        각종 간질환에 있어서 혈청 및 복수내의 7S Collagen 농도의 임상적의의

        N/A The 7S collagen is a large collageneous fragment of basement membrane, which was recent- ly reported to have increased activity in various liver diseases. In order to deterrnine whether 7S collagen concentrations of serum and ascites is helpful to make diagnosis of various liver diseases, we measured 7S collagen concentrations of serum and ascites in various liver diseas- es. Normal values obtained from 20 healthy controls were 3.12k0.70ng/ml for 7S collagen concentration in the serum. Serum concentrations of the 7S collagen exceeding 4.52ng/ml (Mean+2SD) was defined as positive. The mean values of 7S collagen concentration of serum were 5.65k3.20ng/ml in chronic hepatitis, 5.21+4.67ng/rnl in acute hepatitis, 12.54k5.45ng/ ml in liver cirrhosis, and 10.05k4.72ng/ml in hepatoma. The positive rate was extremely high in hepatoma (100%) and liver cirrhosis(97.1%) but low in chronic hepatitis(59.9%) and acute hepatitis(33.3%). The mean values of 7S collagen concentration in ascites were 59.05+ 18.20ng/ml in liver cirrhosis and 55.32k4.23ng/ml in hepatoma. The 7S collagen concentra- tion of ascites shows no parallel correlation to that of serum in patients with ascites.(Korean J Gastroenterol 1994; 26: 655 661)

        당뇨병환자에서의 관상동맥 질환의 위험인자에 대한 임상적 관찰

        Background: The relationship of coronary atherosclerosis and noninsulin dependent diabetes mellitus (NIDDM) has been well established. However, to elucidate the influence of diabetes mellitus itself on coronary atherosclerosis, we studied 395 cases with coronary atherosclerosis defined by coronary angiography in our hospital from September, 1987 to October, 1991, Method: On coronary angiograms, the luminal narrowing more than 75% was defined as coronary disease. And the average score of involved vessels was compared in subjects with and without several coronary risk factors (ie, hyperlipidemia, hypertension, smoking and obesity). Results: 1) Amoing 396 patients, there were 71 NIDDM patients (male; 34, female; 37) and 325 non-diabetics (male; 216, female; 109), mean age 56. 7 and 57. 8 respectively. 2) On coroinary angiogram, the average scores of involved vessel are 2.22 in diabetics and 1.91 in non- diabetics (p<0.05). 3) There were no significant differences in the number of involved v 1s between diabetic patients with hyperlipidemia, hypertension, smoking and obesity and those without them respectively, while in nondiabetic patients, significant diffences were observed (p<0,05). Conclusion: These results suggest that diabetic patients have more extensive coronary atherosclerosis than non-diabetics and diabetes mellitus itself may be the major independent risk factor of the coronary arterial disease.

        몇가지 소화기계통 악성종양에서 CA 72-4치에 대하여

        A new tumor-associated glycoprotein (TAG-72) has been shown to be expressed in a wide variety of epithelial malignant tissues. We studied the serum levels of TAG-72 antigen in some malignant tumors of digestive system (n= 44), compared with benign diseases of digestive organ (n=35) and the normal subjects (n=35). The results were as follows; 1) The mean value of serum CA 72-4 was 1.69±0,50 U/㎖ in the normal subjects. 2) There was no significant difference of serum CA 72-4 levels between patients with benign disease of digestive organ and the normal subjects (p$gt;0.05). 3) Levels of serum CA 72-4 in patients with gastric caner or hepatoma appeared significant increase than those of either the normal subjects or benign diseases (p$lt;0.05). 4) There was significantly increased levels of serum Ca 72-4 in patients with advanced gastric cancer than those with early gastric cancer (p$lt;0.05). In gastric cancer, CA 72-4 identified 45.8% of all patients (CA 19-9, 22. 2%. CEA 40.0%), and combination of CA 72-4 and CEA detected as many as 60% (p$lt;0.05).

        갑상선 기능 항진증에서의 HbAlc와 Fructosamine 치와의 관계

        Thyroid hormone may have various changes of metabolism process but the severity and pathogenesis of metabolism abnormalities in hyperthyroidism are incompletely defined despite vast studies. To evaluate the changes of carbohydrate metabolism in hyperthyroidism, we measured the serum levels of fasting blood sugar (FBS), total protein, albumin, hemoglobin Alc (HbAlc) and fructosamine in 42 normal controls and 60 patients with hyperthyroidism from March 1989 to August 1991. In hyperthyroidism group, those are as follows: FBS 91.9±11.7 ㎎/dl, total protein 7.0±1.1 g/dl, albumin 4.2±0.4 g/dl, HbAlc 5.3±1.0%, and fructosamine 2.1±0.3 ㏖/L. In normal control group, those are as follows: FBS 85.3±8.8 ㎎/dl, total protein 7.8±0.3 g/dl, albumin 4.4±0.2 ㎎/dl, HbAlc 4.9±0.2% and fructosamine 2.6±0.2 ㏖/L. The mean values of FBS and HbAlc in hyperthyroid group are higher than those of normal controls (p$lt;0.01) but those of serum albumin and fructosamine in hyperthyroid group are lower than those of normal contro with correlation each other. The higher level of FBS and HbAlc in hyperthyroid group compared to normal controls appeared as changes of carbohydrate metabolism. And it was revealed that fructosamine was not relable indicator of previous serum glucose concentration in hyperthyroidism. That may be originated from the concomitant decreased level of albumin as the greatest fraction of fructosamine. (J Kor Soc Endocrinol 7:46~51, 1992)
