        피부 점액성 에크린 암 1예

        Mucinous eccrine carcinoma of the skin is a rare skin adnexal malignant tumor that origins from mucin-secreting dark cells, and it arises from the deepest portion of eccrine ducts. It usually affects people in their fifth to seventh decades of life and it predominantly occurs in men. It appears to exhibit a predilection for the head and neck. The histopathologic characteristics are a large mucinous pool with fibrous septae and clusters of tumor cells. The treatment of choice is surgical removal. We present here the case of a patient with a mucinoous eccrine carcinoma on the infraorbital area of the right side of his face and we review the related literature.         최근 10년간 주요 피부 악성종양의 통계적 고찰(2000~2010, 경기동북부지역)

        Background: Major cutaneous malignant tumors (MCMT) including squamous cell carcinoma (SCC), basal cell carcinoma (BCC) and malignant melanoma (MM) result from multifactorial causes such as environmental and host factors. In particular, ultraviolet radiation (UVR) comprised of environmental factors is estimated to be one of the most important risk factors for MCMT. Objective: The purpose of this study was to analyze recent changes in the incidence of MCMT in patients from the north-east area of gyeonggido, and compare that incidence with data previously reported. Methods: We retrospectively reviewed the medical records of 212 patients diagnosed as MCMT at the Department of Dermatology at the Uijeongbu St. Mary`s Hospital during the last 10 years. Results: The average annual incidence of MCMT was 0.3% and it seemed to increase. Among the 212 cases of malignant tumors, the most common malignant tumor was BCC (62.7%). The incidence of SCC and MM were 25% and 12.3%, respectively. The mean age of patients was 68.4±14.15 years for those with MCMT; 66.8 y in BCC, 74.5 y in SCC and 64.5 y in MM. The ratio of men to women was 0.72:1. Compared with the incidence of MCMT in other provinces, the north-east portion of Gyeonggido (0.3%) had a high incidence of MCMT. Conclusion: High altitude may increase the amount of UVR exposure and influence the development of cutaneous malignant tumors. Individual behaviors including outdoor activity and use of sun blocks are important to the development of major cutaneous tumors and should not be ignored. (Korean J Dermatol 2011; 49(2):97~105)

        두피에 발생한 과립세포종 1예

        Granular cell tumors are rare disease can be which found in virtually any body site, including the tongue, skin, subcutaneous tissue, breast, rectum and vulva. However, they are rarely seen on the scalp. We report here on a rare case of granular cell tumor on the scalp. A 55-year-old woman presented with a non-tender, firm papule on the scalp. We removed the papule with excisional biopsy. Histopathologic examination showed well-circumscribed, non-capsulated nodule that consisted of large polygonal cells with abundant eosinophilic granular cytoplasms. The tumor cells were positive for S-100 protein staining. We diagnosed this case as granular cell tumor. (Korean J Dermatol 2009;47(10):1193∼1195)

        흰점박이꽃무지(Protaetia brevitarsis) 추출물의 항혈전 효능

        We aimed to evaluate antithrombotic efficacy of Protaetia brevitarsis extract during 21 days. Rats (SPF rat, weight 240~260 g) were divided into 16 groups (5 rats per group), they were: control group and Protaetia brevitarsis extract groups with dose of 0.1, 0.5, 1, 2.5, 5, 10, 25, 50, 75, 100, 200, 250, 500, 750, 1,000 mg/kg kg of body weight. Thromboplastin time (PT) and activated partial thromboplastin time (aPPT) as antithrombotic efficacy were tested in this animal experiment (at 7, 14 and 21 days). Overall, the admistration dose of Protaetia brevitarsis extract over 50 mg/kg at 7, 14 and 21 days for PT and over 25 mg/kg at 7, 14 and 21 days for aPPT tented to be longer than that of other groups. In addition, the optimal admistration doses of Protaetia brevitarsis extract to improves antithrombotic efficacy were 75, 100, 200 and 250 mg/kg at 7, 14 and 21 days for PT (p<0.05) and 50 and 100 mg/kg at 7 days, 75 mg/kg at 14 days, or 50, 100, 200 and 250 mg/kg at 21 days for aPPT (p<0.05). It can be concluded that Protaetia brevitarsis extract at optimal levels have antithrombotic efficacy.

      • 신경세포 시냅스에서 Shank2, Homer 1b와 PLC-β3의 신호전달 복합체 형성

        포스포리파아제 C-베타(phospholipase C-β: PLC-β) 동위효소는 G-proteins-coupled receptors(GPCR)와 그의 신호전달 매개체인 heterotrimeric G proteins에 의해 활성화된다. PLC-β는 효소적 활성에 필요한 영역 외에 카르복시 말단에 PSD-95/Dlg/ZO-1 (PDZ)-binding motif를 가진다. 이 motif는 PLC-β가 PDZ domain을 가진 단백질들과 결합함으로써 GPCR 매개 신호전달에서 특이적인 역할을 수행할 가능성을 제시한다. 이에 본 연구자들은 효모 two-hybrid assay를 통하여 Shank2가 PLC-β3와 결합함을 이전에 확인하였다. 본 연구에서는, 면역침전반응을 통해 두 단백질의 결합은 PLC-β3의 카르복시 말단의 PDZ-binding motif를 통하여 이루어짐을 확인하였고 신경세포의 시냅스에서 mGluR1 및 IP3 수용체와 결합하는 Homer 1b 와 Shank2가 PLC-β3와 함께 위치하는 것을 발견하였다. 이러한 결과는 전자현미경을 이용하여 Homer 1b와 Shank2가 위치하는 PSD 영역에서 PLC-β3가 존재함을 확인하고 mGluR1과 Shank2, Homer 1b, PLC-β3가 해마와 소뇌의 많은 부분에서 동시에 발현되는 것을 확인함으로써 다시 한번 입증되었다. 결과를 종합해 볼 때, 이들 세 단백질은 같은 신경세포의 시냅스에서 발현되어 복합체를 형성함으로써 mGluR1에 의해 매개되는 신호전달을 보다 효율적으로 수행할 수 있을 것으로 예상된다. Phospholipase C-β isotypes activated by G protein-coupled receptors (GPCR) and heterotrimeric G proteins carry a PSD-95/Dlg/ZO-1 (PDZ)-binding motif in their carboxyl terminus. This motif may enable PLC-β isotypes to play specific roles in GPCR signaling through interacting with PDZ-containing proteins. We identified Shank2 as a PLC-β3 binding protein, using yeast two-hybrid system in the previous study, and here we characterized the binding of PLC-β3 with Shank2. Immunoprecipitation study showed that Shank2 interacts with PDZ-binding motif of PLC-β3. Moreover, PLC-β3 was colocalized with Shank2 and Homer 1b that interacts with mGluR1 and IP3 receptor on the synapse of primary-cultured neuronal cells. Immunogold EM also showed that PLC-β3 immunoreactivity was detected at the PSD of the CA1 pyramidal neurons. In several regions of hippocampus and cerebellum, the expression pattern of Shank2 was similar to that of mGluR1, Homer 1b, and PLC-β3. These results suggest that PLC-β3, Shank2, and Homer 1b may act as active intermediates in mGluR1-mediated signal transduction by forming signaling complex in neuronal synapses.
