악성종양과 연관이 적은 베체트 병에서 발생한 위선종을 내시경적 점막절제술로 시행하였고 출혈 및 천공 등의 합병증과 상처 치유과정의 지연없이 성공적으로 치료되었기에 문헌 고찰과 함께 보고하는 바이다. A Case of endoscopic mucosal resection of gastric adenoma in Behcet`s disease Behcet`s disease (BD) is a multisystem disorder presenting with recurrent oral and genital ulceration and ocular involvement, Clinical manifestation include involvement of the mucocutaneous, ocular, gastrointestinal, respiratory, neurologic, urogenital, articular and cardiovascular systems. The risk of cancer in the presence of connective tissue disease and other vasculitides has shown to be increased. However, the association of maliganant disease with BD is rare, As treatment method of malignancy in BD, surgical method may be risk because of impaired immunity of BD using immunosuppressive drugs. We report a case of gastric adenoma in BD, and treated through endoscopic mucosal resection (EMR). A 67-year-old man with gastric adenoma in BD who has been detected with endoscopy was admitted for EMR, Gastric adenoma was treated through EMR, and no evidence of recurrent gastric adenoma was detected follow up endoscopy.

Extracellular vesicles (EVs) are membrane-bound vesicles produced and released into the extracellular fluid by cells under physiological and stressful conditions. They play a role as intracellular communicators by carrying and delivering biomolecules, such as proteins, lipids, or nucleic acids. Urinary EVs have gained important recognition as potential diagnostic biomarkers in renal disease, as they can originate from diverse cell types, including glomerular podocytes, tubular epithelial cells, or endothelial cells. Accumulating evidence has emphasized the feasibility of using EVs as biomarkers for diagnostic, prognostic, and therapeutic purposes in several forms of renal disease, such as acute kidney injury, glomerulonephritis, and renal transplantation. In this review, we introduce recent studies that attempt to identify urinary EVs as candidate biomarkers for human kidney diseases and consider their potential implications as a therapeutic option in significant kidney diseases. 저자들은 대량의 무통성 복수를 주 증상으로 내원한 젊은 여자환자에서 다른 기저 질환없이 대량의 복수로 발현된 전신성 홍반성 루푸스를 진단할 수 있었고, Prednisolone 투여로 복수가 조절되고 증세 호전 보인 1예를 경험하였기에 문헌 고찰과 함께 보고하는 바이다. Systemic lupus erythematosus (SLE) can affect virtually any organ system. So, clinical manifestation and initial presenting manifestation is variable. But ascites is uncommon in SLE initially. Ascites occurs clinically in 8-11% of patients with SLE, usually associated with nephritic syndrome, protein-losing enteropathy, congestive heart failure, liver cirrhosis, Budd-Chiari syndrome, tuberculosis peritonitis. Lupus peritonitis alone does not cause significant ascites. We describe a patient in whom massive, painless ascites was the initial presenting and major manifestation in SLE.

Sarcoidosis is chronic multisystem disease of unknown etiology characterized by noncaseating granulomatous lesions affecting multiple organs. Any organs or tissues can be affected but sarcoidosis more frequently involves the lungs, lymph nodes, spleen, liver, skin, eyes, muscles, bones, central nervous system, and salivary glands (in descending order of frequency). It is very rare for sarcoidosis to involve the breast parenchyma but it may be confused with benign or malignant tumors. We report a case of breast sarcoidosis in a young woman who presented with breast mass.

Pantoea agglomerans (P. agglomerans) is a ubiquitous gram-negative aerobic bacillus member of the Enterobacteriaceae family that is found in plants, fruits and vegetables, and that can also be found in the human and animal fecal matter. It has been reported to play a limited role as an infectious agent, as well as an unusual pathogen for peritoneal dialysis (PD)-associated peritonitis. Beta-lactam antibiotics have been used against this pathogen with good results. Most of the reported cases of P.agglomerans peritonitis were also resolved with antibiotic therapy. Two cases of PD peritonitis caused by P.agglomerans were reported in Korea; clinical improvement was achieved by antibiotic treatment. However, no patient required catheter removal. Here we report a case of PD-associated peritonitis due to P.agglomerans that was successfully treated after catheter removal.

Venous air embolism (VAE) was reported in up to 23% of patients after contrastenhanced computed tomography (CT) examination; most of the emboli are small and not associated with the symptoms. In cases of small and moderate-sized emboli, most patients recovered with supportive care, including intravascular fluid infusion to increase venous pressure and venous return, oxygen therapy, and patient positioning. However, with larger emboli, VAE can be a serious and even fatal event. There have been few cases of VAE in Korea, in which patients improved with supportive treatment. We report a successfully treated case of VAE in a patient with end-stage renal disease (ESRD) on regular hemodialysis, in whom large air emboli developed after contrast-enhanced CT scan, and who experienced improvement with supportive care including cardiopulmonary resuscitation.
