Fournier’s gangrene is a rare, highly infectious necrotizing fasciitis of the perineum and genital area which is caused by a mixture of various bacterial organism. Treatment is predominantly surgical, including extensive and early drainage and debridement, considering negative-pressure wound therapy for helping to heal the wounds and broad-spectrum intrave-nous antibiotics therapy. Some reports have described the use of negative-pressure wound therapy combined with split thickness skin graft in the defect area. While, other recent reports suggest that local flap coverage rather than skin graft for reconstruction of perineal area. This study aims to suggest the use of medial femoral circumflex artery perforator based thigh flap for scrotal and perineal reconstruction in open areas secondary to the surgical debridement and negative-pressure wound therapy of Fournier’s gangrene.
Calcifying Aponeurotic Fibroma in Dangerous Zone of Hand in Young Child

Pediatric hand masses are often seen in outpatient department of the hospital. There are kinds of masses that can occur and it is important to discriminate between malignant and benign, since the treatment may be different. Calcifying apo-neurotic fibroma (CAF) is a rare benign soft tissue in children and adolescents. It primarily occurs on the distal portion of the extremities, typically fingers, palms of the hands and plantar aspects of the feet. The recommended treatment of CAF is conservative local excision. Considering its benign characteristics and the rare metastasis, a strong and enough efforts should be made, whenever possible, to preserve the function of involved extremities, especially in child. In our report, we present a case of a 4-year-old girl with a CAF found near the course of motor branch of ulnar nerve at the palm and being performed surgical removal successfully with preserving the nerves.

A method is described to obtain the first order force and second order steady force on the fixed two dimensional submerged or semisubmerged cylinders at infinite depth of water due to regular waves. The first order diffraction wave velocity potential which describes the flow diffracted by a body is obtained numerically using source distribution method on the mean wetted surface. And a technique to remove the irregular frequency phenomena of the source distribution method is also applied. The second order steady force is calculates by means of direct integration of the pressures on the body as derived from the first order velocity potential and is also computed by means of reflection wave height derives from momentum conservation theory. The results are compared with those of published works, and show good agreement.
다수의 이질적 IBP/D/1큐잉 모형의 분석을 위한 근사 알고리즘

본 논문에서는 n개의 버스트 입력 트래픽을 처리하는 이산 시간 큐잉 모형을 분석하기 위한 근사 계산 알고리즘을 제안한다. 입력되는 각각의 버스트 트래픽은 IBP(Interrupted Bernoulli Process)로 모형화된다. 이 알고리즘은 n 개의 입력 프로세스를 하나의 상태 변수로 표시하여 n 개의 입력 프로세스로 표현된 마코프 체인(Markov Chain)의 확률 전이 상태를 단순화한다. 이렇게 단순화된 하나의 상태 변수를 이용하여 큐잉모형의 상태 전이를 표현하고 이를 완전 수치 계산에 의해 해를 구한다. 이러한 절차를 통해 구한 큐 길이, 대기 시간 분포를 시뮬레이션에 의해 구한 값과 비교하여 알고리즘의 타당성을 검증한다. We propose an approximate algorithm to analyze the queuing system with n bursty and heterogeneous arrival processes. Each input process is modeled by Interrupted Bernoulli Process(IBP). We approximate N arrival processes by a single state variable and subsequently simplify the transition probability matrix of the Markov chain associated with these N arrival processes. Using this single state variable of arrival processes, we describe the state of the queuing system and analyze the system numerically with the reduced transition probability matrix. We compute the queue length distribution, the delay distribution, and the loss probability. Comparisons with simulation data show that the approximation algorithm has a good accuracy.
