        N/A The Skin Concentration and Minimal Phototoxic Dose Following Administration of Phototoxic Drugs as a Function of Time Yoon-Kee Park, M.D., Moo Yon Cho, M.D., Saung Kyung Hann, M.D., Sungbin im, M.D. Department of Dermatology Yonsei Univeraity College of Medicine, Seoul, Korea Psoralen photochemotherapy(PUVA) is defined as a treatment that depends on the interaction of a photosensitizing substance, psoralen, and long wave jltraviolet(UVA) radiation that results in therapeutically beneficial effects. The rationale of its use is to clear skin disease by repeated controlled phototoxic reactions. The efficacy of PUVA depends on the drug, the UVA dose and wave length, the individual sensitivity to the phototoxic reaction and on the disease to be treated. However, the major factor is the amount of psoralen at the site of photochemical reaction in skin. Daspite the extensive experiences which have now accumulated in the use of PUVA, the relationship between the therapeutic response and phototoxicity as it occurs in normal skin is not established. Comparative data on different psoralen molecules and their skin sensitizing ability are required. From studies on the skin concentration using HPLC and minimal phototoxic dose following oral administration, intraperitoneal administration, a bath of phototoxic drugs as a function of time in guinea pigs, the following results are obtained. 1. The skin concentration of phototoxic drugs after oral administration peaked at 1.5 hours, and the concentration of 8-methoxypsoralen(8-MOP) was 3.7 times greater than that of 5-methoxypsoralen(5-MOP). The skin concetration of 4,5,8-trimethylpsoralen(TMP) was not detected in this study(limit of sensitivity 5ng/g). 2. The skin conentration of phototoxic drugs after intraperitoneal administration peaked at 30 minutes, and the concentration of 8-MOP was slightly greater than that of 5-MOP. The skin concentration of TMP was not detected in this study(limit of sensitivity 5ng/g). 3. The skin concentration of phototoxic drugs after bathing peaked immediately after bathing, and the skin concentrations of phototoxic drugs after bathing decreased in the order of 5-MOP, TMP, and 8-MOP. 4. The minimal phototoxic dose of phototoxic drugs after oral administration was lowest at 1.5 hours, and the minimal phototoxic dose of 8-MOP was 8 times less than 5-MOP, 24 times less than TMP. The phototoxicity of 8-Mop after oral administration was strongest. The time of lowest phototoxic dose and highest skin concentration was the same. 5. The minimal phototoxic dose of phototoxic drugs after intraperitoneal administration was lowest at 30 minutes, and the minimal phototoxic dose of 8-MOP was 1.3 times less than 5-MOP, 5.7 times less than TMP. The phototoxicity of 8-MOP after intraperitoneal administration was strongest. The time of lowest phototoxic dose and highest skin concentration was same. 6. The minimal phototoxic dose of phototoxic drugs after bathing was lowest at 15 minutes. The minimal phototoxic dose of 8-MOP was not significantly different but was significantly greater than that of 5-MOP.

        N/A The Effect on the Recovery of Epidermal Langerhans Cells in C3H Mice after Single and Fractionated Exposure of Ultraviolet B Irradiation Sang-Wahn Koo, M.D., Seung Kyung Hann, M.D., Yoon-Kee Park, M.D., Moo Yon Cho, M.D. Department of Dermatology, Yonsei University College of Medicine Seoul, Korea The study was undertaken to see the recovery in epidermal Langerhans cells in relation to time after UVB irradiation through different amounts and ways of exposure in C3H mice. We irradiated the ears of C3H mice with UVB 200mJ/㎤ and 400mJ/㎤ in a single dose at one time and 5 fractionated doses for 5 days and 10 fractionated doses for 10 days and performed biopsies on the ears of the control and experimental groups after, 2, 7, 14, 21 days of irradiation and stained them by the immunoperoxidase method. The results are summarized as follows. 1. A more significant decrease in the number of Ia-positive epidermal Langerhans cells was observed in single-dose-exposed group than fractionated-dose-exposed groups on the second day of irradiation. 2. On the seventh day of exposure, the number of the Ia-positive epidermal cells in single-dose-exposed group returned to normal, but in fractionated-dose-exposed groups the number of Langerhans cells decreased most remarkably. 3. In fractionated-dose-exposed groups, the number of Langerhans cells returned to normal on the 14th or 21st day of expposure. (Kor J Dermatol 29(4) : 459-465, 1991)

        항염제 및 연화제의 국소도포가 UVB 조사에 의한 홍반발생 및 색소침착에 미치는 영향

        In healthy Korean adult males, 6 hour-MED was 51 2+22.6mJ/cm,24 hour-MED was 67. 5+26. 7mJ/cm, and MMD was 86. 5 t21. 3mJ/cm for UVB irradiation respectively. UVB-MED and UVB-MMD was increased by desoxymethasone, hydrocortisone, and bufexama.c creams. Hydrophilic ointment base increased only 24 hour-MED. For 1 MED of UVB, all test agents inhibited erytherna for 48 hours, For 2 MEDs of UVB, desoxymethasone, bufexamac; and hydrocortisone creams inhibited erythema at 6 hours, while desoxymethasone and hydrophilic ointment base could suppress erythema at: 24. hours after irradiation. However, desoxymethasone was the most effective. Hydrophilic ointment base was efficacious only at 24 hours after irradiation. For 1 MMD of UVB, desoxymethasone, hydrocortisone, and bufexamac creams could reduced the delayed tanning (DT) reaction.

        건선 환자에서 PUVA 치료중 발생된 백반양 저색소증

        We observed a 51-year-old male, who developed depigmented patches on the lower abdomen after photochemotherapy(PUVA) of psoriasis without evidence of phototoxic reaction. The depigmented patches did not orginate from the sites of previous psoriatic lesions. Fontana-Masson stain of the biopsy specimen revealed nearly absent melanin pigments in the epidrmis of the hypopigmented patch, while lots of pigments were obsereved on the surrounding hyperpigmented area. Electron micrograph showed no melanocytes in the hypopigmented patch. Thus we report this case as having vitiliginous patches induced by photochemotherapy of psoriasis.

        We herein report a 28-year-old woman with typical marginal keratoderma of the palms of 4 years duration. She denied any chronic pressure or trauma. Histological findings disclosed marked hyperkeratosis, parakeratosis, acanthosis, elongation of rete ridges, edema and capillary dilatations of papillary dermis, and infiltration of chronic inflammatory cells. There was no changes in collagenous and elastic fibers. Treatment with oral 13-cis retinoid and topical application of 0.02% flumethasone ointment containing 3%, salicylate resulted in only transit effect.

        건선치료를 위한 Methotrexate 와 UVB 병용요법에 대한 연구

        We evaluated the treatment schedule of a 4 week course of methotrexate followed by UVB radiation therapy on 18 patients of generalized psoriasis. The patients received the weekly triple doses of methotrexa,te(MTX), 5 mg given every 12 hours for three doses on weekend for 3 weeks. On 4 th weekend, the doses was halved to 2. 5 mg every 12 hours for three doses, and then stopped. After a 4 week course of methotrexate, total doses 52. 5 mg, UVB treatment was commenced. The results are as follows: 1. 13 patients showed clearing of psoriatic involvolvement over 50, after 4 week course of MTX therapy. 2. Complete clearing of skin lesions was achieved in all 18 patients with the average,j, 8 times of UVB exposure in trunk and 5 6 times in extremities respect ively. 3. The average dose of UVB exposure at clearance was 187 mJ/cm in trunk lesion and 263. 6 mJ/cm in extremities, The mean cummulative dose of UVB exposure at clearance was 800. 7 mJ/cm in trunk lesion and 994. 3 mJ/cm in extremities. In conculuaion, the brief 4 week course of MTX allows for clearing of psoriasis at relatively low dose of UVR, and thus may reduce the long-term cumrnulative toxicity of both agents.

        C 57 BL 마우스에서 광화학 국소요법시 Trimethylpsoralen 농도가 색소 형성에 미치는 영향에 관한 연구

        Topical photochemotherapy is effective for the treatment of vitiligo, However the effectiveness of photosensitizer, 4,5', 8-trimethyl-psoralen is not well known. For thesie reasons, we evaluated the pigment producing effect of various concentrations of TMP in fifty-six male black mice doing photochemotherapy. 1) The number of melanocytes according to the concentrations of TMP: The melanocytes were not increased in the control group. However, melanocytes were increased, in three TMP applied groups in the order of 0.1% 0.3% and 0.5% group at 1,2,3 weeks, respectively but the difference of melanocyte counts in the TMP applied groups was not remarkable at 4 weeks. 2) The area of melanocytes in relation to the concentrations of TMP: The area significantly increased from the second week of UVA exposure in the control group. There was marked increase in the area of the melanocytes in the TMP applied groups compared with those of control group in the order of 0. l%, 0, 3% and 0. 5% respectively. 3) The circumference of the melanocytes in relation to the Concentrations of TMP: Circumference of the control groups increased in proportion to the frequency of UVA exposure. TMP applied groups showed significant increase of the circumference compared that of the control group and generally increased in proportion to the increase in the concentration of TMP.

        백반증 환자에서 항멜라닌세포 자가항체 활성도의 변화에 관한 연구 - 시험관내 보체매개 멜라닌세포 독성능의 측정 -

        Several observation suggest that the antimelanocyte autoantibodies could play a role in melanocyte destruction. Some experiments indicate that melanocyte antibodies from patients with vitiligo can kill melanocyte in vitro. In these experiments, we demonstrated that vitiligo patient's sera containing antimelanocyte antibodies can lyse cultured human melanocytes by complement activation. Melanocyte cytotoxicity was measured using the ethidium bromide/ acridine orange viability assay. Significant melanocyte cytotoxicity was seen in sera from patients with both active and inactive vitiligo(p<0.01). Melanocyte cytotoxicity measured with complement-mediated cytotoxicity decreased after systemic steroid treatment(p<0.05) ; however melanocyte cytotoxicity showed no significant change with systemic PUVA therapy.         건선의 자외선 B 치료 후 발생된 비전형적 색소침착 4예

        The ability of photochemotherapy(PUVA) or UVA phototherapy to stimulate melangenesis is well known. It is now recognized that the PUVA lentigines, irregular in outline and often more darkly and unevenly pigmented than solar lentigines, are related to the additional pigmentary effects of the PUVA therapy. We present herein four cases of unusual pigmentation after one month of UVB irradiation. Raticulated dark brown pigmentation appeared on the lower extremities of the patients. Histopathologicially, an increase in melanin in the basal layer was found, but no dysplasia or malignant change. We suggest UVB as the cause of this unusual hyperpigmentation.
