        류마티스 관절염 환자의 T세포 수용체 Vβ 유전자 레퍼토리 분석

        Objectives: Polymerase chain reaction (PCR) technology was eamine synovial fluid and peripheral T cells in patients with rheumatoid arthritis (RA) to determine the preferential usage of the T cell receptor(TCR) variable region (V) gene. Methods: Oligonucleotide primers specific for individual TCR Vβ gene families were used to amplify the TCR gene products in a semiquantitative assay of their relative utilization in unselected T cell populations. Results: The result of Vβ utilization was generally heterogenous, similar with previous reports. However, the mean expression of Vβ16 and Vβ18 in RA was more preferentially utilized compared to normal donors. The usage of Vβ in peripheral blood from 3 patients with RA demonstrated restrictions in Vβ16, Vβ20 and Vβ18 genes, respectively. Analyses of synovial fluid resulted in restriction in β12, Vβ20 and Vβ20, respectively. Although there was no significant pattern of skewed Vβ gene mean usage when comparing the synovial fluids with the peripheral blood T cells from RA patients, there were significant biased Vβ genes, Vβ12, Vβl and Vβ20, each 3 patients. As the HLA type is a determining factor in shaping TCR repertoire of peripheral T cells, we compared the Vβ utilization in HLA-DR4 expressing groups that have susceptibility and gene dosage effect in disease progression. It was a little different that comparing the pattern of Vβ usage in peripheral blood and synovial fluid from RA patients between HLA-DR4 positive and negative group. Conclusion: The results were consistent with the conclusion that the increased Vβ family T cells infiltrate synovium and are dependent on each patient and may be involved in inducing and maintaining the synovitis that characterizes RA The different outcome of each patient may be due to the difference in disease duration, genetic background and geographic region. A more important factor may be the stage of disease, because epitope induced immune reaction may change over time. Therefore, selecting patients early in the course of disease may be important and may facilitate the need for more in-depth TCR analysis in the future.

        시간지원 데이터 모텔은 시간 의미를 데이터 모델에 추가하여 시간에 따라 변화된 정보를 처리할 수 있는 데이터 모델이다. 시간지원 데이터 모델은 실세계에서 사건이 발생한 시간인 유효시간을 지원하는 데이터 모델과 데이터가 수록된 시간을 지원하는 거래시간 데이터 모델 그리고 거래시간과 유효시간을 모두 지원하는 이원시간 데이터 모델이 있다. 대부분의 시간지원 데이터 모델은 관계형 모델을 확장하여 시간지원 데이터를 처리할 수 있도록 설계된다. 시간지원 데이터 모델의 두부류는 시간을 결합하는 단위에 따라 튜플 타임 스탬프와 속성 타임 스탬프의 두 가지 형식이 있다. 본 논문에서는 기존의 데이터 모델에서 시간추가를 위한 기본적인 시간 개념과 시간지원 데이터 모델을 위한 고려사항을 설명하고 시간지원 데이터 모텔을 지원시간에 따라 비교하였다. 또한 유효시간이 지원되는 시간 지원 집계에 적합한 데이터 모델을 제안하고 그 성능을 분석 하였다. Temporal data model is able to handle the time varying information, which is to add temporal attributes to conventional data model. The temporal data model is classified into three models depending upon supporting time dimension, that are the valid time model to support valid time, the transaction time model to support transaction model, and the bitemporal data model to support valid time and transaction time. Most temporal data models are designed to process the temporal data by extending the relational model. There are two types or temporal data model, which are the tuple timestamping and the attribute timestamping depending on time dimension. In this research, a concepts of temporal data model, the time dimension, types of thc data model, and a consideration for the data model design are discussed Also, temporal data models in terms of the time dimension are compared. And the aggregate function model of valid time model is proposed, and then logical analysis for its computing consts has been done.

        Hypothyroidism may be accompanied by different rheumatic syndromes such as neuropathy, arthropathy and myopathy. Severe myopathy with high elevations of creatine kinase (CK) is rarely seen in hypothyroidism. Authors experinced a case of myopathy with hypothyroidism, similar to rheumatoid arthritis or polymyositis-like syndrome. We have treated with thyroid hormone replacement (Levothyroxine sodium 100ug/day) for 10 months. The initial serology resulted in: free T4 0.51ng/dl, TSH 110uIU/ml, LDH 465 units, and CK 4385 units. After treatment, we gained free T4 1.60ng/dl, TSH 0.14uIU/ml, LDH 110 units, and CK 80 units and musculoskeletal symptoms are much improved. We report this patient with a review of literatures.

        복수의 성상을 감별 진단함에 있어서 복수의 지질 , 혈청과 복수의 Albumin 농도비 , LDH 동위효소 및 복수내 Fibronectin 치의 의의

        Background: There are many parameters available to differentiate between malignant and non-malignant acsites. We studied the diagnostic value of ascitic lipid, LDH isoenzymes, serum ascites albumin gradient ratio and fibronectin levels to differentiate between malignant & non-malignant ascites. Methods: 51patients with ascites (21cirrhosis, 2nephrotic syndrome 17, various neoplasms and 11both cirrhosis & hepatocellular carcinoma) were studied to evaluate the diagnostic value of above mentioned 4parameters. Results; 1) Above mentioned 4parameters are statistically very significantly different between malignant and non- malignant ascites than previous parameters. 2) Total ascitci LDH levels ard LDH and LDH levels are markedly high in malignant ascites than LDH and LDH levels. 3) The median and mean values of cholesterol, triglycerides, phospholipids and total lipids were significantly high in malignant specificity of ascitic cholesterol in the differentiation between malignant ascites and benign ascites were 82% and 100% respectively as a cut off value of ascites cholesterol is 42mg/dl. 4) Serum ascites albumin gradient ratio in malignant ascites (0.88±0.62) and benign ascites (4.77±1.250) showed signifacantly difference, and diagnostic sensitivity and specificity are 80% and 100% as a cut off value is less than 1.1. 5) Ascitic Fibronectin levels are significantly high in malignant ascites (116±75 mg/1) than benign ascites (32±21mg/1). Its diagnostic sensitivity-and specificity are 61% and 96% as a cut off value is over 85mg/l. Conclusion; Above 4 parameters ard statistically significant to differenciate between malignant and non- malignant ascites, especially serum ascites albumin gradient ratio and ascitic lipids levels are more valuable for the differential diagnosis of ascites.

        류마티스 관절염과 HLA-B27과의 연관성에 관한 연구

        Objective: In rheumatoid arthritis(RA) patients, HLA-B27 has been mainly found with the same frequency as in the normal population. An increased frequency of HLA-B27 in RA has, however, repeatedly been found in northern Sweden and in Filand. The results concerning the association of HLA-B27 to the outcome of the disease have been contradictory in RA. In RA, the presence of HLA-B27 has shown to be a prognostic index for cervical spine subluxation. There has been no report regarding the association between RA and HLA-B27 in the Korean population. Methods: 188 patients with RA were randomly selected from the patients who were diagnosed at Rheumatism Center of Hanyang University Hospital from October 1994 to June 1995 to establish the frequency of HLA-B27 in RA and to investigate the possible influence and prognostic significance on clinical outcome including atlantoaxial subluxations. Results: The results were as follows: 1) HLA-B27 was present in 12.2% of the RA patients studied. The relative risk (RR) and etiologic fraction (EF) of HLA-B27 were 5.99 and 0.1019 respectively. 2) The rheumatoid factor was positive in 79.8% of the total patients, 73.9% in HLA-B27 positive patients, and 80.6% in HLA-B27 negative patients. HLA-B27 was not associated with the presence of rheumatoid factor in this study. 3) HLA-B27 was not associated with the duration of morning stiffness, Ritchie index, extraarticular manifestations, the number of swollen joints, ARA functional class, ESR. C-reactive protein, or hemoglobin. HLA-B27 was not associated with the positivity of ANA and antiperinuclear factor. 4) HLA-B27 was not associated with the positive rate of erosion and Steinbrocker class in peripheral joints. 5) Atlantoaxial subluxation was present in 13% of HLA-B27 positive patients and 17% of HLA-B27 negative patients. The involvement of atlantoaxial joint including narrowing of lateral facet joints was 26.1% in HLA-B27 positive patients and 20.6% in HLA-B27 negative patients. Subaxial subluxation was presented in only 1 case in HLA-B27 negative group of total patients. HLA-B27 was not associated with the cervical changes in RA. Conclusions: An increased freuency of HLA-B27 in RA patients was significantly found in Korea (RR=5.99, EF=0.1019). However, patients with HLA-B27 had a similiar clinical profile to patients without HLA-B27 and HLAB27 positivity did not further contribute to the severity with respect to clinical and laboratory variables or to radiological progression in peripheral joints or cervical spine.

        관절의 연골조직에서 VLA항원의 발현에 관한 연구

        Objective: The adhesion molecule that mediate cell-cell and cell-extracellular matrix adhesion provides very important role in growth and differentiation of cells and tissue. VLA integrin is a prototype of adhesion molecule which participate in cell-cell and cell-extracellular matrix interacton, especially for collagen, laminin and fibronectin. The biologic functions of VLA-integrin are very diverse. Cartilage is the target tissue of various arthritides including rheumatoid arthritis and osteoarthritis and the process of homeostasis in cartilage matrix may be very important in preservation of cartilage. Although VLA integrin may participate in the process of cartilage degeneration and repair mechanism, tissue distribution and exact role of VLA integrin in cartilage was not yet clearly defined. Methods: Immunohistochemical analysis of VLA-integrin in cryostat section of articular cartilage was conducted using monoclonal antibody and avidin-biotin complex method. Analysis was performed in 10 rheumatoid arthritis specimens, 7 osteoarthritis specimens and 1 normal control. Results: 1) Normal cartilage showed strong and diffuse stain with CD29, CD51 and moderate stain of VLA-5. Staining pattern of VLA-1 and 3 was inconstant and weak in intensity. 2) The intensity of VLA expression in articular cartilage of osteoarthritis was upregulated chiefly in CD29, CD51 and slightly in VLA-5. The positive rate of VLA-l and 3 was similar to that of normal cartilage though the intensity was increased especially at cluster of chondrocytes. 3) VLA-integrin expression of rheumatoid arthritis cartilage was similar to that of osteoarthritis cartilage. Conclusion: VLA integrins functioning as fibronectin receptor such as VLA-5 and α,β1 were upregulated in osteoarthritis and rheumatoid arthritis. Intensity was increased at clusters of chondrocytes. It was able to presume from above findings that VLA molecule has some role in the maintenance and repair of articular cartilage. The regulation of expression by cytokines and growth factors and exact function of VLA molecule in cartilage have to be elucidated.

        조기 천장골염의 자기공명영상 소견

        Objective: The early detection of sacroiliitis on X-ray is important to diagnose spondyloarthropathy(SpA). The conventional radiography, bone scan, and computer tomogram have the problems of sensitivity, specificity, and side effects. We examined sacroiliitis using MR imaging in patients who had inflammatory back pain, but no definite changes in conventional radiogrphy. Other radiologic techniques were compared in detection of early sacroillitis. Methods: Twenty six patients with SpA were examined. The MRI findings of both sacroiliac joints were evaluated in every patients, such as pannus, periarticular osteitis and fat accumulation, subchondral sclerosis, and joint space changes(irregularity, ankylosis). Also we analyzed the changes of sacroiliac joints in sacroiliitis. Results: The predominant sites of involvement were upper aspect and iliac surface of sacroiliac joint in subchondral sclerosis, lower and iliac in pannus, periarticular fat accumulation, and iliac in periarticular osteitis. The MPGR images were superior to spin echo images in evaluation of joint space changes(irregularity, and ankylosis). Conclusions: Sacroiliitis is initiated in iliac side and lower portion of sacroiliac joints. MRI can detect early changes of sacroiliitis in patients with no conventional radiographic changes. MRI seems to be needed in patients with inflammatory back pain.
