목 적: 슬개골 횡 골절의 치료에 있어 유관 나사만의 단독 고정술의 적응증과 효과를 알아보고자 하였다. 대상 및 방법: 1991년 1월부터 2005년 5월까지 슬개골 골절로 본원에서 치료한 90예의 환자 중 유관 나사만을 이용하여 고정하고 1년 이상 추시가 가능하였던 14예의 환자를 대상으로 하였으며 평균 추시 기간은 47개월 (12∼143개월)이었다. 수술 후 방사선학적 골 유합 유무 및 전위 정도, 수술 시간, 슬관절 운동 범위 등을 조사하였고, Lysholm 점수로 슬관절 기능을 평가하였다. 결 과: 수술 후 전 예에서 골 유합을 얻었고, 평균 전위 정도는 수술 전 2.2㎜ (0∼6.0㎜)에서 수술 후 0.3㎜ (0∼1.5㎜)로 감소하였다(p=0.001, Wilcoxon signed rank test). 평균 수술 시간은 34분 (20∼60분)으로 비교적 짧았으며, 13예 (92.9%)에서 정상 관절 운동 범위를 보였고 최종 추시상 Lysholm 점수는 평균 95.9점이었다. 결 론: 유관 나사 단독 고정술은 슬개골 횡 골절과 큰 골편이 있는 복합 골절의 치료에 있어 신전 기전의 손상이 없고, 비교적 간단하고, 수술 시간이 비교적 짧고 절개창이 작아 유용한 방법으로 사료된다. Purpose: To assess the indication and effect of screw fixation in the transverse patellar fractures. Materials and Methods: We analysed the results of 14 transverse patellar fractures fixed with screws from January 1991 to May 2005. Mean follow -up period was 47 months (range, 12~143 months). We analysed the radiologic union, operation time, ROM and postoperative Lysholm score. Results: All fractures healed uneventfully. The mean displacement was decreased from 2.2 ㎜ preoperatively to 0.3 ㎜ postoperatively (p=0.001,Wilcoxon signed rank test). The mean operation time was 34 minutes (range, 20 to 60 minutes).Normal range of motion was achieved in 13 knees (92.9% ). Average Lysholm score w as 95.9 at final follow-up. Conclusion: Screw fixation seemed to be useful for treatment of transverse patellar fracture even in comminuted fractures with large fragments. The advantage of this technique was the preservation of extensor mechanism , simplicity, short operation time and good cosm esis.

슬와부 베이커 낭종에 의한 총비골 신경 및 경골 신경의 포착증후군

하퇴부의 위약감을 동반한 하지 통증은 흔히 요추부의 퇴행성 디스크 병변에 의해 유발되는 복합적인 병변이다. 그러나 신경 압박에 의한 하지 통증과 하퇴부 위축이 드물게 보고되고 있다. 우리는 슬와부 베이커 낭종에 의한 총비골 신경 및 경골 신경의 포착증후군이 발생한 드문 증례를 보고하고자 한다. 일차적으로 관절경하 반월상 연골 절제술 및 관절경하 낭종 감압술을 시행하였으나 낭종이 재발하여, 개방적 낭 절제술을 시행하였다. 술 후 1년이 경과한 상태로 낭종은 재발하지 않았으며 하퇴부 근육 둘레는 증가하였고, 근전도 검사에서도 호전되고 있었다. 그러나 환자는 여전히 하퇴부의 위약감과 발바닥의 감각이상을 호소하고 있는 상태로 근전도의 조기 진단 및 조기 감압 치료가 필요한 증예로, 문헌 고찰과 함께 이를 보고하는 바이다. Degenerative disc disease of the lumbar spine caused leg pain with associated calf weakness and atrophy. However, an unusual case of leg pain and calf atrophy due to neural compression is reported. We reported a rare case of a Baker's cyst causing common peroneal nerve and tibial nerve entrapment neuropathy. Initially, after arthroscopic menisectomy and cyst decompression, Baker's cyst was recurred and then open excision was done. Until 1 year after surgery, the cyst was not recurred, increased muscle bulk was noted and EMG showed improved findings. But she continued to complain of lower leg weakness and sole paresthesia owing to delayed diagnosis and cyst decompression.
