우리나라의 인구고령화 속도는 세계에서 가장 빠른 속도록 진행되고 있으 며 이런 관점에서 2008년 7월 노인장기요양보험의 도입과 운영은 그 중요성 을 더해 가고 있다. 노인장기요양보험제도가 도입된 지 햇수로 벌써 7년차에 접어들고 있으며 지금까지는 장기요양 등급체계를 1-3등급으로 유지했는데, 2014년 7월부터 도입되는 치매특별등급을 포함하여 장기요양 등급체계를 1-5등급 체계로 개편함으로써, 등급별 적정한 수급자 비중, 수급자의 요양필 요도에 부합하는 서비스 제공 등 제도의 안정적 운영을 도모하고자 하기 위 함이다. 이는 비교적 양호한 신체기능으로 인해 노인장기요양 서비스 혜택을 받기 어려운 치매환자 중 일상생활 수행에 어려움을 겪는 노인들을 위해 치 매특별등급을 신설함과 동시에, 장기요양 인정점수 인하 등으로 동일 등급내 수급자간 기능상태 차이가 커진 등급을 분할․조정함으로써 안정적 제도 운 영을 골자로 하고 있다. 노인인구수 증가 및 노인성질환자 증가에 따른 장기요양 인정자 수는 단연 증가할 것이며 소요 재정 역시 증가 추이를 보일 것이다. 지금까지는 제도의 안정적 정착을 위해 시설, 인력 등 공급 측면에 초점을 맞춘 인프라 확충에 주력한 시기였다면 지금부터는 점증하는 요양 수요를 안정적으로 흡수하고 장기요양보험의 내실화와 재정적 지속가능성 대책을 마련해야 할 것이다. 노 인장기요양보험제도의 정책 형성에서 증가하는 재정의 낭비 요인을 제거하 고, 전 국민의 부담을 완화하기 위해서는 정확한 재정 예측이 필요하며 노인 장기요양보험 수요와 공급 구조를 분석하고 재정 소요를 예측하여 합리적이고 지속가능한 재정 운영방안 을 도출할 필요성이 대두되고 있다. Seven years has already passed since the long-term care insurance system for the elderly was introduced. So far, it has maintained a 1-3 grade rating system, but it will be reorganized as a 1-5 grade rating system(establishing a special grade of dementia) including a special grade of dementia which will be introduced in July 2014.

Purpose: To verify the usability of tympanic temperature measurement for adults, a comparison of tympanic and axillary temperatures was done. Method: The study was conducted during October 2008, and participants were 110 female nursing students. Axillary temperatures were taken with glass mercury thermometers for 5, 7 and 10 minutes. Tympanic temperatures were taken with Infrared Thermometer IRT 4520 on both ears, twice at a 5-second interval. The data were analyzed using the SPSS 12.0 program. Results: In the 1st measurement, the mean for right tympanic temperatures (0.06℃) and for left (0.03 ℃) were significantly higher than the 2nd. A comparison of mean temperatures for right and left, showed that the mean for the left side on the 1st measurement was significantly higher (0.01 ℃) than the right. Also the temperature on left side in the 2nd measurement was higher (0.04 ℃) than the right 2nd, but not significantly higher. The mean temperature for right and left tympanic on 1st and 2nd measurements were significantly higher than axilla for 5 minutes (0.58 ℃), for 7 minutes (0.52 ℃), and for 10 minutes (0.43 ℃). The tympanic temperature was the most closely correlated with the axillary temperature at 10 minutes. Conclusion: Findings indicate that measurement of tympanic temperature is a useful alternative to axillary temperature taken for 10 minutes.
베이지안 네트워크 모형을 이용한 낙동강 유역의 가뭄과 수질관리의 인과관계에 대한 확률론적 평가

This study investigated the change of the achievement rate of the target water quality conditioned on the occurrence of severe drought, to assess the effects of meteorological drought on the water quality management in the Nakdong River basin. Using three drought indices with difference time scales such as 30-, 60-, 90-day, i.e., SPI30, SPI60, SPI90, and three water quality indicators such as biochemical oxygen demand (BOD), total organic carbon (TOC), and total phosphorus (T-P), we first analyzed the relationship between severe drought occurrence water quality change in mid-sized watersheds, and identified the watersheds in which water quality was highly affected by severe drought. The Bayesian network models were constructed for the watersheds to probabilistically assess the relationship between severe drought and water quality management. Among 22 mid-sized watersheds in the Nakdong River basin, four watersheds, such as #2005, #2018, #2021, and #2022, had high environmental vulnerability to severe drought. In addition, severe drought affected spring and fall water quality in the watershed #2021, summer water quality in the #2005, and winter water quality in the #2022. The causal relationship between drought and water quality management is usufaul in proactive drought management.
구취 환자의 음허지제 치료에 대한 연속 증례보고 : OralChroma<sup>TM</sup>를 이용하여

Objective: This study investigated the effect of Yin deficiency medicine on halitosis patients and the relationship between subjective recognition and objective symptoms of oral malodor. Methods: We administered Yin deficiency medicine, such as Jaeumganghwa-tang, and conducted the Halitosis Associated Life-quality Test (HALT) and $OralChroma^{TM}$. Subjective recognition of halitosis was evaluated using HALT. Objective symptoms of halitosis were evaluated using $OralChroma^{TM}$. In addition, before and after administering medicine, we determined the halitosis according to percentages at the patient's word. Results: The total gas and HALT were not related to each other after Maekmoondong-tang or Saengmaek-san were administered to 10 halitosis patients. Conclusion: Yin deficiency medicine is effective in treating halitosis. Moreover, the subjective recognition and objective symptoms of halitosis are not related to each other.
