Ulcerative colitis is a diffuse inflammatory disease of the mucosal lining of the colon, but relatively rare in this country. Clinical studies of this disease are lacking in this country despite of the limited some observations of the surgical approach of it. we observed for purpose of clinical observation of twenty-eight patients with ulcerative colitis seen in this hospital from November 1968 to August 1986. 1) The most prevalent age group was ranged 30 to 49 and the mean age approximated as 36.4. Occurrence of male to female was 1.3:1. 2) The most common initial symptom in these patients was diarrhea. The additional cardinal symptoms were included as mucobloody diarrhea, abdominal pain and tenesmus as well as fever. 3) According to the anatomical site of the lesion in this disease, left colitis and proctitis were most common (79%) but pancolitis was few (29%). In a view point of the severity in this disease, the relapsing remission form was most common (54%) but the acute fulminating form included only two cases (7%). 4) Nineteen patients given the medical treatment in this disease were effective except three cases. The medical treatment was included with SASP with or without adrenal corticosteroid by the mouth or enema. The untoward action of the medical agents in these patients were minimal and transient. 5) Twelve patients had the surgical operation in our series. They were included as six cases with intestinal obstruction, two intestinal perforation, a case acute fulminating ulcerative colitis, and three cases refractory to the medica1 treatment, respectively.

N/A Barrett's esophagus is a condition in which the normal stratified squamous mucosa of the esophagus is replaced by columnar mucosa, and it may be suspected on abnormal findings of endoscopy and the diagnosis is usually proved by biopsy. We have observed 10 cases suspecting with Barrett's esophagus on the endoscopic findings. Only 5 cases of them, however, could be proved as columnar epithelium-linked esophagus with histological examination. We observed for clinical features regarding age, sex, symptorns, habits, locations, combined diseases, and combined laboratory abnormalities for them. No significant differences of clinical features except serum lipid levels were seen between both groups of Barretts esophagus and chronic esophagitis. 1) All 5 patients with columnar epithelium-linked esophagus was male, and age distribution was from 35 to 60 with mean age of 50. 2) Number of the patients who complained reflux symptoms was 2 out of 5 patients. 3) There were 2 heavy alcohol consumers, 1 coffee drinkers, and 2 heavy cigarette smokers. 4) Location of the lesion was seen on upper esophagus in 3 cases, and on lower esophagus in 2 cases. 5) As a combined diseases, 2 cases with duodenal ulcer, 2 cases with fatty liver, one case with hepatitis, and one patient with rectal polyp. 6) In all 5 patients, serum cholesterol, serum triglyceride level, or both were elevated.

Mortality rate for large bowel cancer in this country have slightly increased in recent years but are still much lower than those in western countries, while Japanese and Caucasians living in Hawaii are the same risk of developing this tumor. Diverticulum is also extremely low incidence in the Koreans, which correspond to the view that diverticulosis seldom occurred in population with low risk to colorectal carcinoma. We have seen recently a very interesting case of multiple diverticulosis of entire colon associated with colonic carcinoma and gallbladder stone. This 72-year-old man had had frequent occasion of upper abdominal colicky pain and melena since about couple years ago, when his condition was diagnosed as multiple colonic diverticulosis and gallstone disease at this hospital. The medical conservative treatment was given for a while with transient improvement. He was admitted to this hospital because of intractable abdominal pain and vomiting on 27th september 1983. He appeared acutely and chronically ill and moderately anemic. There was some diffuse tenderness overall abdomen but no ascites or masses. Rectal examination was no abnormalities. Several test for occult blood in stool disclosed negative, His condition was not responded for the medical conservative treatment. He received the surgical operation on the 11th hospital day. Post operative abnormalities was additionally found a large tumor along the right colonic surface which was proved as well differentiated adenocarcinoma, associating with gallbladder stone and numerous colonic diverticulosis just like preoperative findings.
