Objective: To investigate individual and interactive associations of baseline serum leptin levels and physical comorbidity with short- and long-term treatment outcomes in outpatients with depressive disorders who received stepwise antidepressant treatment in a naturalistic prospective study design. Methods: Baseline serum leptin levels were measured, and the number of concurrent physical disorders ascertained from 1,094 patients. These patients received initial antidepressant monotherapy; then, for patients with an insufficient response or who experienced uncomfortable side effects, treatment was administered using alternative strategies every 3 weeks in the acute treatment phase (at 3, 6, 9, and 12 weeks) and every 3 months in the continuation treatment phase (at 6, 9, and 12 months). Then, 12-week and 12-month remission, defined as a Hamilton Depression Rating Scale score of ≤7, was estimated. Results: In multivariable logistic regression analyses, individual effects were found only between higher baseline serum leptin levels and 12-week non-remission. Significant interactive effects between higher leptin levels and fewer physical disorders (< 2 physical disorders) on 12-week non-remission were observed. However, neither individual nor interactive effects between leptin levels and physical comorbidity were associated with 12-month remission. Conclusion: The combination of serum leptin level and number of physical disorders may be a useful predictor of short-term treatment responses in patients with depressive disorders receiving pharmacotherapy.

BACKGROUND: Two-dimensional (2D) strain provides more predictive power than ejection fraction (EF) in patients with ST-elevation myocardial infarction (STEMI). 3D strain and EF are also expected to have better clinical usefulness and overcome several inherent limitations of 2D strain. We aimed to clarify the prognostic significance of 3D strain analysis in patients with STEMI. METHODS: Patients who underwent successful revascularization for STEMI were retrospectively recruited. In addition to conventional parameters, 3D EF, global longitudinal strain (GLS), global area strain (GAS), as well as 2D GLS were obtained. We constructed a composite outcome consisting of all-cause death or re-hospitalization for acute heart failure or ventricular arrhythmia. RESULTS: Of 632 STEMI patients, 545 patients (86.2%) had a reliable 3D strain analysis. During median follow-up of 49.5 months, 55 (10.1%) patients experienced the adverse outcome. Left ventricle EF, 2D GLS, 3D EF, 3D GLS, and 3D GAS were significantly associated with poor outcomes. (all, p < 0.001) The maximum likelihood-ratio test was performed to evaluate the additional prognostic value of 2D GLS or 3D GLS over the prognostic model consisting of clinical characteristics and EF, and the likelihood ratio was 15.9 for 2D GLS (p < 0.001) and 1.49 for 3D GLS (p = 0.22). CONCLUSIONS: The predictive power of 3D strain was slightly lower than the 2D strain. Although we can obtain 3D strains, volume, and EF simultaneously in same cycle, the clinical implications of 3D strains in STEMI need to be investigated further.

Background: High demand and low supply of fishmeal due to overexploitation of fisheries resources have resulted in a dramatic increase in the price of this ingredient. Olive flounder (Paralichthys olivaceus) commercial feed contains approximately 60% fishmeal and limited success has been achieved in identifying sustainable alternative protein sources for this species. Methods: An on-farm feeding trial was conducted to compare a basal diet containing 65% as the control (CONT) with two experimental diets replacing 10% of fishmeal by animal protein (AP<sub>10</sub>) or 20% of fishmeal by animal and plant protein (APP<sub>20</sub>). Sub-adult olive flounder averaging 327 ± 9.3 g (mean±SD) were fed one of the three diets in triplicate groups for 16 weeks. Results: Weight gain, specific growth rate, feed efficiency, protein efficiency ratio, and survival were not significantly different among fish fed all the experimental diets (P > 0.05). Also, non-specific immune responses (superoxide dismutase and lysozyme activity), serum biochemical parameters, and intestinal villi length were not significantly different among fish fed all the experimental diets (P > 0.05). Conclusions: Therefore, based on growth performance, non-specific immune responses, serum biochemical parameters, and intestinal histology, dietary animal and plant protein mixtures could replace up to 20% of fishmeal in the diet of sub-adult olive flounder.

Consideration to minimize power losses in Synchronous Rectification

A synchronous rectification in high performance converter design is an essential block for low voltage and high current applications since significant efficiency and power density improvements can be achieved by replacing schottky rectification with synchronous rectification MOSFETs. Many critical parameters for synchronous rectification MOSFETs and even parasitic components in devices and printed circuit board directly affect to system efficiency of synchronous rectifications. Optimization of the MOSFETs plays an important role in improving efficiency of synchronous rectification. This paper shows the impact of power MOSFET parameters and parasitic inductance on efficiency of synchronous rectification through loss analysis of power MOSFET from an experiment and a simulation results.
