N/A Background : Although several cases of primary cutaneous CD3O-positive large cell lymphoma have been reported, immunohistochemical study on the tumor has not been reported in Korea. Objective : Our purpose is to evaluate the clinicopathologic and immunohistochemical findings of primary cutaneous CD3O-positive large cell lymphoma. Methods : An immunohistochemical study was performed on paraffin sections of 8 primary cutaneous CD3O-positive large cell lymphoma using CD3, CD45RO, CD45, CD2O, CD3O, and epithelial membrane antigen(EMA). Results : 1. Primary cutaneous CD3O-positive large cell lymphoma occurred equally on males and females. The average age of the patients was 49. The lesion occurred most commonly as nodules on the extremities. Spontaneous regression occurred in 3 cases. The most common treatment was radiotherapy. 2. Histopathologically, 2 of 8 cases showed epidermal hyperplasia with intraepidermal neutrophils. Epidermotropism was found focally in 1 case. Inflammatory cells were present at the periphery of the lesions in 2 cases, but they were admixed with the large atypical cells in 1 case. Tumor cells invaded the subcutaneous tissue in 4 of 6 cases. 3. Immunohistochemically, the large anaplastic cells showed CD3O positivity. Tumor cells in all cases showed positivity for CD3, CD45RO, and CD45, but negative for CD2O. Tumor cells stained with EMA in 1 of 6 cases. Conclusion : Primary cutaneous CD3O-positive large cell lymphoma showed immunophenotype of T-cell lymphoma. It showed some clinicopathologic features of lymphomatoid papulosis, which suggests that both entities represent 2 ends(malignant and benign) of a spectrum of CD3O-positive lymphoproliferative disorders. Therefore, for the diagnosis of primary cutaneous CD3O-positive large cell lymphoma, all clinicopathologic and immunohistochemical findings should be considered together. N/A Background: Kaposi`s sarcoma (KS) is a mesenchymal tumor involving blood and lymphatic vessels. Viral oncogenesis by human herpesvirus 8 (HHV8) and cytokine-induced growth together with some state of immunocompromise represent important conditions for this tumor to develop. Objective: The purpose of this study was to document the clinical and histopathological features of KS in Korea. Methods: The medical records and histopathologic slides of patients with KS diagnosed at Yonsei University Medical Center from January, 1992 to March, 2003 were reviewed. We used immuno-histochemical stains for HHV8 to determine whether HHV8 is present in KS. Results: 1. Among the 21 patients, classic KS was found in 7, acquired immunodeficiency syndrome (AIDS)-associated KS in 3, and introgenic, immunosuppressive KS in 11. 2. Classic and introgenic KS most often have a preference for the lower extremities, while the upper body in AIDS-KS. Mucosal involvement and systemic organ involvement could be detected in AIDS-KS. 3. Immunohistochemical stains for HHV8 were positive in 100% with classic KS and AIDS KS, and 90.9% with immunosuppressive KS. 4. Classic KS responded well to local therapy and showed indolent course. Iatrogenic, immunosuppressive KS generally regressed after reduction or cessation of immunosuppressive drug therapy, but some of them showed resistance to therapy. For AIDS-KS, no systemic treatments have been shown to prolong survival. Conclusion: Because classic KS and iatrogenic, immunsoupressive KS generally have a benign coures, cautions are taken not to overtreat them. However, some cases of organ transplantation associated KS have an aggressive coures, prompting us to consider active treatments to save transplanted organ. (Korean J Dermatol 2003;41(12):1603~1611)
선근종에서 aromatase cytochrome P450의 표현 양상

Objective : To determine whether local estrogen production takes place in adenomyosis and in normal endometrium. Methods : The study included 23 cases of adenomyosis and 17 cases of normal uterine endometrium obtained through hysterectomy or curettage at Kangnam Cha Hospital. The frozen tissue specimens were examined by immunohistochemistry using P450 arom. Results : P450 arom was immunolocalized exclusively in the cytoplasm of glandular cells of adenomyotic tissue. However, no apparent staining was observed in stromal cells. Aromatase was expressed in the ectopic glands (82.6%), but also in the eutopic endometrium of patients with adenomyosis (23.5%). In the case of normal endometrium, P450arom was not detected. Conclusion : These results suggest that the aromatase activity is correlated to the pathophysiology of adenomyosis.
