We report the case of a 32-year-old female who developed polymyositis associated with scrub typhus. She exhibited the eschar with high titer of anti-Rickettsia(R)-tsutsugamushi antibody, severe muscle weakness, markedly elevated serum levels of muscle enzymes, EMG changes, and infiltration of macrophages in a muscle biopsy specimen. Initiation of appropriate antibiotic therapy resulted in complete healing of scrub typhus and disappearance of symptoms and signs of polymyositis within 1 month after high dose steroid therapy. We suggest that scrub typhus also be included within the causes of idiopathic inflammatory polymyositis. (Korean J Med 61:298-301, 2001)

In this study, molecular dynamics simulation of nano imprint lithography in which patterned stamp is pressed onto amorphous polyethylene(PE) surface are performed to study the behaviour of polymer. Force fields including bond, angle, torsion, and Lennard Jones potential are used to describe the inter-molecular and intra-molecular force of PE molecules and stamp, substrate. Periodic boundary condition is used in horizontal direction and canonical NVT ensemble is used to control the system temperature. As the simulation results, the behaviour of polymer is investigated during the imprinting process. The mechanism of polymer deformation is studied by means of inspecting the surface shape, volume, density, atom distribution, crowd number, position mark, and so on. We found the difference of characteristics between cavity region and stamp region through the many kinds of parameters.

As the simulation results, the behaviour of polymer is investigated during the imprinting process. The mechanism of polymer deformation is studied by means of inspecting the surface shape, volume, density, atom distribution. Especially, density in the stamp region is found to be larger than the cavity region due to compression of polymer molecules. The result indicates that small scale patterning of polymer resist largely depends on compression than flow of molecules.

Deformation of the polymer resist was found for various of the stamp geometry and the alignment state of the polymer molecules.

해역이용협의에 따른 협의서 검토현황과 연안이용형태를 분석한 결과, 공유수면 점 사용은 공작물설치와 해수 취배수, 공유수면 매립은 산업단지조성, 항만 어항개발과 도로건설의 점유율이 높았고, 점 사용은 서해에서 그리고 매립은 남해에서 우세하였다. 지역별로는 전남, 경남지역과 인천 경기지역에서 이용행위가 많았다. 해역이용협의 검토량은 2008년이 전년도에 비해 약 200건 이상 증가하였고, 협의과정에서 준설토 해양투기, 매립, 준설, 해수 취배수, 바다골재채취와 규사채취에 대해 보완요청이 주로 이루어졌다. 연안이용은 대부분 해양환경기준 I등급 또는 II등급의 지역에서 집중되었고, 특히, 법령상 해양 규제지역인 특별관리해역에서는 항만 어항개발과 관련된 매립과 공작물설치, 국립공원에서는 공작물설치와 호안정비가 많았다. 또한, 수산자원보호구역에서는 호안정비, 공작물설치와 해수의 취배수 행위가 많이 이루어진 것으로 평가되었다. 해역이용협의제도의 제반적인 합리성과 신뢰성을 높이는 방법 등 개선방안을 제도운영 및 정책적인 측면과 협의서 작성과 관련한 해양환경영향평가 측면으로 구분해서 정책제언을 제시하였다. The review of statements and coastal development characteristics in South Korea were diagnosed by the consultation on the coastal area utilization in Korea. The occupation and the use of public water were dominated by installation of coastal structure and seawater supply and drainage whereas the reclamation areas were predominantly utilized for developing industrial complex, harbor, fishing port and road. The occupation and the use were dominant in western coast, but reclamation was dominant in southern coast of Korea. The number of utilization cases was particularly greater in Jeonnam, Gyeongnam, and Gyeonggi Province including Incheon. The number of the statements reviews increased by more than 200 cases in 2008 compared with 2007, and most of them were the simple statements. The statements related to ocean disposal of dredged sediment, reclamation, dredging sediment, seawater supply and drainage, and marine sand mining were submitted for review. Coastal utilization was especially active in the regions of seawater quality criteria I and II. In particular, special management sea areas designated under the relevant marine regulations were utilized mostly by reclamation for developing harbor, fishing port and coastal structure. Development activities in national parks comprised primarily structure installation and coastal maintenance. In the fisheries resources protection areas, 40% of the total cases accounted for coastal maintenance, 31% for structure installation, and 16% for seawater supply and drainage. In addition, alternative plans for the improvement in policies and system of marine environmental impact assessment were suggested to enhance the function and confidence of the consultation on the coastal area utilization in Korea.
말기 신부전 환자에서 적극적 투석 요법이 염증 반응을 통해 좌심실 수축기능에 미치는 영향

목적: 울혈성 심부전은 말기신질환자에 있어 생명을 위협하는 급성 합병증이다. 적극적 투석 요법은 효과적으로 체액량을 감소시키고 요독물질을 제거하여 심장의 수축기 기능을 향상시킨다. 비록 심혈관계 질환의 병인에 염증반응이 중요한 역할을 하나, 적극적 투석 요법이 염증인자들과 좌심실의 수축기 기능장애에 어떠한 영향을 미치는지에 대해서는 명확하지 않다. 이번 연구는 적극적 투석요법이 좌심실의 수축기 기능과 혈청 싸이토카인에 어떠한 변화를 일으키는지 조사하였다. 방법: 투석을 받는 말기신질환자 중에서 좌심실 기능부전으로 인한 급성 폐부종이 발생한 환자를 대상으로 하였다. 적극적 투석요법은 매일 4시간씩 7일간 연속적으로 시행하였다. 모든 결과는 전향적으로 모았으며 적극적 투석요법 전후로 심장초음파 검사를 시행하였다. 혈청 TNF-α 와 IL-10을 적극적 투석요법 전후로 비교하였고 내독소 자극 후 이들 싸이토카인들 역시 비교하였다. 결과: 적극적 투석요법 이후에 환자들의 체중과 평균동맥혈압은 의미 있게 감소하였고 그리고 심박출계수는 적극적 투석 이후 의미 있게 증가하였다. 혈청 IL-10과 TNF-α는 적극적 투석요법 후에 의미 있게 감소하였다. 반면 내독소로 자극 후 분비되는 IL-10과 TNF-α는 의미있게 증가하였다. 또한 CRP 변화 정도와 심박출계수 변화 정도는 의미 있는 상관관계를 보여주었다. 결론: 급성 좌심실 기능부전을 동반한 말기신질환자에서 적극적 투석요법은 심박출계수를 의미 있게 증가시켰으며 적절한 면역반응을 회복시키며 염증반응을 조절하는 것으로 나타났다. 따라서 급성 좌심실 기능부전을 동반한 말기신질환자에 있어 적극적 투석요법을 시행하는 것이 심기능 개선 및 염증반응 조절 및 면역반응의 회복증진에 도움이 될 것으로 생각된다. Purpose: CHF is a life threatening acute complication in ESRD populations. An intensive hemodialysis (HD) has been effective in reducing intravascular volume and in removing uremic toxin with improved systolic function. Although recent progress has identified an inflammation as an important contributor to the pathogenesis of CVD, the effect of intensive HD on inflammatory parameters and left ventricle (LV) systolic dysfunction is not clear. The purpose of this study is to examine the effect of intensive HD on LV systolic function and serum cytokines levels. Methods: Among ESRD patients who underwent dialysis and developed acute pulmonary edema due to LV systolic dysfunction were enrolled. Intensive HD consisted of daily 4 hours HD for 7 consecutive days. Data were prospectively collected and 2-D echocardiography was done before and after intensive HD. Serum levels of TNF-α and IL-10 were compared and lipopolysaccharide (LSP)-stimulated of these cytokines were measured. Results: After intensive HD, weight and mean arterial blood pressure decreased significantly and ejection fraction (EF) increased significantly. Serum IL-10 and TNF-α levels decreased significantly after intensive HD. In contrast, LPS stimulated production of these cytokines increased significantly after intensive HD. The difference of CRP between after HD and before HD was negatively correlated with the difference of EF. Conclusion: In ESRD patients with acute LV dysfunction, intensive HD significantly improved EF and restored the immune responsiveness. These results suggest that intensive HD has the advantage of improving EF through modulating inflammation and correcting immune dysfunction in ESRD patients with acute LV dysfunction.
