within 103 days (25–254). In patients with viremia, the frequency of acute rejection did not increase and allograft function did not differ significantly compared with those in patients without viremia during the first year post‐transplant (<I>P</I> > 0.05, in both). Plasma‐PCR is useful to predict an increased risk for BKVAN, and regular monitoring is effective to prevent the development of BKVAN.</P>

<P>The current study demonstrates the process of selecting an optimal structure for a caspase-3-cleavable doxorubicin prodrug that could be synthesized by simple chemistry in high yields. The prodrug was intended to activate in the presence of caspase-3, whose expression can be exogenously regulated by inducing apoptosis with radiation therapy at a specific site of interest. For this purpose, doxorubicin was conjugated with a DEVD peptide via a heterobifunctional linker. Since the active form of the prodrug comprises the linker besides doxorubicin, we tested several different linkers and selected EMCS based on the examination of its <I>in vitro</I> biological activities. Consequently, DEVD-cysteamide-EMCS-doxorubicin was synthesized as the final compound. <P>The integration of carbon nanotube (CNT)-based sensor and readout complementary metal-oxide-semiconductor integrated chip (CMOS IC) to detect hydrogen gas in a single chip is presented. First, we have fabricated the CMOS IC using the standard 0.35-μm CMOS process. Then, we have built 8 × 8 CNT-based sensor cells on it using a proposed tractable postprocessing strategy and judicious electrode scheme. The fabricated sensor IC can operate down to 10-ppm concentration of hydrogen in air as a hydrogen sensor. This paper is expected to have a major impact upon the integration of the CNT technology with CMOS technology and be extended to the development of CMOS IC integrated with various nanomaterials.</P>

Vasoconstriction Potency Induced by Aminoamide Local Anesthetics Correlates with Lipid Solubility

<P>Aminoamide local anesthetics induce vasoconstriction <I>in vivo</I> and <I>in vitro</I>. The goals of this <I>in vitro</I> study were to investigate the potency of local anesthetic-induced vasoconstriction and to identify the physicochemical property (octanol/buffer partition coefficient, pKa, molecular weight, or potency) of local anesthetics that determines their potency in inducing isolated rat aortic ring contraction. Cumulative concentration-response curves to local anesthetics (levobupivacaine, ropivacaine, lidocaine, and mepivacaine) were obtained from isolated rat aorta. Regression analyses were performed to determine the relationship between the reported physicochemical properties of local anesthetics and the local anesthetic concentration that produced 50% (ED<SUB>50</SUB>) of the local anesthetic-induced maximum vasoconstriction. We determined the order of potency (ED<SUB>50</SUB>) of vasoconstriction among local anesthetics to be levobupivacaine > ropivacaine > lidocaine > mepivacaine. The relative importance of the independent variables that affect the vasoconstriction potency is octanol/buffer partition coefficient > potency > pKa > molecular weight. The ED<SUB>50</SUB> in endothelium-denuded aorta negatively correlated with the octanol/buffer partition coefficient of local anesthetics (<I>r</I><SUP>2</SUP> = 0.9563; <I>P</I> < 0.001). The potency of the vasoconstriction in the endothelium-denuded aorta induced by local anesthetics is determined primarily by lipid solubility and, in part, by other physicochemical properties including potency and pKa.</P>
