        A New Classification for Hemorrhoidal Disease: The Creation of the “BPRST” Staging and Its Application in Clinical Practice

        Purpose: Present an updated classification for symptomatic hemorrhoids, which not only guides the treatment of internal hemorrhoids but also the treatment of external components. In addition, this new classification includes new treatment alternatives created over the last few years. Methods: Throughout the past 7 years, the authors developed a method to classify patients with symptomatic hemorrhoids. This study, besides presenting this classification proposal, also retrospectively analyzed 149 consecutive patients treated between March 2011 and November 2013 and aimed to evaluate the association between the management adopted with Goligher classification and our proposed BPRST classification. Results: Both classifications had a statistically significant association with the adopted management strategies. However, the BPRST classification tended to have fewer management discrepancies when each stage of disease was individually analyzed. Conclusion: Although there is much disagreement about how the classification of hemorrhoidal disease should be updated, it is accepted that some kind of revision is needed. The BPRST method showed a strong association with the management that should be adopted for each stage of the disease. Further studies are needed for its validation, but the current results are encouraging.

        Balance of Antagonisms versus the Myth of “Racial Democracy” in Casa Grande & Senzala: A Hypothesis that a Hasty Reading Inspired the Myth

        In this paper we discuss the hypothesis which centers on the interpretation of Gilberto Freyre’s democratic ideas, proposed by Arthur Ramos and Roger Bastide, which led to the emergence of the myth of “racial democracy.” The basis of the interpretation is the author’s concept of “social democracy,” found in his writings of the mid-1930s; however, this concept is not found in his essay that Casa Grande & Senzal published in 1933, although many claim to find there. Due to its importance, the book was translated into English as The Masters and Slaves, and published in 1946. We try to demonstrate that the origin of the myth of racial democracy cannot be found in the book, not even as an implicit idea; however, it originated from the interpretation by its critics. Therefore, in this paper we show that Gilberto Freyre proposed a balance of opposites, in a negotiated relationship that was not peaceful, but violent; however, in a relationship which Freyre conceptualized as a “balance of antagonisms” rather than as the “racial democracy,” which was interpreted by his critics.

        Constitutive Model for Aggregate Interlock in FEM Analyses of Concrete Interfaces with Embedded Steel Bars

        A nonlinear finite element model (FEM) is developed to assess the behaviour of a cracked concrete interface, reinforced with embedded steel bars and subjected to monotonic loading. A dowel action finite element modelling approach is conceived for that purpose. The bond between the steel bars and the surrounding concrete is also considered in the model and an interface finite element is included to simulate aggregate interlock. Then, the comparison of the model results with experimental values allowed the calibration of aggregate interlock constitutive relations for cracks in monolithic concrete restrained by embedded steel bars. New constitutive relations are also proposed for shear transfer by aggregate interlock in a concrete joint.

        Pathological fracture induced by Halicephalobus gingivalis (Nematoda: Rhabditida) in a horse limb

        Halicephalobus gingivalis is a free-living nematode that occasionally causes infections inhorses. We report a rare case of limb fracture of horse caused by infection with H. gingivalis. An 8-year-old mare was referred to the Veterinary Hospital of the Federal Universityof Lavras with claudication grade 5 of the right hind limb, that had been started 3months ago. The patient had aseptic arthritis in the tarsal joint and edema that extendedto the quartile. The radiographic examination showed punctate osteolysis with exacerbation of bone trabeculation along the calcaneus, talus, proximal epiphysis of the thirdmetatarsal and distal epiphysis of the tibia. Treatment for arthritis was initiated, and theanimal showed a slight improvement in limb function. However, 21 days after hospitalization, due to a comminuted fracture of the tibia, it was euthanized. At necropsy, yellowish masses were found from the metatarsal to the tibia, and around the tarsal bonesand joint. Similar masses were also found in the left kidney. Numerous nematodes compatible with H. gingivalis were identified. This is the first description of a pathologicalfracture caused by H. gingivalis infection in an equine limb.

        Winkler spring behavior in FE analyses of dowel action in statically loaded RC cracks

        A nonlinear finite element modeling approach is developed to assess the behavior of a dowel bar embedded on a single concrete block substrate, subjected to monotonic loading. In this approach, a discrete representation of the steel reinforcing bar is considered, using beam finite elements with nonlinear material behavior. The bar is connected to the concrete embedment through nonlinear Winkler spring elements. This modeling approach can only be used if a new constitutive model is developed for the spring elements, to simulate the deformability and strength of the concrete substrate. To define this constitutive model, an extensive literature review was conducted, as well as 3 experimental tests, in order to select the experimental data which can be used in the calibration of the model. Based on this data, an empirical model was established to predict the global dowel response, for a wide range of bar diameters and concrete strengths. This empirical model provided the information needed for calibration of the nonlinear Winkler spring model, valid for dowel displacements up to 4 mm. This new constitutive model is composed by 5 stages, in order to reproduce the concrete substrate response.

        Anti-Inflammatory Activity of Aqueous and Alkaline Extracts from Mushrooms (Agaricus blazei Murill)

        The effects of aqueous and alkaline extracts from Agaricus blazei Murill, an edible mushroom used as folk medicine in Brazil, Japan, and China to treat several illnesses, were investigated on the basis of the inflammatory process induced by different agents. Oral administration of A. blazei extracts marginally inhibited the edema induced by nystatin. In contrast, when complete Freund's adjuvant was used as the inflammatory stimulus, both extracts were able to inhibit this process significantly (P < .05, analysis of variance followed by Tukey-Kramer multiple comparison post hoc test), although it inhibited the granulomatous tissue induction moderately. These extracts were able to decrease the ulcer wounds induced by stress. Also, administration of extracts inhibited neutrophil migration to the exudates present in the peritoneal cavity after carrageenin injection. Therefore, it is possible that A. blazei extracts can be useful in inflammatory diseases because of activation of the immune system and its cells induced by the presence of polysaccharides such as $\beta$-glucans.

        Effect of passive ultrasonic agitation during final irrigation on cleaning capacity of hybrid instrumentation

        Objectives: To evaluate the effect of passive ultrasonic agitation on the cleaning capacity of a hybrid instrumentation technique. Materials and Methods: Twenty mandibular incisors with mesiodistal-flattened root shape had their crowns sectioned at 1 mm from the cementoenamel junction. Instrumentation was initiated by catheterization with K-type files (Denstply Maillefer) #10, #15, and #20 at 3 mm from the working length. Cervical preparation was performed with Largo bur #1 (Dentsply Maillefer) followed by apical instrumentation with K-type files #15, #20 and #25, and finishing with ProTaper F2 file (Denstply Maillefer). All files were used up to the working length under irrigation with 1 mL of 2.5% sodium hypochlorite (Biodyn$\hat{a}$mica) at each instrument change. At the end of instrumentation, the roots were randomly separated into 2 groups (n = 10). All specimens received final irrigation with 1 mL of 2.5% sodium hypochlorite. The solution remained in the root canals in Group 1 for one minute; and ultrasonic agitation was performed in Group 2 for one minute using a straight tip inserted at 1 mm from working length. The specimens were processed histologically and the sections were analyzed under optic microscope (x64) to quantify debris present in the root canal. Results: The samples submitted to ultrasonic agitation (Group 2) presented significant decrease in the amount of debris in comparison with those of Group 1 (p < 0.05). Conclusions: The hybrid instrumentation technique associated with passive ultrasonic agitation promoted greater debris removal in the apical third of the root canals.

        Effect of Propionic Acid in the Germination of Rice Genotypes

        The objective of this work was to evaluate the germination of 12 rice genotypes under propionic acid stress, a phytotoxic compound produced in low drainage soils with high organic matter content. The tests were conducted with the first count of germination (PCG) and germination (G) of the genotypes subjected to 0, 3, 6, and 9 mM propionic acid concentrations. The seeds of each genotype were placed in germitest paper pre-soaked in treatment solutions forming individual bags. The germination was performed at 25 °C and the counts were carried out at 7 (PCG) and 14 days (G). A factorial random block design was performed with four replications of 50 seeds per genotype. Our study revealed that doses up to 9 mM propionic acid in the pre-soaking solution were efficient for genetic variability studies involving the character germination in rice; genetic variability for germination was detected in the collection of rice genotypes when subjected to propionic acid toxic effects. The genotypes Guichow, Dawn, and Toride-1 showed germination stability when subjected to increasing levels of propionic acid, and genotypes originated from irrigated system cultivation performed better when subjected to propionic acid stress. These three genotypes will be a good biological material to for enhance the resistance to phytotoxic compounds in rice.
