다채널 컴퓨터 단층 촬영 영상을 이용한 저 에너지성 전자부 골절의 분석: 골수강내 금속정 고정술을 위한 응용

목 적: 저 에너지성 전자부 골절에서 다채널 컴퓨터 단층 촬영 영상을 통한 방사선적 분석을 시행하여 골수강내 금속정을 위한 정보를 알아보고자 하였다. 대상 및 방법: 단순 실족으로 단독 골절이 발생하여 다채널 컴퓨터 단층 촬영을 시행한 117예(안정성 골절 39예, 불안정성골절 78예)를 대상으로 하였으며, 평균 나이 78.4세(범위, 60-96세)였다. 컴퓨터 단층 촬영 영상으로 방사선적 분석을 시행하였으며, 골절의 위치 및 형태를 파악하였다. 결 과: 골절의 위치는 환자의 연령, 골밀도 및 기저 질환에 따른 통계적 차이가 없었으며, 외측 부위는 대 전자 첨부에서 평균 원위 10.2 mm (범위, 1.0-22.2 mm)에 위치하였고 내측은 소 전자부 시작점에서 평균 원위 평균 8.1 mm (범위,0.0-18.3 mm)였다. 골절의 전내측 피질골 부위는 전 예에서 분쇄성이 관찰되지 않았으며, 내 외반 및 회전 변형에 따라 접촉소실이 발생하였다. 결 론: 전내측 피질골 부위는 분쇄성이 없어 접촉 회복을 통해 골절 변형의 회복 및 정복이 가능함을 알 수 있었으며, 외측골절 선은 대전자부 첨부에서 평균 원위 10.2 mm에 위치하여 첨부를 통한 골수정 삽입 시 근위 골편이 포함됨을 알 수 있었다. Purpose: The purpose of this radiologic study was to evaluate the geographic patterns of low-energy trochanteric fractures using multiplanar computed tomography (CT) images for application of intramedullary nailing. Materials and Methods: In this study, 117 trochanteric fractures (stable fracture, 39 cases, unstable fractures, 78 cases) sustained from simple slip-down were assessed. The mean age was 78.4 years (range, 60-96 years). Multiplanar CT images were assessed for evaluation of geographic features of trochanteric fracture, and the fracture exit and geographic patterns were analyzed. Results: The medial and lateral exit of the trochanteric fracture showed no statistical difference by age, bone density, and comorbid disease. The exit was located at an average distance of 10.2 mm (range, 1.0-22.2 mm) from the tip of the greater trochanter (GT), and the medial exit, average distance of 8.1 mm (range, 0.0-18.3 mm) from the tip of the lesser trochanter. It was also found that there was no comminution around the anteromedial cortex of the fracture, and its contact loss was from fracture deformity. Conclusion: Because of no comminution, the contact restoration of the anteromedial cortex resulted in correction of fracture deformity and reduction. Trochanteric nailing by GT tip could be fixed through the proximal fragment of the fracture because the lateral exit is placed at an average distance of 10.2 mm from the GT tip.

원위 경골 골절에서의 전방 접근법을 통한 최소 침습적 금속판 고정술

목적 : 관절내 골절을 포함한 원위 경골 골절에서 전방 접근법을 통한 최소 침습적 금속판 고정술을 시행하여 결과 및 합병증에 대한 분석을 통해, 유용성에 대해 보고하고자 한다. 대상 및 방법 : 2007년 3월부터 2008년 12월까지 전방 접근법을 통한 최소 침습적 금속판 고정술로 치료한 관절내 골절을 포함한 경골 원위부 골절 13예를 대상으로 하였으며, 평균 추시 기간은 16.2개월(범위, 12-30개월)이었다. AO/OTA의 분류에 따른 골절 형태는 43A 6예, 43B 4예, 43C 3예였다. 치료 결과의 판정은 Olerud and Molander ankle scoring system을 통해 기능적 평가를 하였으며, 골 유합 및 술 후 발생한 합병증을 조사하였다. 결과 : 1예를 제외한 12예에서 평균 15.7주(범위, 12-24주)에서 골유합을 얻었으며, 합병증으로는 표재성 감염 2예, 비골 단축 1예, 금속판 파열 1예, 전경골 건 유착 2예가 있었다. Olerud & Molander 족관절 기능 점수는 평균 79점(범위, 35-95점)으로 우수 4예, 양호 6예, 보통 3예로 평가되었다. 결론 : 전방 접근법을 통한 최소 침습적 금속판 고정술은 관절면 골절을 포함한 원위 경골 골절에서 내측 연부조직 상태에 영향을 받지 않고 시행될 수 있으며, 하나의 절개부로 관절내 골절 및 골간단부 골절을 동시에 고정할 수 있는 장점이 있으나, 전경골 건 손상과 같은 전방 접근법으로 인한 합병증으로 인해 제한적으로 시행되어야 할 것으로 생각된다. Purpose: To evaluate functional results and complications after minimally invasive plate osteosynthesis through an anterior approach for distal tibial fractures, including pilon fracture. Materials and Methods: Between March 2007 and December 2008, thirteen patients with fractures of the distal tibia were treated with minimally invasive plate osteosynthesis through an anterior approach, and were followed for a mean of 16.2 months (range, 12-30 months) Fractures according to the AO/OTA classification were six 43A, four 43B and three 43C. We analyzed functional results by bone union, postoperative complications, and the Olerud and Molander ankle scoring system. Results: All fractures were united after a mean of 15.7 weeks (range, 12 to 24 weeks) except one case. There were 2 cases of superficial wound infection, one case of fibular shortening and metal failure, and two cases of tibialis anterior tendon adhesion. The average functional score was 79 points (range, 35-95 points) and results were four excellent, six good and three fair. Conclusion: Minimally invasive plate osteosynthesis through an anterior approach may be used for distal tibial fracture with medial soft tissue injury, and has an advantage in that the metaphyseal and distal articular fracture are fixed at the same time through a single incision However, it should be approached with caution because of the risk of complications due to the anterior approach, such as iatrogenic injury of the tibialis anterior tendon.
